Fatal toxic epidermal necrolysis in a patient on teriflunomide treatment for relapsing multiple sclerosis.

Gaspard Gerschenfeld; A. Servy; Laurence Valeyrie-Allanore; Nicolas de Prost; Jérôme Cecchini

We report a case of toxic epidermal necrolysis in a 46-year-old woman on teriflunomide treatment. Such a severe adverse cutaneous drug reaction with this new therapy for relapsing forms of multiple sclerosis should be early recognized in order to ensure the rapid withdrawal of the drug.

Skin biopsy polymerase chain reaction for rapid microbiological diagnosis in patients with purpura fulminans

Purpura fulminans (PF) is mostly caused by Neisseria meningitidis or Streptococcus pneumoniae. Prompt identification of the causative bacteria is essential, but cerebrospinal fluid (CSF) and blood cultures may lack sensitivity, mostly because of prior antibiotic administration, and be time-delayed. A skin biopsy of a purpuric lesion with N. meningitidis polymerase chain reaction (PCR) is now recommended by French guidelines, still, the usefulness of this tool remains debated in some countries, owing to the lack of high-level evidence. We used two non-invasive diagnostic tests, as our study is the first to analyse the prevalence of HP infection in VLS. As a pilot study, we chose inexpensive, rapid and acceptable tests, with high diagnostic accuracy [8-10]. A limitation of this study is that we did not perform a vulvar biopsy in controls, thus our two cohorts did not have identical surveillance for the disease. Moreover, our population was composed of Caucasian women and the results may not be generalized to different ethnic groups. Hence, further research based on isolation in culture, PCR, and molecular analyses is warranted to analyse the importance of different ethnicity and genetic predisposition, as well as environmental factors, on the pathogenesis of VLS among different populations.

Self-diagnosed drug allergies: the belief of patients

Arnaud Jannic; A. Servy; Xavier Chevalier; A. Colin; Olivier Chosidow; Saskia Ingen-Housz-Oro; Pierre Wolkenstein

The term of “drug allergy” (DA) may include true hypersensitivity or non-allergic reactions. The rate of self-reporting DA ranges from 4.7% to 11% regardless of age. When patients report a DA, the medical team often decides a full and definitive elimination of the incriminated drug from future therapy, which deeply affects medical care.

A. Servy; Laurence Valeyrie-Allanore; F. Alla; X. Duval; Olivier Chosidow
