The diabetics impairment of the red cell filterability is all the more important if the diabetic subject is suffering from an acute and progressive lower limb arteriopathy. Red cell 2-3 diphosphoglycerate (2-3 DPG) increases when red cell filterability decreases among these patients. Pentoxifylline (i.v. 600 mg per day) produces a rapid initial improvement of red cell filterability and then increases red cell 2-3 DPG level.

Two diabetic populations were determined, one for those under 30 years of age who had no complications and had been insulin-treated for less than 5 years (I), the other for those over 50 who had complications and insulin treatment for more than 5 years (II). In both groups, whole blood filtration (WBF) was tested on Nucleopore 5μm filters (Reid et al., 1976), and current factors known to influence WBF were also tested. In addition, erythrocyte 2–3 diphosphoglycer-ate (2–3 DPG) was measured. WBF was statistically increased in the group I (P<0.02).

In twenty control subjects, twenty diabetic patients without macro-angiopathy and forty diabetic patients with an acute and severe arteriopathy of lower limbs, we studied the red cell filterability (TFE) on total blood, the 2-3 diphosphoglycerate level (2-3 DPG) and the Adenosine Triphosphate level (ATP). The diabetics impairement of the red cell filterability and ATP level are all the more important than the subject is suffering from an acute arteriopathy of lower limbs. For the same subjects, we notice that the 2-3 DPG level increases significantly. The filterability is related to the ATP red cell concentration. An ATP depletion produces an increase membrane rigidity and a minor filterability. These findings strongly suggest a relation that can exist between the red cell metabolic state and filterability in diabetic population with or without macroangiopathy.

28 Patienten mit pAVK wurden vor und nach Behandlung mit Ginkgobiloba-Extrakt 761 (Rokan) untersucht. Sie bildeten bezuglich Alter, Geschlechtsverteilung, Stadium (III oder IV) und distaler Lokalisation der Erkrankung einen reprasentativen Querschnitt durch eine Population von 300 Diabetikern, die zuvor untersucht worden waren. Die Beurteilungskriterien fur den Krankheitsverlauf unter Behandlung waren: Plethysmographie, segmentaler Perfusionsdruck in verschiedenen Hohen, Oszillogramm und reaktive Hyperamie. Die klinischen Ergebnisse waren besonders in Hinsicht auf die Ruheschmerzen und die trophischen Storungen gut. Die gemessenen Parameter zeigten vor allem eine Verbesserung der vaskularen Compliance (Oszillogramm) und des systolischen Knochel-Index. Rokan wirkt im wesentlichen uber eine Verbesserung der hamorheologischen Parameter sowie uber eine Verminderung der peripheren Widerstande und begunstigt damit eine bessere Blutverteilung in den distalen Gefasbezirken.

The most recent French studies on vitamins and public health have identified folates as one of the primary high-risk vitamins, that is, for which there is the greatest risk of deficiency. While we have only recently become aware of this situation, it is actually not new. As in other industrialized countries, changes in life-style and dietary habits in France have been particularly rapid, and our current diet is very different from traditional models in terms of composition, scheduling, distribution, etc. These dietary changes and the transformation in our life-style have had an influence not only on the composition of meals but perhaps also upon nutritional requirements themselves.
