Abstract


Dear Editor, We have perused with interest the article entitled ‘‘Treating Anisomastia and Tuberous Breast with Fat Grafting: Technique and Evaluation of Outcomes Using BREAST-Q Surveys’’ [1] in which Papadopoulos et al. presented a technique for the surgical correction of tuberous breast deformity employing autologous fat grafting. We would like to congratulate Papadopoulos and coworkers for their paper, and we would like to thank them for the citations. The described work is really interesting, and furthermore it highlights some points previously mentioned in our paper that deal with the classification of tuberous breast deformity [2]. In particular, as mentioned by the authors, we are convinced that one of the main features of tuberous breast deformity stands in the contraction of the mammary gland in either a horizontal or a vertical-horizontal plane. According to the different presentation and features of tuberous breast deformity, different surgical approaches can be adopted. As Papadopoulos et al. state, the unique use of autologous fat grafting is indicated only in very selected cases; on the other hand, we believe that the association of autologous fat grafting and round mammary implants allows to fulfill a natural result which is often desired by the patient [3]. Autologous fat grafting plays not only a fundamental role in aesthetic surgery but also in reconstructive field, thanks to the biological effects linked with this procedure [4]. Moreover, it is not simply the grafting of adipose tissue to promote the regenerative effect, but the combination with needletomy amplifies the benefits, allowing for a better definition of the lower breast pole and an easier correction of the tuberous breast deformity [5]. A multi-centric study aimed at establishing the best approach to treat tuberous breast deformity would be desirable.
