Analysis of the Multi-Item Dimensionality of Patients' Perceived Value in Hospital Services

The purpose of this research is to analyse the multi-item dimensionality of patients’ perceived value in hospital service. A total of 564 patients administered to Gulhane Military Medical Academy were surveyed. Sample population was specified through convenience sampling procedure. A face to face survey was conducted by three interviewers in August 2008. Confirmatory Data Analysis was carried out to reach the final data analysis. A scale of overall perceived value in health service was developed, which was composed of three dimensions and represented by 21 items. These dimensions were: Functional value, emotional value, and social value. The results indicated that perceived value was a multidimensional construct that affects the whole process of service purchasing. If the hospitals managers give importance to the sub-dimensions of the patient value inclined to their own hospitals and they regularly examine the situation of these dimensions, they can move one step ahead in achieving patient satisfaction and loyalty.

Military Medical Graduates' Perceptions of Organizational Culture in Turkish Military Medical School

Organizational culture is the term used to describe the shared beliefs, perceptions, and expectations of individuals in organizations. In the healthcare environment, organizational culture has been associated with several elements of organizational experience that contribute to quality, such as nursing care, job satisfaction, and patient safety. A range of tools have been designed to measure organizational culture and applied in industrial, educational, and health care settings. This study has been conducted to investigate the perceptions of military medical graduates on organizational culture at Gülhane Military Medical School. A measurement of organizational culture, which was developed by the researchers from Akdeniz University, was applied to all military medical graduates in 2004. This was a Likert type scale that included 31 items. Designers of the measurement grouped all these items into five main dimensions in their previous study. The items were scored on a five-point scale anchored by 1: strongly agree and 5: strongly disagree. Study participants included all military physicians who were in clerkship training period at Gulhane Military Medical Academy in 2004. A total of 106 graduates were accepted to response the questionnaire. The mean age of participants was 25.2 ± 1.1. At the time of study only 8 (7.5%) graduates were married. The study results have showed that the measurement tool with 31 items had a sufficient reliability with a Cronbachs alpha value of 0.91. Factor analysis has resulted a final measurement tool of 24 items with five factors. Total score and the scores of five subdimensions have been estimated and compared between groups based on living city and marital status. The study has shown the dimension of symbol received positive perceptions while the dimension of organizational structure and efficiency received the most negative perceptions. GMMS has a unique organizational culture with its weak and strong aspects. Conducting this kind of studies contribute to improve organizational culture in order to increase educational and research capability.

The Purpose of this study is to investigate to correlation among customer satisfaction, customer commitment, and loneliness. The population of the research involves totally 103 person entitled residing in TAF Ankara Special Care Center. “Person Entitled Satisfaction” were measured by adapting items from Çimen et al. Commitment level of person entitled were measured by adapting items from Erdem et al. The information concerning the loneliness level of person entitled was collected by using UCLA Loneliness Scale. Bu calismanin temel amaci; narsistik kisilik egilimleri ile kurumsal baglilik arasinda bir iliski olup olmadiginin belirlenmesidir. Calisma uc ana bolumden olusmaktadir. Birinci bolumde; narsizm kavrami, ikinci bolumde; kurumsal baglilik kavrami ele alinmistir. Ucuncu bolumde ise belirtilen iki kavram arasindaki iliski uzerine gorgul bir arastirma yapilmis ve analiz sonuclarina yer verilmistir. Anket 2011 yili Ocak ayinda Ankara ilinde Gulhane Askeri Tip Akademisi’nde (GATA) ogrenim goren 543 ogrenci uzerinde uygulanmistir.

The Effect of Socio-Cultural Characteristics on the Effectiveness of Teamwork: A Study in the Gülhane Military Medical Faculty Training Hospital

In this study, our aim was to determine the effect of the socio-cultural characteristics of health workers on the effectiveness of teamwork. In this study, a questionnaire method was used for data collection. To this end, a scale was first developed to assess the effectiveness of teamwork. Setting and Participants: The study was conducted in 34 departments/divisions within the GMMF Training Hospital with 423 health workers. “Specialist opinion” was used to determine the content validity of the “Teamwork Effectiveness Scale” developed for this study, while “factor analysis” was used to test the scale’s construct validity. Cronbach Alpha values were calculated to test the reliability of the scale. To determine the effect of socio-cultural characteristics on the effectiveness of teamwork, the “Kruskal-Wallis” test, the “Mann-Whitney U” test and “Logistic Regression Analysis” were used within the context of the study. Based on the study results, it was observed that “assignment, “age” and “status” did not have an effect on the effectiveness of teamwork (p > 0.05). On the other hand, a significant and negative relationship was observed between “the obligation to perform compulsory service” and the attitudes that considered teamwork to be effective (p = 0.029). Similarly, a difference was identified between the workers’ attitudes towards the effectiveness of teamwork depending on the size of the place of assignment (p = 0,042). It was thus observed that the “effectiveness of teamwork” was affected by the presence or absence of the “obligation to perform compulsory service” and by the “size of the place of assignment.

An Assessment of Physician’ Attitudes and Behaviors on Cost-Effectiveness During Medical Treatments -

Purpose: The purpose of this study is to reveal attitudes and behaviors of physicians with regard to cost effectiveness with an aim to provide efficient use of economic resources consumed for delivery of health care services. Methodology: The study was conducted at Gulhane Military Medical Academy through February-March 2010. For the purposes of the study, the scale titled “Cost Effectiveness as a Criterion for Medical and Coverage Decisions” issued by “Visible Fairness Partner Organizations” in 2001 was taken as basis and the questionnaire form developed considering the specific characteristics of Turkish Armed Forces was implemented. It was aimed in the study to reach 206 physicians commissioned at internal and surgical departments assumed to be in direct contact with the patients by using the simple layered random sampling method. Among the participants in the study, however, data from 191 physicians who filled out the questionnaire completely was taken into evaluation. Findings: The study concluded a significant difference (Z=2,369, p=0,018) between “Patient Related Factors” and “Administration Background of Physician” and a significant difference (Z=2,096, p=0,036) between “Physician Related Factors” and “Term of Office of Physician” and a significant difference (X2=6,825, p=0,033) between “Physician Related Factors” and “Status”. Conclusion: Findings from the study revealed a high level of perception in physician related factors. Basing on this conclusion, it is considered to be helpful to include certain topics such as health care services administration and organization, health economy and health care service financing especially in pre-graduation curriculum for the physicians for more efficient and effective use of limited resources.
