Effect of Maternal Age on Pregnancy Outcome and Cesarean Delivery Rate

Background The aims of this retrospective study were to evaluate the maternal and prenatal outcomes between 35 years and older pregnancies and younger pregnancies, and the effects of the age of pregnancy, mother and newborn. Methods Pregnant women who gave birth in Vakif Gureba Training and Research Hospital, Clinic of Obstetrics and Gynecology in 2006 were retrospectively screened. Pregnant women aged 35 years and over were included in this study and the pregnant women between age range of 30 - 34 years were included in the control group. Results Pregnancy rate was found as 7.1% in 35 years and older women in all the deliveries, cesarean delivery rate was found as 46.1% in this group at 1 year period. However, cesarean delivery rate was 40.9% in the control group. Cesarean delivery rate was found as 31.6% in all the deliveries. The most common cause of cesarean section indication was fetal distress in advanced maternal age (AMA) (11.7%), whereas previous cesarean section was found as the most common cause in the control group (15.1%). Conclusion No significant difference was found between AMA group and normal pregnancies in terms of preterm labor, caesarian section, morbidity, mortality and chronic diseases such as hypertension and diabetes mellitus.

Changing Efficacy of Wet Cupping Therapy in Migraine with Lunar Phase: A Self-Controlled Interventional Study

Background The aim of this study was to evaluate the effect of blood-letting with wet cupping therapy (WCT) in migraine treatment and to determine whether there was any difference according to the phase of the moon when the treatment was applied. Material/Methods This self-controlled study was conducted in Karabuk between 2014 and 2016. Patients who were diagnosed with migraine were enrolled in the study. Migraine disability assessment questionnaire (MIDAS), demographic characteristics, migraine attack frequency and duration, and family history was used to assess the severity of headache. WCT was applied in 3 sessions and the questionnaire was administered before and 3 months after the final WCT session. Wilcoxon signed-rank test was used in pre-and posttreatment measurements, and the Chi-square test was used to check independence in two-way categorical tables. Results A total of 85 patients were included. The reduction in MIDAS scores and number of migraine attacks was significantly greater in the WCT applications made in the first half of the month compared to those in the second half of the month. Although the reduction in visual analog scale (VAS) scores was greater in the second half of the month, it was also significant in the applications made in the first half of the month. Conclusions WCT was found to be an effective treatment of migraine. The effect on MIDAS, VAS, and the number of attacks was significantly better when the application was made in the second half of the month compared to those made in the first half.

Predictive Value of Mean Platelet Volume in Variceal Bleeding due to Cirrhotic Portal Hypertension

Aim: To investigate whether mean platelet volume (MPV) is a predictor of variceal bleeding in patients with cirrhotic portal hypertension. Materials and methods: This prospective cohort was performed in the internal medicine department of our tertiary care center. Cirrhotic patients were allocated into two groups: Group I consisted of 31 cases without a history of variceal bleeding, whereas group II was made up of 31 patients with a history of variceal bleeding. Data derived from medical history, physical examination, ultrasonography, gastrointestinal system endoscopy, complete blood count, hepatic, and renal function tests were recorded and compared between two groups. On physical examination, encephalopathy and ascites were evaluated and graded with respect to Child-Pugh-Turcotte classification. Results: There was no significant difference between the two groups in terms of age, duration of the disease, and gender of the patient. The only remarkable difference was that hemoglobin (p = 0.02) and hematocrit (p = 0.02) values were lower in group II. Neither the etiology of bleeding was different between groups nor did MPV seem to have a noteworthy impact on bleeding. Interestingly, risk of variceal bleeding increased in parallel to the higher grade of varices. Conclusion: Our results imply that there is a correlation between the grade of varices and esophageal vari-ceal bleeding in cirrhotic patients. However, association between MPV and variceal bleeding could not be demonstrated. Utilization of noninvasive tests as predictors in these patients necessitates further controlled trials on larger series. A model of collaboration between palliative care unit and home health care services: Karabük

Amac: Bu calismada bir universite hastanesi palyatif bakim merkezinin (PBM) evde saglik hizmetleri (ESH) ile entegre calisma orneginin sunulmasi ve PBM’nin bir yillik hizmetlerinin tanimlanmasi amaclanmistir. Materyal ve Metot: KBU Karabuk Egitim ve Arastirma Hastanesi PBM de 01.12.2015 ile 01.12.2016 tarihleri arasinda yatan hastalar retrospektif olarak degerlendirildi. Hastalarin demografik ozellikleri, mevcut hastaliklari, uniteye yatirilma nedenleri, hastanede kalma sureleri kaydedildi. Hastalara yatis ve taburculuk zamanlarinda performanslarini degerlendirmek amaciyla uygulanan Karnofsky Performans (KPS) ve Palyatif Performans Skalalari (PPS) sonuclari kaydedildi. Bulgular: PBM de 01.12.2015 ile 30.11.2016 tarihleri arasinda 149 hasta takip edildi. Hastalarin yas ortalamasi 74,93±14,61 (min:17-maks:96) yil olup, %43’u (n=64) erkek, %57’si (n=85) kadindi ve serviste yatis sureleri ortalamasi 14,50±12,03 (min:1- maks:79) gundu. En sik yatis nedenleri beslenme bozuklugu ve dekubit ulserleri iken altta yatan hastaliklar en sik norolojik bozukluklar ve malignitelerdi. Hastalarin %78,52’si (n=117) yataga tam bagimli idi. Bir yil icerisindeki mortalite orani %12,08 (n=18) idi. Hastalarin taburcu olduklarindaki KPS ve PPS skorlari yatis zamanlarina gore daha yuksekti (p<0,001). Sonuc: Hastalarin hastanede yatis surelerinin optimal surede tutulabilmesi, PBM ile ESH nin koordineli calismasina bagli olabilir. Hastalarin taburcu olduklarinda degerlendirilen performanslari ve potansiyel bakim ihtiyac skorlarinin yatis esnasindaki skorlarindan daha yuksek olmasi palyatif bakimin onemini gostermektedir.

In early pregnancy, it is very difficult to distinguish the clinical and laboratory values of normal pregnancies, complete or incomplete abortions, and ectopic pregnancies (1). Late diagnoses can cause troubles that may result in maternal deaths (2). Ectopic pregnancies are still responsible for 10% of maternal deaths in the Western world and are the leading cause of maternal mortality in the first trimester (3). Therefore, they should be diagnosed as soon as possible so treatment can begin.

Candida glabrata Pneumonia in a Patient with Chronic Obstructive Pulmonary Disease

Pneumonia remains an important cause of morbidity and mortality among infectious diseases. Streptococcus pneumoniae and viruses are the most common cause of pneumonia. Candidiasis in such patients has been associated with haemodialysis, fungal colonization, exposure to broad-spectrum antibiotics, intensive care unit (ICU) hospitalization, and immunocompromised patients. The most common cause of infection is C. albicans. The case presented here is of a 66-year-old male patient diagnosed with C. glabrata. The patient suffered from chronic obstructive pulmonary disease.

Uyku Apne Sendromu Hastalarında Pozitif Hava Yolu Basıncı Tedavisinin Yaşam Kalitesi Üzerine Etkisi

Amac: Uyku apne sendromu obezite ve kardiyovaskuler hastaliklar ile yakin iliskilidir. Uyku apne sendromu, norokognitif fonksiyonlari ve yasam kalitesini olumsuz etkiler. Uyku apne sendromunun tedavisinde positive airway pressure (PAP) cihazlarinin kullanimi halen altin standarttir. Bu calismada ulkemizde PAP ile tedavi edilen uyku apne sendromu olgularinda kisa form-36 anketi ile yasam kalitesindeki degisim olculmustur. Materyal ve Metot: Calisma PAP cihazi ile tedavi edilen 52’si erkek toplam 67 ardisik uyku apne sendromu hastasi ile yapildi. Butun olgulara tum gece uyku laboratuvarinda polisomnografi yapildi. SF-36 skoru PAP cihazi kullanmadan once ve sonrasindaki 6-18 aylik sure icinde yapildi. Bulgular: PAP cihazini uygun sekilde kullanan hastalarda SF-36’nin butun parametrelerinde duzelme oldugu saptandi (P<0,05). Sonuc: Bu calisma PAP tedavisinin yasam kalitesini olumlu yonde etkiledigini gostermistir. Calismamiz ulkemizde SF-36 ile bu konuda yapilan ilk calismadir.
