Highlights • This study suggests a PQM methodology for breast cancer radiotherapy evaluation.• The risk/benefit balance estimation includes tumor biology and smoking status.• Smoking status influenced risk/benefit balance for different treatment techniques.• Survival benefit dominated for all patients with high-risk breast cancer.• Survival benefit for smokers with low- or intermediate- risk cancer was not seen.
