Laser-assisted uvulopalatoplasty with Nd : YAG laser for sleep disorders

We evaluated the apnea index (AI), the oxygen saturation above 95% (SA95), the lowest oxygen saturation (LSAT), and snoring before and after laser‐assisted uvulopalatoplasty (LAUP) in 106 patients with obstructive sleep apnea syndrome (n=59) or snoring (n=47). Type 1 LAUP was performed in 42 patients and type 2 LAUP in 64 patients. A 50% or greater reduction in AI was observed in 15 patients (35.7%) who underwent type 1 LAUP and 37 patients(57.8%) who underwent type 2 LAUP. Snoring was diminished in 18 (51.4%) of 35 patients who underwent type 1 LAUP and 30 (55.6%) of 54 patients who underwent type 2 LAUP. SA95 and LSAT showed no difference. No serious complications such as significant bleeding, postoperative episodes of asphyxia, nasopharyngeal stenosis, or nasal regurgitation were observed. LAUP was an effective outpatient treatment.

An application of outer membrane protein p6-specific enzyme-linked immunosorbent assay for detection of haemophilus influenzae in middle ear fluids and nasopharyngeal secretions.

An enzyme-linked immunosorbent assay specific to outer membrane protein P6 (P6-ELISA) was applied for detecting Haemophilus influenzae in middle ear fluids (MEFs) from acute otitis media (AOM) patients and in nasopharyngeal secretions (NPSs) from acute rhinosinusitis patients. P6-ELISA had a sensitivity of 83.3% for MEFs and 71.5% for NPSs and a specificity of 85.6% for MEFs and 92.5% for NPSs, respectively. Real-time PCR exhibited significant differences in the number of ompP1 gene copies among samples determined by P6-ELISA to be positive and negative for H. influenzae. However, because the P6-ELISA test has the reactivity in Haemophilus species include two commensals H. haemolyticus and H. parainfluenzae, it is thus a weak method in order to detect only NTHi correctly. Consequently, diagnosis using the P6-ELISA should be based on an overall evaluation, including the results of other related examinations and clinical symptoms to prevent misleading conclusions in clinical setting.

A case of hysterical trismus.

A 19 year-old male with hysterical trismus reported. The patient had various signs such as sudden facial palsy, progressive trismus and spastic type gait disturbance. On the laboratory examinations including X-ray Computed tomography, Electric myogram and Cerebro-spinal fluid analysis, there were no findings suggesting any pathological changes.The results of psychiatric analysis suggested that these clinical symptoms represented a hysterical reaction.By Jacobsons progressive relaxation method and symptomatic treatment, these symptoms improved and disappeared completely after three weeks.

Quantitative analysis of sialic acid and L-fucose in patients with chronic sinusitis or nasal allergy.

慢性副鼻腔炎 (副鼻炎), 鼻アレルギー (鼻ア) の病態解明の一端として, 血清, 組織, 鼻汁中の可溶性蛋白結合シアル酸およびフコースの定量をチオバルビツール酸法, システイン硫酸法を用いてそれぞれ行い, 以下の結果を得た.1. 血清中シアル酸は副鼻炎, 鼻ア症例ともに正常者と有意差はなく, フコース値は副鼻炎, 鼻ア症例の順で有意に高値を示した. また副鼻炎患者レ線所見の程度, および鼻茸の有無と血清中フコース値との間には高い相関関係が認められた.2. 副鼻炎上顎洞粘膜, 鼻茸中のシアル酸は, 肥厚性鼻炎下甲介および鼻ア下甲介のそれに比して有意に高値, もしくは高い傾向がみられた.3. 副鼻炎鼻汁と上顎洞内貯留液中のシアル酸, フコースは, ともに鼻ア鼻汁のそれに比し高値をしめした.
