THE KRUKENBERG PROCEDURE IN THE JUVENILE AMPUTEE.

The Krukenberg procedure has been performed unilaterally on juvenile amputee patients with congenital bilateral absence of the hands and observed by long-term follow-up examinations. The opposite extremity was routinely fitted with a standard prosthetic prescription as an assistive hand. In all cases, the Krukenberg stump became the leading hand. In some of the cases, the prosthesis on the opposite hand has been discarded in favor of the sensibility available without. The prehension ability of the Krukenberg stump has provided the patient with all of his functional needs. The results have been remarkably rewarding to these patients. There has been virtually no concern evidenced by the patient or the parents about the appearance of the stump. The surgical procedure has been considered to be reasonably uncomplicated. Postoperative rehabilitation has been simple and rapid. The patients have continued to improve in their rehabilitation, and the Krukenberg forceps with its improved prehension and sensibility has improved the quality of life of these people. This procedure should be extended to more individuals who have the severe handicap of bilateral hand loss.

Restoration of Hand Function by the Use of Partial or Total Prosthetic Replacement

The partial prosthesis fills the gap between a total prosthesis and reconstructive surgery. It allows the patient who still has a functioning hand remnant to have useful prehension; at the same time, available sensation is not masked. It may aid a patient awaiting reconstructive surgery and avoid total amputation and a total prothesis. A partial prosthesis may be used when a patient is reluctant to undergo multiple reconstructive procedures. A satisfactory result sometimes may be achieved with a combination of surgery and a partial prothesis. The development of efficient partial prostheses for the patient who lacks major structures of the hand due to trauma or congenital deficiency should be continued.

Studies of the hip joint by means of lateral acetabular roentgenograms.

In the following investigation studies were made of 400 normal and abnormal hip joints by means of a lateral roentgenographic view of the acetabulum. It is recognized that there are several variable anatomical factors present in congenital dislocation of the hip. The anterior projection of the acetabulum, or the relationship of the acetabulum to the long axis of the patient’s body, appears to be an important factor in determining the head-to-acetabulum relationship. Accordingly, the authors have attempted to work out a method of measuring the angle made by the acetabulum with the sagittal plane of the body.
