SCAN Health Plan (SCAN) is a non-profit health insurance plan founded in 1977 and headquartered in Long Beach, California. It mainly provides coverage for more than 285,000 health insurance beneficiaries. As SCAN expands into California, Arizona, Texas and Nevada, it has become one of the largest not-for-profit health insurance plans in the United States.
SCAN originated from a proposal by a team at the University of Southern California (USC). Its original name was Long Beach Geriatric Healthcare Council, Inc. and was later renamed SCAN. The agency's care delivery model focuses on assessing the needs of each senior in order to coordinate appropriate medical and social services based on each individual's unique circumstances.
In 1979, the California government selected SCAN as one of eight locations across the state to participate in the Multipurpose Senior Services Program (MSSP). The program is designed to help older Medi-Cal recipients avoid entering nursing facilities and provide home care services.
Before 1983, SCAN was not widely known outside of Long Beach, but its popularity began to grow with the establishment of the U.S. government's Social Health Maintenance Organization (SHMO) demonstration program. The purpose of the program is to expand SCAN's role in providing home health services, especially for high-risk older adults.
From 1996 to 1998, SCAN became a success story in doubling the number of members at that time by entering Orange County, Riverside County and San Bernardino County in Southern California. As the SHMO program ended, SCAN continued to operate as a health insurance plan, gradually expanding to Northern California and other states. In 2021, SCAN added more than 60,000 new members, mainly due to the "Embrace Venture" it provides, the launch of which has helped alleviate the financial burden faced by many seniors.

SCAN received a 5-star rating in 2022 due to their continued high-quality medical care in the plans they offer. SCAN's Independence at Home program focuses on care management for low-income seniors while building community resources to support healthy and independent living. This program provides connections to resources to help seniors get the medical and social support they need.
Since 2020, SCAN has launched a series of diversified programs under its parent company SCAN Group, aiming to provide evidence-based central care for all elderly patients. In 2021, SCAN launched new companies, such as Healthcare in Action, which focuses on the homeless, and Welcome Health, which provides remote and in-home visits, to further expand its service scope.
