Retroperitoneal Lymphatic Cyst Mimicking Appendicular Mucocele and Lymphangioma

Abstract

Chronic pain in right lower abdominal quadrant is a common entity in young adults in surgical cases. We report a case of 16\xa0years old female who presented with complaints of pain in right lower quadrant and high grade fever. CECT (Contrast Enhanced Computed Tomography) abdomen suggested the possibility of giant mucocele of appendix and other cystic lesions such as retroperitoneal lymphangioma. USG (ultrasonography) suggested possibility of lymphangioma. Exploratory laparotomy was done suspecting an appendicular mucocele, which revealed a large retroperitoneal cyst and a normal appendix. Retroperitoneal cyst was extending from retroperitoneal part of the duodenum, kidney and lower margin of liver up to caecum and pelvis and reaching up to midline. The case is being reported to highlight large retroperitoneal cyst as one of the differential diagnosis in right lower abdomen pain although laparotomy would aid in the final diagnosis.
