Cavitary pulmonary nodules tend to present in patients with sero-positive RA with a long standing disease and can give rise to serious pulmonary manifestations. Pulmonary mycetoma can occur in any pre-existing pulmonary cavities and usual predisposing conditions are tuberculosis, lung abscess, bronchiectasis, diabetes and malignancies. Fungal colonization is a very rare complication of rheumatoid nodules with cavitation and clinical as well as radiological evaluation is important. Treatment depends on extension of the disease and drug administration along with surgical intervention should be considered whenever possible.
