Therapeutic value of decompressive craniotomy with standard bone flap for severe craniocerebral injury

Abstract

Objective \nTo analyze the clinical effects of decompressive craniotomy with standard bone flap for severe craniocerebral injury. \n \n \nMethods \nA total of 114 patients with severe craniocerebral injury diagnosed and treated in Shouguang People’s Hospital from January 2016 to December 2017 were selected as research objects. Patients were divided into observation group and control group by random number method, with 57 cases in each group. Patients in the control group were treated by traditional craniotomy, and patients in the observation group were treated by decompressive craniotomy with standard bone flap. The intracranial pressure was compared between the two groups before and after treatment, and the postoperative Glasgow outcome scale score and incidence of complications were compared between the two groups. \n \n \nResults \nThe intracranial pressures in the two groups were high before operation (P>0.05), and the intracranial pressure in the observation group was significantly lower than that in the control group within 7 days after operation (P<0.05). The incidence of vagetative survival and case fatality rate in the observation group were lower than those in the control group during the 12-month follow-up, while the recovery rate was significantly higher than that in the control group (P<0.05). The postoperative complication rate in the observation group was 3.51%(2/57), significantly lower than the 19.30%(11/57) in the control group (P<0.05). \n \n \nConclusions \nDecompressive craniotomy with standard bone flap for patients with severe craniocerebral injury can achieve satisfactory results. And it can help to reduce the risk in complications and improve the prognosis. This surgical protocol is worthy of clinical application and popularization. \n \n \nKey words: \nSevere craniocerebral injury;\xa0Decompression craniotomy with standard bone flap;\xa0Prognosis;\xa0Complications
