Volvulus of the Stomach and Small Bowel

Abstract

Abstract Volvulus is a clinical entity wherein there is a greater than 180-degree twisting of a hollow organ about its mesentery. This causes luminal obstruction, impaired venous return, and eventually ischemia and perforation. Although much less common than colonic volvulus, small bowel volvulus and gastric volvulus (GV) are clinical problems that, when not recognized promptly, can lead to necrosis of the involved organ with resultant high morbidity and mortality. Volvulus of the small bowel is more common in children, is most often secondary to malrotation, and is the cause of 1% of adult small bowel obstructions in the United States. Primary GV is rare and results from disruption or laxity of the stomach s supporting ligaments. Secondary GV is more common and is usually the result of a paraesophageal hernia.
