Lichen planopilaris: A clinicopathologic study

Muna Matta; Abdul-Ghani Kibbi; Joe Khattar; Salah Salman; Shukrallah Zaynoun

Three clinicopathologic variants of lichen planopilaris are described. The first is characterized clinically by individual keratotic follicular papules and histologically by a lichenoid inflammatory cell infiltrate confined to the follicular epithelium. The second variant consists of erythematous to violaceous plaques, some of which show follicular prominence; the histologic appearance is that of a lichenoid inflammatory cell infiltrate that affects both follicular and interfollicular areas. The third variant manifests as follicular papules of the scalp with concomitant or subsequent cicatricial alopecia. In this variant the histologic hallmark is a lichenoid, follicular and interfollicular inflammation, associated with or followed by scarring. Overlap among the three variants exists, and hence the concept of a disease spectrum ranging from pure follicular involvement without evidence of clinical scarring to cicatricial alopecia of the scalp is advocated.

Mucocutaneous pseudoepitheliomatous hyperplasia: a review.

Jinane El-Khoury; Abdul-Ghani Kibbi; Ossama Abbas

Abstract Pseudoepitheliomatous hyperplasia, also called pseudocarcinomatous hyperplasia because of its resemblance to well-differentiated squamous cell carcinoma, is a reactive epithelial proliferation that is characterized by prominent irregular hyperplasia of the epithelium with tongue-like epithelial projections into the dermis. This reactive pattern may involve cutaneous or mucosal surfaces and has been described in association with a wide variety of stimuli including infectious, neoplastic, inflammatory, and traumatic among others. In this review, we will discuss the pathophysiology of pseudoepitheliomatous hyperplasia, the histopathological findings that are helpful in its differentiation from squamous cell carcinoma, and the spectrum of conditions that may show this unique feature on microscopic examination.

Cutaneous metastasis: clinicopathological study of 72 patients from a tertiary care center in Lebanon

Backgroundu2002 Cutaneous metastasis is the result of malignant cell spread from primary malignancy to the skin. This is not uncommon, and rates reported in the literature are as high as 10.4%. To the best of our knowledge, there are no studies assessing the epidemiologic, clinical, and histopathological features of cutaneous metastasis in our region.

Lina F. Kanj; Nelly Rubeiz; Adnan M. Mroueh; Abdul-Ghani Kibbi

Vulvar melanosis and/or lentiginosis (VM/L) is a cause of concern for patients and clinicians, t3 These lesions may be single or multiple and are characterized by large size, irregular borders, and variegated pigmentary patterns: Despite their clinical appearance, the histologic features are benign. t, 3-8 Little is known about the biologic behavior of VM/L. However, during the past few years 17 cases have been reported; malignant melanoma has not occurred in any of these. We report the case of a woman with multiple pigmented macules on her genitalia.

Prurigo pigmentosa: a clinicopathologic study of 4 cases from the middle East.

Abstract:Prurigo pigmentosa (PP) is a rare inflammatory dermatosis originally reported in Japan. Since then, most reports have originated from Asia, and to a lesser extent from Europe. Although the pathogenesis remains unclear, it is now established that PP is linked to ketoacidotic states. Four patients diagnosed with PP were identified from the dermatopathology database at the American University of Beirut Medical Center between January 2009 and December 2013. Clinicopathologic findings in the 4 patients were similar to those previously reported in the literature. The patients were all female with a mean age of 23.5 years. They all presented with itchy erythematous reticulated papulovesicles/plaques leaving variable reticulated brownish patches. Two patients had, in addition, annular lesions arranged en cocarde and pustules, respectively. In 3 patients, the rash was associated with fasting or dieting. The rash had a predilection to the trunk and proximal part of the upper extremities. One patient had intergluteal area involvement. Two biopsy specimens revealed psoriasiform hyperplasia and neutrophilic exocytosis mimicking psoriasis or an impetiginized spongiotic dermatitis. One biopsy specimen exhibited a mild superficial perivascular lymphocytic infiltrate with ballooning and reticular degeneration, a picture mimicking a viral exanthema. Another biopsy specimen exhibited a picture similar to chronic spongiotic dermatitis. Although mostly described in Japan, PP has been described much less frequently in the Middle East region likely due to mis/underdiagnosis. Therefore, increased awareness is necessary especially because fasting is a common religious practice among Arab countries. Further investigations are necessary to better understand the etiopathogenesis of this rare entity.
