Visual Performances With Monofocal, Accommodating, and Multifocal Intraocular Lenses in Patients With Unilateral Cataract

PURPOSE To compare the visual performance of patients with unilateral cataract following implantation of monofocal, accommodating, refractive, and diffractive multifocal intraocular lenses (IOL). DESIGN Prospective nonrandomized clinical trial. METHODS Eighty-seven patients with unilateral cataract were enrolled in 4 groups for phacoemulsification and IOL implantation. Twenty-four patients had monofocal (Alcon Acrysof) (group 1), 21 patients had accommodating (Human Optics 1CU) (group 2), 22 patients had diffractive multifocal (Tecnis ZM900) (group 3), and 20 patients had refractive multifocal (AMO Rezoom) (group 4) IOL implantations. Ages of patients were between 40 and 70. Parameters analyzed at the 18th postoperative month were subjective refractions, monocular and binocular distance, intermediate and near uncorrected visual acuities, monocular distance and near best-corrected visual acuities, monocular distance-corrected intermediate and near visual acuities, stereopsis, visual complaints, and spectacle dependency. RESULTS No significant difference was observed between distance and near best-corrected visual acuities of IOL groups, and between intermediate visual acuities of groups 2, 3, and 4. Groups 3 and 4 had statistically better near vision than the other groups (P < .05). No significant difference was observed between near visual acuities of groups 3 and 4. Number of patients with better stereoscopic function, spectacle independence, and complaints of halo in groups 3 and 4 was significantly higher than in other groups (P < .05). CONCLUSIONS Multifocal IOLs provide better stereopsis, higher spectacle independence rates, and satisfactory functional vision over a broad range of distances in presbyopic patients with unilateral cataract compared with the monofocal and accommodating IOLs.

Effectiveness of the Lower Eyelid Suspension Using Fascia Lata Graft for the Treatment of Lagophthalmos due to Facial Paralysis

Purpose. To evaluate of functional and cosmetic effectiveness of lower eyelid sling technique with fascia lata graft in patients with lagophthalmos due to facial paralysis. Material and Method. Ten patients with a mean age of 55.1 ± 19.77 years who underwent lower eyelid sling surgery with a fascia lata graft between September 2011 and January 2014 were included in this prospective study. Preoperatively and postoperatively patients were evaluated in terms of corneal epithelial defects, Schirmers test, and tear break-up time (TBUT). Cosmetically, vertical eyelid aperture, margin reflex distances 1 and 2 (MRD1 and MRD2) and scleral show were evaluated preoperatively and postoperatively. Results. One patient had facial paralysis on the right side whereas the other 9 patients had facial paralysis on the left side. Preoperatively, 3 patients were detected with corneal ulcer, whereas 7 patients were detected with persistent corneal epithelial defects localized in the lower half of the cornea. In the 3 patients with preoperative corneal ulcer, the ulcer recovered with corneal opacity, whereas in the 7 patients with punctate epitheliopathy, postoperative corneal transparency was obtained. Discussion. Lower eyelid sling technique with fascia lata graft is an effective technique for the repositioning of the lower eyelid and preventing the corneal complications.

Comparison of Modifications in Flap Anastomosis Patterns and Skin Incision Types for External Dacryocystorhinostomy: Anterior-Only Flap Anastomosis with W Skin Incision versus Anterior and Posterior Flap Anastomosis with Linear Skin Incision.

Purpose. To compare the outcomes of external dacryocystorhinostomy (E-DCR) by using two different flap anastomosis patterns and skin incision types. Methods. This study included 79 patients (88 eyes) with lacrimal drainage system disorders who underwent E-DCR surgery. Fifty eyes of 44 patients (group A) underwent E-DCR by suturing anterior and posterior flaps (H-flap) of the lacrimal sac with curvilinear skin incision whereas in 38 eyes of 35 patients (group B) DCR was performed by suturing only anterior flaps (U-flap) with W skin incision. Results. The success rate was evaluated according to lacrimal patency and scar assessment scores. Patency was achieved in 78 patients (88.6%). In terms of groups, patency was 44 eyes (88.0%) in group A and 34 eyes (89.5%) in group B. There was no statistically significant difference in the success rates of lacrimal patency between the two groups. Further, there was no statistically significant difference concerning cutaneous scar scores. Conclusion. Our study suggests that anastomoses of only anterior flaps or both anterior and posterior flaps have similar success rates; suturing only anterior flaps is easier to perform and shortens the operative time. In addition, W skin incision is a reasonable alternative to curvilinear incision for reducing scar formation.

Peripapillary choroidal thickness in contralateral eyes after evisceration of diseased eyes

PURPOSE To evaluate peripapillary choroidal thickness changes in contralateral eyes of patients who had undergone evisceration of their diseased eyes. METHODS In this retrospective study, peripapillary choroidal thickness parameters in 34 eyes of 34 patients who had undergone diseased-eye evisceration between March 2014 and May 2016 were evaluated using spectral domain optical coherence tomography. The scans were manually delineated to identify the principal surfaces of Bruchs membrane, the Bruchs membrane opening, and the anterior sclera. Peripapillary choroidal thickness was measured between the Bruchs membrane and the anterior sclera at increasing distance away from the Bruchs membrane opening. The mean peripapillary choroidal thickness values in the contralateral eyes of the patients and those of the control group were compared. RESULTS The mean peripapillary choroidal thickness was higher in the contralateral eyes of the patients compared with that of normal eyes at all distances from the Bruchs membrane opening. CONCLUSION Increased peripapillary choroidal thickness was noted in the contralateral eyes of the patients, potentially resulting in a thicker choroid. Although further investigation is required to determine the cause, these findings indicate the presence of a compensatory factor in the contralateral eyes.

Anterior tarsal flap rotation combined with anterior lamellar reposition in the repair of cicatricial upper eyelid entropion

PURPOSE This study aimed to share the results of patients who underwent anterior tarsal flap rotation combined with anterior lamellar reposition because of cicatricial upper eyelid entropion, and to determine the effectiveness and reliability of this surgical technique. METHODS Fifteen eyes of 11 patients (2 right eyes; 5 left eyes; and 4 bilateral eyes) on whom we performed anterior tarsal flap rotation surgery combined with anterior lamellar reposition because of cicatricial entropion were included in this study. The medical records of the patients were analyzed retrospectively, and the causes of cicatricial entropion as well as the preoperative and postoperative ophthalmic examination findings were recorded. Normal anatomical appearance and function of eyelid were considered to have been achieved. RESULTS The mean age was 59.81 ± 18 years. The mean follow-up period was 21.72 ± 14 months (range, 5-43 months). The causes of cicatricial entropion were postoperative cicatrices development due to multiple electrolyzes for trichiasis and/or distichiasis in 7 eyes, trachoma in 6 eyes, and trauma in 2 eyes. Irritation and watering were detected in all patients preoperatively, whereas corneal opacity and erosion were detected in 10 patients and epithelial erosion was detected in one patient. Full anatomical and functional success was achieved for all patients. CONCLUSION Anterior tarsal flap rotation combined with anterior lamellar reposition in the repair of cicatricial entropion was found to be an effective and reliable alternative surgical procedure.

Intraorbital space-occupying lesions: the evaluation of clinical, radiological and pathological results

Intraorbital space-occupying lesions: the evaluation of clinical, radiological and pathological results Objective: To present clinical findings, radiological and pathologic data of the intraorbital spaceoccupying lesions (ISOL) treated with surgical intervention Materials and Methods: The medical records of 81 patients who underwent surgery for ISOL between January 2010 and April 2017 were reviewed retrospectively. The demographical data of the patients, preoperative and postoperative ophthalmologic examination findings and the location of mass radiologically were examined in detail. The surgical approach and surgical technique, peroperative and postoperative complications, histopathological results, the duration of follow-up were recorded. Results: A total of 81 patients (37 female, 44 male) were included in this study. The mean age was 37.14±23.53 year. Intraorbital Space-Occupying Lesions (ISOLs) were intraconal in 3 patients, extraconal in 71 and both intraconal and extraconal in 7 patients. The lesion was in the form of a nontumoral mass in 25 patients, while tumoral in 56 patients. Trans-conjunctival orbitotomy approach was performed in ten patients, while the orbitotomy via skin incision was performed in 71 patients. Conclusions: Intraorbital Space-Occupying Lesions (ISOLs) cause functional and cosmetic important problems according to their localizations. The definitive treatment of these lesions is mostly surgical and full surgical excision may provide permanent healing.

Surgical approach to retinal detachment with primary congenital glaucoma

We present the clinical features and treatment approach of a case with congenital glaucoma, trabeculectomy and rhegmatogenous RD, and other ophthalmic complications. A 41-year-old male patient with trabeculectomy due to congenital glaucoma in both eyes was admitted to our clinic due to decreased visual acuity for the past three days. Visual acuity was at the level of hand movement in the right eye, and his left eye was phthisic. The bleb on the upper nasal side of the right eye was well formed but cystic and avascular. Fundus examination revealed a patchy RD in the upper half including the macula. Phacoemulsification and intraocular lens (IOL) implantation combined with pars plana vitrectomy-liquid-perfluorocarbon-air-silicone oil change and endolaser application was performed under general anesthesia. Silicone oil tamponade was removed 2 months later, and IOL was implanted with optic capture in the same session. The final visual acuity was 1/10. There was non-functional, avascular and flat bleb due to bleb sclerosis. From the beginning to the last follow-ups, progressive shallowing of the bleb was observed in anterior OCT sections.In a patient with monocular buphthalmos, cataract surgery combined with RD surgery involves multiple risks. In this case, we achieved good visual and anatomic outcome of the retina, but we had bleb functional loss because of bleb sclerosis. We recommend periodic fundus examinations in patients with congenital glaucoma. Rhegmatogenous RD in patients with congenital glaucoma can be reattached, but full recovery may not be achieved.
