Antithrombotics/beta-adrenergic-receptor-antagonists/nitrates
Abstract
Lack of efficacy: case report A 65-year-old woman exhibited lack of efficacy during treatment with fondaparinux-sodium, unspecified antiplatelet therapy, βadrenergic-receptor-antagonists and nitrates for non-ST segment elevated myocardial infarction (NSTEMI) [routes and dosages not stated]. The woman was hypertensive and a smoker. She presented to the cardiology clinic after experiencing chest pain for an hour. Subsequent analyses revealed NSTEMI. She was transferred to the emergency room at hospital, where she received anticoagulant therapy with fondaparinux-sodium [fondaparinux] as well as unspecified antiplatelet therapy [dual-antiplatelet therapy], βadrenergic-receptor-antagonists [β-blockers] and nitrates along with oxygen; however, after 2 hours, her symptoms worsened (lack of efficacy). After subsequent investigations, it was found that her clinical picture evolved from NSTEMI to posterior antero-septal STEMI. She was also found to have occlusive thrombus in left anterior descending artery (LAD). She underwent percutaneous transluminal coronary angioplasty and drug-eluting stent placement, after which she achieved clinically favourable outcomes and improvement in her laboratory investigations.
