Effect of tuberculosis treatment on leptin levels, weight gain, and percentage body fat in Indonesian children

Purpose Tuberculosis (TB) remains a problem in the community. TB patients usually experience malnutrition, which is characterized by both decreased body weight (BW) and body fat percentage (BFP). Leptin, an important regulator of BW, also plays an important role in cellular immunity, which is integral to defense against Mycobacterium tuberculosis infection. We analyzed the effect of an anti-TB treatment regimen on the leptin level, BW, and BFP of children with TB. Methods The design of this study was a group interrupted time series. The subjects were children with probable TB according to clinical criteria based on an Indonesian scoring system adopted from the Consensus of Expert Panel. BW; BFP; energy intake; fat and protein intake; and leptin levels before, 2 months after (intensive phase), and 6 months after (continuation phase) anti-TB treatment, were measured. About 40 children, aged 5–14 years, participated in this study. Results The BW, BFP and leptin level increased from before treatment to after completion of the intensive phase and still showed an increased during the continuation phase: BW 18.65 kg, 19.75 kg, and 20.85 kg; BFP 18.3%, 19.5%, and 20.2%; and leptin level 1.9 mg/dL, 3.07 mg/dL, and 3.4 mg/dL, respectively (P<0.01). Conclusion Leptin level, BW, and BFP increased throughout the course of anti-TB treatment, compared with pretreatment values. Further research is needed to compare the results with data for healthy children.

Low zinc serum levels and high blood lead levels among school-age children in coastal area

The coverage of environmental lead toxicant was quiet wide. Lead exposure recently has been expected to be associated with zinc deficiency and blood indices disturbance. Emphasizing on children, which could absorb more than 50 % of lead that enters the body. Lead became the issue on the coastal area due to it has polluted the environment and waters as the source of fisheries products. This was a cross sectional study to determined nutritional status, blood lead levels, zinc serum levels, blood indices levels, fish intake among school children in coastal region of Semarang. This study was carried out on the school children aged between 8 and 12 years old in coastal region of Semarang. Nutritional status was figured out using anthropometry measurement. Blood lead and zinc serum levels were analyzed using the Atomic Absorbent Spectrophotometry (AAS) at a wavelength of 213.9 nm for zinc serum and 283.3 nm for blood lead. Blood indices was measured using auto blood hematology analyzer. Fish intake was assessed using 3-non consecutive days 24-hours food recall. The children had high lead levels (median 34.86 μg/dl, range 11.46 - 58.86 μg/dl) compared to WHO cut off. Zinc serum levels was low (median 18.10 μg/dl, range 10.25 – 41.39 μg/dl) compared to the Joint WHO/UNICEF/IAEA/IZiNCG cut off. Approximately 26.4% of children were anemic. This study concluded that all school children had high blood lead levels, low zinc serum, and presented microcytic hypochromic anemia. This phenomenon should be considered as public health concern.

ASUPAN SENG, KADAR SERUM SENG, DAN STUNTING PADA ANAK SEKOLAH DI PESISIR SEMARANG

ABSTRACT The aim of this study was to analyze difference and compare the determinants of stunting among under five years children considering different ecosystem in Kupang. It is an observational study using case control of under five children with and without stunting. There were 132 subjects chosen from lowland, middleland region, and mountainous ecosystem zones. Data was analyzed using Multiple Logistic Regression Analysis. The analysis resumes that determinants of stunting in lowland area is energy intake (p=0.002; OR=0.059; 95%CI:0.010-0.359), energy intake is a protective factor, in the middleland region is love and cares (p=0.002; OR=9.247; 95%CI:2.213-38.644) and sanitation (p=0.046; OR=2.832; 95%CI:1.020-7.860). Furthermore, the environmental sanitation as the factor in mountainous area (p=0.034; OR=3.978; 95%CI:1.112-14.230). The impact of stunting if left untreated will cause problems for generation in the future, for example impairment of mental development and physical growth, and it will end in poverty and the threat of life survival, so it need a proper intervention to handle it. Keywords: determinants, ecosystem zone, stunting ABSTRAK Tujuan dari penelitian ini adalah menganalisis gambaran dan membandingkan pengaruh variabel faktor penentu stunting anak balita pada berbagai zona ekosistem di Kabupaten Kupang. Jenis penelitian menggunakan desain observational dengan rancangan case control antara kelompok balita stunting dan normal. Jumlah subjek penelitian ini sebanyak 132 subjek yang terbagi pada tiga zona ekosistem dataran rendah, dataran sedang, dan pegunungan pada bulan Maret 2015. Data dianalisis dengan menggunakan Analisis Regresi Logistik Berganda. Hasil penelitian menunjukkan faktor penentu stunting pada zona ekosistem di Kabupaten Kupang adalah sanitasi lingkungan (p=0,002; OR=2,307; 95%CI:0,120-0,721), dan kejadian sakit (p=0,007; OR=0,294; 95%CI:0,120-0,721); di zona ekosistem dataran rendah adalah asupan energi (p=0,002; OR=0,059; 95%CI:0,010-0,359) asupan energi sebagai faktor protektif; di zona dataran sedang adalah praktik kasih sayang (p=0,002; OR=9,247; 95%CI:2,213-38,644) dan sanitasi lingkungan (p=0,046; OR=2,832; 95%CI:1,020-7,860) dan di zona eksosistem pegunungan adalah sanitasi lingkungan (p=0,034; OR=3,978; 95%CI:1,112-14,230). Dampak yang ditimbulkan dari stunting jika tidak ditangani akan menimbulkan permasalahan bagi generasi di masa mendatang misalnya perkembangan mental dan pertumbuhan fisik terganggu, dan berakhir pada kemiskinan dan ancaman kelangsungan hidup sehingga memerlukan intervensi yang tepat dalam penanganannya.
