Revision Functional Endoscopic Sinus Surgery: Completion Sphenoethmoidectomy
Abstract
Abstract Chronic rhinosinusitis, particularly in the setting of nasal polyposis, is unfortunately associated with significant need for revision surgery following initial functional endoscopic sinus surgery and failed medical management. Goals of these revision procedures include removal of recurrent inflammatory disease, scar tissue, and residual bony lamellae, such that the final postsurgical anatomy reflects comprehensive opening of the sinonasal tract, including completion of a contiguous sphenoethmoid cavity. The present chapter illustrates methodology to surgically revise, or complete, functional endoscopic sinus surgery addressing the ethmoid and sphenoid sinuses. This will be discussed in the context of preoperative planning, necessary instrumentation, specific procedural steps, and postoperative management.
