Response to Disodium Etidronate Treatment in Three Siblings with Pulmonary Alveolar Microlithiasis

Pulmonary alveolar microlithiasis (PAM) is a rare chronic genetic lung disease in childhood with no proven therapy. It is characterized by the deposition of calcium phosphate microliths within the alveolar air spaces. The effect of disodium etidronate (DE) treatment on PAM is controversial. We report 3 siblings (an 11-year-old boy and 4-year-old twin girls) with PAM diagnosed by chest X-ray, thoracic high-resolution computed tomography, technetium-99m bone scan and bronchoalveolar lavage fluid findings. After the administration of DE (200 mg/day) for a 1-year period, 2 siblings showed radiological improvement, while 1 sibling did not. No drug side effects were observed within the treatment period.

Nontuberculous pulmonary cavitary diseases of childhood.

We describe the demographic, clinic and radiologic features of nontuberculous cavitary pulmonary diseases in 42 patients with a mean age of 91.1 ± 6.8 months. Infectious etiology was the most common cause (64%), including necrotizing pneumonia (n = 15), ruptured hydatid cyst (n = 5), lung abscess (n = 5) and fungal infection (n = 2). Other causes were bronchiectasis, congenital anomalies, foreign body aspiration, sarcoidosis and tumor.

The value of nasopharyngeal aspirate, gastric aspirate and bronchoalveolar lavage fluid in the diagnosis of childhood tuberculosis

Turk J Pediatr 2018; 60: 10-13. Pulmonary tuberculosis (TB) is an important cause of morbidity and mortality especially in developing countries. A definitive microbiologic confirmation of Mycobacterium tuberculosis is important in the diagnosis of childhood TB. We aimed to compare the diagnostic value of nasopharyngeal aspirate (NPA), gastric aspirate (GA) and bronchoalveolar lavage (BAL) specimens in children with highly suspected pulmonary tuberculosis (TB). NPA, GA and BAL samples were obtained from forty patients. The mean age was 9.2±4.7 years. Sixty-eight percent of children had a history of household contact and 82% had tuberculin skin test positivity. Acid-fast bacilli (AFB) stain was positive in 22.5% (N=9) of BAL, 17.5% (N=7) of GA, and 10% (N=4) of NPA samples. Positive Lowenstein-Jensen culture was 27.5% (N=11) in BAL, 22.5% (N=9) in GA, and 12.5% (N=5) in NPA samples. Positive AFB stains and growth in TB cultures from BAL fluid and GA samples were both higher than NPA samples (p < 0.006 and p < 0.004, respectively GA). We conclude that NPA specimen fails to determine Mycobacterium tuberculosis in children with highly suspected pulmonary TB when compared to GA or BAL fluid.

Oxygen saturation/minute heart rate index: Simple lung function test for children

The severity of airway obstruction can be accurately determined on spirometry in children with asthma. Other assessments may include peak expiratory flow and pulse oximetry. In the present study, we evaluated the validity and reliability of oxygen saturation/minute heart rate (SpO2/MHR) index in the prediction of degree of severe airway obstruction in children with asthma.

İnfant döneminde nadir bir stridor nedeni: konjenital laringosel

Bes aylik erkek olgu dogumdan itibaren solunum zorlugu ve stridor nedeniyle basvurdu. Akciger grafisinde peribronsial belirginlesme disinda parankim normal bulundu. Hastaya tanisal fleksible bronkoskopi uygulandi ve on laringeal bolgede vokal kordlarin ustunde buyuk bir kistik dilatasyon saptandi. Akabinde yapilan ultrasonografi ve MR goruntulemesiyle konjenital laringosel tanisi kesinlestirildi. Trakeotomi ve takibinde kistik yapinin igne aspirasyonu ile bosaltilarak hava yolu stabilize edildi. Konjenital laringosel ust hava yolunda darlik ve boyunda kitle yapan cok nadir dogumsal bir anomaldir. Ses kisikligi, yutma guclugu, solunum zorlugu ve aspirasyonla seyredebilir.
