A Simple Method to Evaluate Residual Renal Function by Spot Urinary Cystatin C-to-Creatinine Ratio in Peritoneal Dialysis Patients

Residual renal function (RRF) is a key element in the management of chronic peritoneal dialysis (PD) patients, and 24-hour creatinine clearance (24-h Ccr) and arithmetic mean of creatinine and urea nitrogen clearances [24-h (Ccr+Curea)/2] are still standard clinical techniques for the assessment of glomerular filtration rate (GFR) to represent RRF. However, it is sometimes difficult to monitor urine collection for 24 hours, especially in outpatients, and it requires serum sampling. Therefore, we devised a new and simple method to measure RRF in prevalent PD patients. Levels of urinary cystatin C (Cys-C) and creatinine in spot urine samples [24-h (Ccr+Curea)/2] were measured in 15 stable Japanese PD patients. Although no statistical correlation was seen between Cys-C and 24-h (Ccr+Curea)/2 values, a strong correlation was found between the spot urine Cys-C-to-creatinine ratio [U (Cys-C/Cr)] and 24-h (Ccr+Curea)/2. By simple linear regression analysis, the following regression equation was derived: y = 17.0 – 6.1x, where x = log 10 000*U (Cys-C/Cr) ratio and y = 24-h (Ccr+Curea)/2. Measurement of U (Cys-C/Cr) ratio does not require serum sampling or 24-hour urine collection but requires only a spot urine sample. We suggest that the U (Cys-C/Cr) ratio is a simple and easy screening method to estimate GFR in PD patients.

Serum levels of the complements and circulating immune complexes in chronic hemodialysis patients with diabetes mellitus.

糖尿病を原疾患とする慢性透析患者 (DHD患者) について, 補体成分 (CH50, C3, C4), 補体分解産物C3d, また補体活性化因子として, 循環免疫複合体 (CIC) を測定し, 補体の動態を検討した.対象は, 全例インスリン依存型糖尿病 (IDDM) とし, DHD患者10名の他, 補体動態を原疾患との関連において検討するために, 糖尿病性腎症 (DN) 10名, 細小血管症を合併しない糖尿病 (F) 9名, 健常者 (N) 42名についてこれらの測定を行った.その結果, 以下の結果が得られた. 1. 補体成分は, DHD群のC3値以外は, F, DN群と共に高補体値を示した. また, C3分解産物C3dは, DHD群, DN群において, N群に比し有意に高値を示し, 両群における補体活性の亢進が示唆されたが, DHD群のCH50, C4, C3dの変動は原疾患における補体動態との関連が示唆された. 2. CICについても, F, DN, DHD各群共, N群に比し有意に高値を示すと共に, DHD群ではさらにF群に対してもより高値を示した. すなわち, CICが腎症の進行に何らかの関与をしている可能性が示唆された. 3. DHD群におけるCICと補体との関連を検討した結果, CICとC4間にr=-0.676 (p<0.05) と有意な負の相関がみられた他, CICとCH50間にr=-0.5843 (p<0.10), CICとC3d間にr=0.5639 (p<0.10) とそれぞれ負, あるいは正の相関の傾向がみられ, DHD群では, CICに基づくclassical pathwayを介した補体活性の亢進が示唆された.これらの結果は, 今後DHD患者の病態を考える上で意義あることと思われた.

Plasma levels of complement fragments. C4d, iC3b & Bb.

慢性透析患者の補体変動の意義を知るために, 慢性腎炎に基づく慢性透析患者 (HD患者) 48名, 健常者50名の補体分解産物, C4d, iC3b, Bbを測定し活性化の面から検討した. また補体活性化因子としてcirculating immune complexes (CIC) を測定し, 補体との関連を検討した.C4d, iC3b, Bbはモノクローナル抗体を用いたELISA法 (富士レビオ) により, CICはCytotec社製のkit (C1q固相法) を用いたELISA法により測定した. CH50はMayer法に基づく稲井らの方法, C3, C4は寒天平板を作製し一元免疫拡散法により測定した.その結果, N群に比しHD患者群では, それぞれ, classical pathway (CP) を反映するC4d, alternative pathway (AP) の活性化を反映するBb, 両経路の影響を受けるC3の活性化を反映するiC3b共に有意に高値を示し, 両経路の活性亢進が示唆された. またCICは, HD患者群ではN群に比し有意に高値を示すと共に, 補体, 補体分解産物をCIC高値群, 正常値群間で比較すると, C3, C4, CH50, Bbは両群間で有意差なくも, C4d, iC3bはCIC高値群では正常値群に比し共に有意に高値を示し, CP活性化に, CICの関与が示唆された.その他, 補体分解産物と透析期間の関連を検討した結果いずれも有意な相関は認められなかった.以上のことは, 今後HD患者の病態, 合併症を考える上で興味あることと思われた.
