Pseudoaneurysm Formation in Surgically Treated Behçet's Syndrome A Case Report

Behçets syndrome is a multisystem disorder with unknown etiology. Clinically it is mostly seen as a systemic vasculitis; almost 30% of the patients have vascular involvements, and most of these are venous thrombosis and arterial aneurysms. Obstructions of the femoral and tibial arteries have also been reported in the literature. The authors present here a patient with Behçets syndrome who had pseudoaneurysms on both femoral arteries after aortobifemoral bypass surgery.

SUBXIPHOID APPROACH FOR TREATMENT OF PERICARDIAL EFFUSION

Subxiphoid pericardiotomy was the primary treatment in 305 patients with pericardial effusion from January 1984 to June 1996. There were 198 males and 107 females, ages ranged from 15 days to 75 years. The procedure was carried out with local anesthesia and sedation in 263 (86.2%) patients and under general anesthesia in 42 (13.8%). Median drainage was 975.25 ± 48.46 mL in 264 patients with benign effusion and 1131.25 ± 97.48 mL in 41 (13.4%) with malignant disease; cytology was positive in 14 of 38 (36.8%) and pericardial biopsy showed cancer in 12 of 36 (33.3%). Intraoperative complications in 22 patients (7.2%) included cardiac arrest in 12 (3.9%) of whom, 7 (2.3%) died. Overall 30-day mortality was 16.3%; it was 46.3% (19/41) in malignant cases versus 11.7% (31/264) in cases of benign effusion. Follow-up of 234 (91.8%) hospital survivors for 18 ± 3.62 months (range, 2 to 54 months) showed recurrent pericardial effusion needing further intervention in 31 (13.2%) of whom, 8 had cancer and 23 had benign disease. Median survival in benign cases was more than 107 days versus 56 days in malignant cases. Because of its acceptable mortality and morbidity, subxiphoid pericardiotomy is recommend as an initial procedure.

A Case with Weaver Syndrome Operated for Congenital Cardiac Defect

Abstract. An 11-month-old Turkish female infant with Weaver syndrome together with atrial septal defect and patent ductus arteriosus which was operated successfully is reported. Weaver syndrome is a very rare disorder of unknown etiology characterized by accelerated growth of prenatal onset, advanced osseous maturation, special craniofacial features, umbilical hernia, and hoarse low-pitched cry. Congenital cardiac defect is not a usual finding. The presented case is the first reported child with Weaver syndrome in the literature operated for a congenital cardiac defect.

Behçets syndrome is a multisystem disorder of unknown etiology with a worldwide distribution. It is considered a systemic vasculitis affecting the skin, joints, eyes, gastrointestinal tract, neural tissue, and blood vessels. About 30% of the patients have vascular involvement, mainly as venous thrombosis, but aneurysms of the aorta, peripheral, and pulmonary arteries have also been reported. The authors present a patient with Behçets syndrome who had aneurysms both of the popliteal artery and the carotid artery treated surgically.

Abnormal connection of the inferior vena cava to the left atrium with double outlet right ventricle and heterotaxia: A case report

A 4-year-old boy with abnormal connection of the inferior vena cava to the left atrium and double outlet right ventricle and right atrial isomerism is presented. The anomalies were detected by echocardiography and angiography, and later verified through surgical intervention.

Two surgically treated patients with severe symptoms of Takayasu arteritis

Takayasu arteritis, also known as the aortitis syndrome, is a panarteritis that affects the arcus aorta and its major branches, seen usually in young women. As Takayasu arteritis has a progressive nature, surgery is usually indicated only in complications of the arteritis. In this report we present two surgically treated cases with severe complications of Takayasu arteritis. We also review the syndrome.

Extracranial Internal Carotid Artery Aneurysm with Concomitant Subclavian Artery Aneurysm Initially Diagnosed with Scintigraphic Evaluation A Case Report

Internal carotid and subclavian artery aneurysms combined in a single case, diagnosed noninvasively with scintigraphy, are described here. A tomographic brain perfusion study was also performed to evaluate hemispheric perfusion distribution for deciding on the use of a carotid shunt. A carotid angiogram was also obtained before the operation, in which the aneurysms were removed and the artery was repaired with end-to-end primary anas tomosis.
