Phenobarbital

Abstract

Takotsubo Syndrome: case report A 58-year-old man developed Takotsubo syndrome during treatment with phenobarbital. The man, who had bipolar syndrome, was hospitalised for phenobarbital treatment [route and dosage not stated]. His medical history was significant for monovasal coronary artery disease. During the hospital stay, he developed dyspnoea and angor. A subsequent ECG demonstrated negative T-waves in V2-V3 with a concomitant increase in the indexes of myocardial necrosis. Due to suspected non-ST segment elevation myocardial infarction, he underwent coronary angiography. Which did not showed coronary stenosis. However, his ventriculography and echocardiography showed hypokinesia of the basal segments with hyperkinesia of the mid-apical segments. He was thus diagnosed with Takotsubo syndrome, which was determined to be related to phenobarbital [time to reaction onset not stated]. Therefore, the man received treatment with ACE inhibitors and beta-blockers. Consequently, his condition improved.
