AIMS The objective of this work was to evaluate the acute toxicity of long-circulating and pH-sensitive liposomes containing cisplatin (SpHL-CDDP), after their intraperitoneal administration in male and female mice. MAIN METHODS After single administration of free CDDP (5,10,and 20 mg/kg) or SpHL-CDDP (7,12,30,45 and 80 mg/kg), the body weight was recorded and the LD(50) was calculated. Blood samples were collected for biochemical and hematological analysis. Kidneys, liver, spleen and bone marrow were removed to histopathological examination. KEY FINDINGS Mice treated with high doses of free CDDP showed a greater loss of body weight and more delayed recovery time than those treated with SpHL-CDDP. The LD(50) values for SpHL-CDDP treatment for male and female mice groups were 2.7 and 3.2 fold higher, respectively, than that obtained for free CDDP. The red and white blood cells counts and quantification of hemoglobin and hematocrit presented no change upon administration of SpHL-CDDP treatment. Free CDDP treatment, however, did lead to an appearance of mild anemia and a reduction in total white blood cell counts. As regards nephrotoxicity, it was observed that free CDDP treatment caused pronounced alterations in the blood urea and creatinine levels of mice. In contrast, these parameters were slightly altered only after SpHL-CDDP treatment at a dose of 30 mg/kg. Microscopic analysis of kidneys from mice treated with SpHL-CDDP showed no morphological alteration. Concerning hepatotoxicity, no histopathological alteration was observed after both treatments. SIGNIFICANCE These findings reveal that SpHL-CDDP can eliminate CDDP-induced toxicity and is thus a promising candidate for intraperitoneal chemotherapy.

Cisplatin (CDDP) is one of the most active cytotoxic agents and has been widely used in the treatment of peritoneal carcinomatosis by the intraperitoneal (i.p.) route. However, CDDP, a low-molecular-weight compound, is rapidly absorbed by the capillaries in the i.p. serosa and transferred to the bloodstream, inducing the appearance of systemic side-effects, such as nephrotoxicity. Furthermore, the i.p. CDDP chemotherapy is limited to patients whose residual tumor nodules are less than 0.5 cm in diameter after surgical debulking. The failure of i.p. therapy is attributed to the poor penetration of CDDP into larger tumors. One strategy to improve drug delivery in the peritoneal region and reduce toxicity is the use of drug delivery systems. The objective of the present work was to evaluate the biodistribution and antitumoral effect of long-circulating and pH-sensitive liposomes containing CDDP (SpHL-CDDP), as compared with free CDDP, after their i.p. administration in Ehrlich ascitic tumor-bearing mice. After administering a 6 mg/kg single i.p. bolus injection of either free CDDP or SpHL-CDDP, ascitic fluid (AF), blood and organs (kidneys, liver, spleen and lungs) were collected and analyzed for CDDP content. The area under the CDDP concentration–time curve (AUC) obtained for AF and blood after SpHL-CDDP administration was 3.3-fold larger and 1.3-fold lower, respectively, when compared with free CDDP treatment, thus indicating its high retention within the peritoneal cavity. The determination of the ratio between AUC in each tissue and that in blood (Kp) showed a lower accumulation of CDDP in kidneys after SpHL-CDDP treatment. The SpHL-CDDP treatment demonstrated a significant uptake by the liver and spleen. SpHL-CDDP treatment led to a higher survival rate of mice with initial or disseminated peritoneal carcinomatosis than CDDP treatment. These results indicate that SpHL-CDDP may be useful for i.p. chemotherapy due to their greater concentration in the peritoneal cavity.

The incidence of melanoma is increasing by 1:1500 in 1935 to 1:75 in 2000. However, there is five-year survival improvement with decrease in mortality due to early diagnosis. Initial staging is based on signs and symptoms showing metastatic disease. Special care should be taken at lymphatic examination. The two most powerful independent prognostic variables are tumor thickness and ulceration. In patients with nodal metastasis, the three most powerful prognostic factors are number of positive nodes, tumor burden and the presence or absence of ulceration of primary lesion. Great differences exist in survival between patients with melanoma metastasis in visceral sites and those with metastasis in no visceral sites. Patients with lung as the only site of visceral metastasis had a better survival compared with other visceral sites. In many large series high DHL level has been shown to be a consistent, independent, and powerful prognostic factor in patients with stage IV disease. The lack of treatment pattern in melanoma has lead some patients to a bad prognosis because an error in the initial approach. The analysis of the sentinel lymph node techniques was included in the staging and therapeutics decision of melanoma patient. Treatment has changed during the years and the purpose of this paper is present a review reinforcing conducts, based on literature.

Aims The aim of this study is to confirm the function of tumor-infiltrating lymphocytes (TILs) in sentinel lymph node (SLN) metastasis. Materials and Methods This retrospective study included 633 patients with invasive melanoma who underwent sentinel lymph node biopsy in 7 referral centers certified by the Brazilian Melanoma Group. Independent risk factors of sentinel node metastasis (SNL) were identified by multiple logistic regression. Results SLN metastasis was detected in 101 of 633 cases (16.1%) and in 93 of 428 patients (21.7%) when melanomas ≤ 1mm were excluded. By multiple logistic regression, the absence of TILs was as an independent risk factor of SLN metastasis (OR = 1.8; 95%CI: 1.1–3.0), in addition to Breslow index (greater than 2.00 mm), lymph vascular invasion, and presence of mitosis. Conclusion SLNB can identify patients who might benefit from immunotherapy, and the determination of predictors of SLNB positivity can help select the proper population for this type of therapy. The absence of TILs is a reproducible parameter that can predict SLNB positivity in melanoma patients, since this study was made with several centers with different dermatopathologists.

With the increased number of cancer diagnoses among young women and the advances in treatment, many patients who may have had their fertility compromised by chemotherapy express desire to become pregnant in the future. Freezing embryos for later IVF so as to preserve fertility is a well established process. Oocyte cryopreservation by vitrification has evolved greatly in recent years and is no longer considered experimental. By 2009 more than 900 children were born from cryopreserved oocytes, without increased risk of congenital anomalies. The preventive use of GnRH analogues for ovarian suppression during chemotherapy to avoid premature ovarian failure has uncertain outcomes. Other techniques such as cryopreservation of ovarian tissue for later autograft are still considered experimental. Although use has already been reported in 24 births, doubts still persist and motivate further study. In vitro maturation of ovarian follicles is a promising alternative for preserving patient fertility and has shown positive results in rodents, monkeys, and humans. Caution should be used with experimental techniques, especially when offered to emotionally fragile patients. Therefore it is important to thoroughly convey information on the chances of pregnancy with existing treatments and the limitations of experimental techniques.

OBJECTIVE To analyze retrospectively a series of cases culminating in amputation for advanced squamous cell carcinoma. METHODS We studied eight patients with histologically confirmed squamous cell carcinoma of the skin that had limbs amputated by tumor invasion at our institution between 2005 and 2008. We evaluated the histological factors and the institutional and psychosocial factors that contributed to this unfavorable outcome. RESULTS The mean age at diagnosis was 63 years, 37.5% of patients (three patients) had a history of abusive and continuous exposure to sunlight and six (75%) patients had other risk factors for SCC of the skin. Seven patients were diagnosed when the tumor was already larger than 2cm, and it required a large period of time (6.7 years on average) between the onset of the initial lesion and the correct histopathological diagnosis of locally advanced tumor. CONCLUSION The unfavorable outcome (amputation) in patients with squamous cell carcinoma may be associated with aggressiveness of cancer and related comorbidities, and may also be influenced by factors such as access to public health, quality of care and patients relationship with the disease.

Introducao: o melanoma e um câncer muito agressivo, sua incidencia vem aumentando, e e atualmente o quinto câncer mais comum em homens e o sexto mais comum em mulheres nos Estados Unidos. O melanoma nao e incomum na gravidez, com uma taxa de ocorrencia estimada em 1: 1.000. Apesar de nao ser o câncer mais comum na gravidez, o melanoma e o tumor com maior incidencia de metas-tases da placenta e do feto. Descricao: paciente de 29 anos foi submetida a resseccao de lesao pigmentada, tendo o diagnostico de melanoma atipico T4b, quatro semanas apos a concepcao. Com 16 semanas de gravidez paciente foi submetida a alargamento de margens e biopsia de linfonodo sentinela (SLN). SLN foi avaliado histologi-camente sendo positivo para melanoma. A paciente foi entao submetida a linfadenectomia axilar radical, sem evidencias de metastase em outros linfonodos. Na 40(a) semana de gravidez, o parto foi induzido, e por cesarea, nasceu um menino saudavel. Discussao: a abordagem do melanoma na gravidez e mais complexa e exige uma coordenacao multidisciplinar, bem como uma ampla discussao com a paciente e sua familia. Apresentamos uma descricao do caso onde as recomendacoes de tratamento foram estabelecidas de acordo com a experiencia em pacientes nao gravidas.

Traunatic perineal hernia remains a rare clinical entity despite an overall increase in blunt trauma. Because of the incidence of other associated injuries, the mortality is high. Most of the perineal defects are repaired during the orthopaedics surgery to reconstitute the pelvis and few patients develop a true perineal hernia without pelvic instability. A 80-year-old woman was involved in a running over accident with disjunction of pubic symphysis, dislocation of sacrum-iliac junction and fracture of pubis and ischium. The patient was submitted to an orthopaedic surgery and latter development an perineal hernia through the genitalia. The diagnosis could be established with physical examination alone. Conventional radiology, computadorized tomography, and ultrasound should also be done to progran the surgery. The repair approach was performed using a marlex mesh, fixed in the pelvic bones, Cooper ligament, and the abdominal wall. The mesh was stood in a retro- peritoneal position, rebuilding the pelvic floor without reconstruction the pelvic bones. We conclude that this is an efficient approach to repair of traumatic postoperative perineal hernia, mainly in patients with high operative risk, when the osseous repair is not necessary.
