Distribution of injuries among patients of Department of Trauma and Emergency Medicine of Independent Public Teaching Hospital No. 1 in 2011

Abstract Introduction. Injuries are an extremely important public health problem, in both developed and developing countries. Various traumas contribute to some one third of deaths of the entire human population followed by cardiovascular diseases and cancer only. Understanding the epidemiology of trauma might improve hospitals’ ability to provide proper diagnoses and treatment. Aim. The aim of this study was to provide epidemiological characteristics of injuries in the analyzed group of patients. Material and methods. A retrospective analysis of patients admitted in state of emergency and hospitalized in the Department of Trauma Surgery and Emergency Medicine at the Medical University of Lublin due to injuries that occurred from January 1st 2011 till December 31st 2011 was conducted. Mechanism of injury, mortality rate, age and gender correlations were studied. Results. Some 485 patients were looked at in the study. The mortality rate reached 4.52% and was highest among males. Falling was the leading cause of injuries among patients Discussion. The results of this study correspond with the observations of other authors concerning the age distribution. Mortality rates noticed in this study were lower than in other studies. Conclusions. Trauma in elderly patients is a growing problem in trauma centers. Moreover, in the analyzed material incidence of blunt and non-transport related injuries increased.

Prognostic factors in traumatic brain injury and their association with outcome

Introduction. Traumatic brain injuries (TBI) are a real social problem, with an upward trend worldwide. TBI is the leading cause of death and disability, especially among young men. Each year in Europe, and also in Poland, 243 per 100,000 individuals suffer from TBI. Assessing prognosis after traumatic brain injury is very important in order to help clinicians to make a decision about the implementation of specific methods of treatment, and to make communication with the patient and the patient’s family easier. Objective. The objective of this review was to present prognostic factors, to assess outcomes within a short time after a moderate to severe traumatic brain injury, as well as to predict functional outcome. The most important independent variables were: age, Glasgow Coma Scale (GCS) motor score, pupil response, Marshall CT classification and traumatic subarachnoid haemorrhage. Other important prognostic factors included hypotension, hypoxia, glucose, coagulopathy, haemoglobin and category of CT characteristic, such as midline shift, mass lesion, basal cistern. Conclusions. Gender and intraventricular haemorrhage did not have predictive value. This subject needs much more research in the area of new prognostic factors which would be better associated with outcome after traumatic brain injury.

Polymorphisms in the promoter region of the CRBN gene as a predictive factor for the first-line CTD therapy in multiple myeloma patients

Cereblon is a primary molecular target for immunomodulatory drugs. The aim of this study was to evaluate the influence of selected clinical and molecular factors including single nucleotide polymorphisms (SNPs) in CRBN gene on the efficacy of first line CTD (cyclophosphamide, thalidomide, dexamethasone) chemotherapy in patients with multiple myeloma. Study group consisted of 68 patients. Analysis of CRBN gene SNPs (rs6768972, rs1672753) was performed using Real-Time PCR genotyping technique. Median progression free survival (PFS) was 15 months and overall survival (OS) 79 months. Factors associated with significantly shorter OS included ISS 3, kidney disease, weight loss, anemia, thrombocytopenia, hypoalbuminemia, elevated β2-microglobuline and CRP. The presence of t(4;14) was associated with significantly shorter PFS and OS. Both examined SNPs proved to be statistically significant, independent predictive factors of efficacy of the CTD chemotherapy. The presence of AA genotype (rs6768972) correlated with longer median PFS (18 vs 9 months; HR=0.49,95% CI: 0.26-0.91, p=0.0062). Conversely, in the carriers of CC genotype (rs1672753) significantly shorter median PFS was observed (4 vs 16 months; HR=3.93, 95% CI: 0.26-59.64, p=0.0321). In conclusion, SNPs of the CRBN gene may be useful in qualifying patients for treatment with regimens containing thalidomide.

Behavior of new hydroxyapatite/glucan composite in human serum

Biomaterials for bone tissue regeneration, including polymer-based composites, are typically evaluated in vitro prior to the clinical trials. However, such composites tested in vivo may behave different due to the specific body conditions. For example, some composites implanted into the tissue acidified due to transient postoperative inflammation may unexpectedly swell which delays the wound healing. Such massive swelling in acidic medium was previously observed for new elastic hydroxyapatite (HAp)/β-glucan biomaterial. However, in further clinical cases concerning the composite implantation in patients without significant inflammation indicators, no side effects were observed. Therefore, it was reasonable to test the effect of human serum of neutral pH (typical for noninflamed tissues) on the composite parameters, in particular volume changes. Thus, this article shows the characterization of physicochemical parameters of the composite after incubation (5 days) in human serum of neutral pH by means of weight and volume measurement, scanning electron microscopy, X-ray diffraction, Fourier-transform infrared spectroscopy, microcomputed tomography, mercury intrusion, and biochemical techniques. Results showed that human serum collected from healthy people caused no uncontrolled changes in weight and volume, porosity and mechanical properties of the composite. Therefore, this suggests the lack of volume change-related side effects of HAp/glucan composite in bone defects treatment if postoperative inflammation is prevented.

Assessment of the efficacy of ofatumumab in patients with chronic lymphocytic leukaemia treated in the Department of Haematooncology and Bone Marrow Transplantation of the Medical University in Lublin – Prelimary results

INTRODUCTION Despite significant recent advances in the treatment of chronic lymphocytic leukaemia (CLL), most cases of the disease are still incurable. Treatment with monoclonal antibodies, such as ofatumumab, is one of the new therapeutic options. OBJECTIVE Retrospective analysis of the efficacy of ofatumumab in patients with chronic lymphocytic leukaemia (CLL) treated in the Haematooncology and Bone Marrow Transplantation Department of the Medical University of Lublin, Poland, during 2011-2013. MATERIAL AND METHODS The analysis included 5 patients (3 women and 2 men), aged 47-65, with Rai stage II-IV CLL, after a few lines of treatment. Three patients received ofatumumab in monotherapy and 2 patients received ofatumumab in combination with cyclophosphamide (50 mg/day) and dexamethasone (40 mg/day). All patients included in the study were diagnosed with an active form of leukaemia with symptoms such as lymphocytosis or massive lymphadenopathy. RESULTS All patients responded to the treatment. Within the first 8 weeks of the treatment, levels of white blood cells returned to normal in patients with baseline lymphocytosis (3 patients). An increase in platelet levels was reported in 3 patients. Haemoglobin levels were higher or comparable to the baseline values in all studied patients after the completion of immunotherapy. In the patient with massive lymphadenopathy and hepato- and splenomegaly, the size of the lymph nodes, spleen and liver decreased and neutrophil levels increased. Time of progression was 5-12 months, and in one patient partial remission has been maintained. The treatment was well-tolerated in most cases. Asymptomatic neutropenia and an infection with Candida glabrata were observed. CONCLUSIONS Ofatumumab may be a new and safe therapeutic option for patients with CLL after a few lines of treatment.

History of trauma and posttraumatic disorders in literature

In 1980 a third edition of Diagnostic and Statistical Manual of Mental Disorders (DSM) brought diagnostic criteria for a new diagnosis - posttraumatic stress disorder (PTSD). This disorder is a result of highly intensive stressor and in many cases leads to sever psychiatric distress. Despite relatively recent introduction of PTSD as a new diagnosis, this disorder was excessively described in scientific papers as well as in fiction novels. Analysis of those descriptions across ages allows for the conclusion that character and type of stressors has changed, however, peoples reactions to highly intensive stressors are basically similar. First descriptions are found in notes of Egyptian physicians and then in papers of Homer, Herodotus and Plutarch. In consecutive parts of this paper, the authors present history of posttraumatic stress disorder describing contribution of Polish authors - Kepiński and Szymusik. Presented historical perspective of posttraumatic stress disorder allows for better understanding of reasons for introducing PTSD into classifications as well as controversies related to it.

Evaluation of Early Surgical Treatment Results Considering Venous Leg Ulcers

UNLABELLED Lower leg ulcers are characterized by a chronic and recurrent course, being considered as a difficult therapeutic problem. Based on epidemiological data chronic ulcers concern 0.2 to 2% of the Western European population. The above-mentioned problem affects women more often than men, and increases with age. Treatment and recurrence prevention methods remain under discussion and research. The aim of the study was to evaluate the early surgical treatment results of venous leg ulcers at the Department of Traumatology and Emergency Medicine, Medical University of Lublin. MATERIAL AND METHODS We evaluated patients hospitalized at the Department of Traumatology and Emergency Medicine, Medical University of Lublin during the period between 2009-2014. Inclusion criteria were as follows: venous leg ulceration, full documentation. The study group comprised 62 patients subjected to treatment for leg ulcerations, including 35 women and 27 men, aged between 30 and 94 years. The treatment method included the excision of the ulcers with the deep fascia, and the implantation of the mesh graft (thickness of skin) directly into the exposed muscle. RESULTS Amongst patients evaluated at discharge, the healing of the transplant was as follows: in case of 44 patients 90-100%, in 8 patients - 75-89%, in 5 patients 50-74% and in 5 patients less than 50%. The predominating bacteria collected from ulceration samples was Pseudomonas aeruginosa (32%) and Staphylococcus aureus (35%). CONCLUSIONS The patient outcome shows that surgery is an effective method considering treatment of venous leg ulcers. Earlier qualification for this type of surgery will improve patient outcome.

Modern methods of treatment of post-traumatic bone defects in Department of Trauma Surgery and Emergency Medicine, Medical University of Lublin

Abstract Motor organ injuries are one of the most common consequences of trauma, out of which comminuted fractures with a bone loss are the most challenging. Numerous methods are applied to treat these injuries; however, still in many cases we are unable to suggest successful medical treatment. Therefore, treating these injuries using elastic and surgically handleable bone replacement materials was started at Trauma Surgery and Emergency Medicine of Medical University of Lublin (provided for the experiments by Medical Inventi Lt). Preliminary assessment based on the results of treatment with bone replacement materials of two patients with comminuted fractures of femur was promising. Bone union was achieved without any adverse effects.

Rules, indications and documentation for applying physical coercion by emergency medical services

Abstract Helping people with mental disorders poses a challenge to the members of medical emergency services (EMS). Psychiatric patients are often unpredictable and applying physical coercion is necessary in some cases. The aim of this paper was to present and comment on legal foundations of application of different forms of physical coercion by EMS members and describe how to fill out medical records required every time physical coercion was used. According to the amendments of Polish Mental Health Act made in 2010, the EMS members were granted the right to apply physical coercion. Further amendments to the Mental Health Act and the introduction of appropriate Ministry of Health decree define forms of physical coercion, indications to apply physical coercion and include a sample of proper medical records which are required in all cases of application of physical coercion. Application of physical coercion should always be treated as last-line treatment option while helping patients suffering from mental disturbances. Obeying the law every time a decision regarding physical coercion is made protects patients’ right to receive dignified care and treatment as well as the rights of medical professionals

Efekt tlenoterapii hiperbarycznej u chorej na ziarniniaka Wegenera - opis przypadku

Streszczenie Ziarniniak Wegenera jest chorobą autoimmunologiczną, charakteryzującą się martwiczym i ziarniniakowym zapaleniem małych naczyń. Schorzenie to dotyczy górnych i dolnych dróg oddechowych, nerek, gałki ocznej. W 50% przypadków choroba zajmuje skórę, prowadząc do powstawania trudno gojących się owrzodzeń. W poniższej pracy przedstawiamy przypadek 23-letniej pacjentki, u której rozpoznano ziarniniaka Wegenera z zajęciem skóry małżowiny usznej prawej, z naciekiem na śluzówce jamy ustnej i asymetrią twarzy. W leczeniu, obok pulsów sterydowych oraz leczenia immunosupresyjnego, stosowano tlenoterapię hiperbaryczną, uzyskując bardzo dobry efekt terapeutyczny oraz całkowite ustąpienie zmian skórnych.
