Balloon dilatation was performed in two cases of supravalvular aortic stenosis with variable results. There was remarkable reduction in gradient in the patient with membranous obstruction and the benefits are sustained at 18 months follow-up. The patient with hour-glass obstruction had only a partial success with reappearance of obstruction within 1 week. Balloon dilatation may emerge as a viable treatment modality for membranous obstructions. However, a large study is needed to establish the place of this procedure.

Dissimilar atrial rhythms are generally defined by the coexistence of atrial fibrillation in one atrium and a more regular rhythm in the other atrium, and are reported with left atrial enlargement and/or digitalis toxicity. We report a unique case of dissimilar atrial rhythms controlling two different parts of the same atrium without interfering with one another. The mechanism of the dissociation of two foci is not clear; however, scarring due to previous surgery remains a possibility. (PACE 1997; 20[Pt. I]:1363‐1364)

We report a case of pacemaker syndrome where the diagnosis of retrograde ventriculoatrial conduction was made by Doppler analysis of left atrial appendage flow. Doppler analysis of left atrial appendage flow provides another noninvasive parameter to diagnose retrograde Ventriculoatrial conduction.

We report a patient with tetralogy of Fallot and pulmonary atresia in whom the predominant extracardiac supply of arterial blood was via an aortopulmonary window. Major aortopulmonary collateral arteries originating from the descending thoracic aorta provided an additional source of pulmonary blood flow. The patient underwent successful correction and the postoperative recovery was uneventful.
