BackgroundSplenic artery aneurysms (SAA) are uncommon but the most common visceral artery aneurysm. Splenic artery aneurysms are important to recognize because up to 25% may be complicated by rupture and the mortality rate after rupture is between 25% and 70%.Case reportWe present a patient who have abdominal pain. Previously healthy 22-year-old female admitted to emergency department with abdominal pain. Her physical examination reveals only left upper quadrant tenderness. Suddenly she developed hypovolemic shock. On emergent laparotomy massive blood collection within peritoneal cavity and retroperitoneal space at the left upper quadrant was detected. The source of bleeding was evident as rupture of splenic artery aneurysm. Splenectomy was performed following the ligation of splenic artery proximal to lesion. On the tenth day she was discharged from the hospital with complete recovery.ConclusionIt is important to remember rupture of splenic artery aneurysm in patients with abdominal pain and hypovolemic shock status.

Gallstone ileus is a rare complication of cholelithiasis that has high morbidity and mortality. An intestinal obstruction can be caused by migration of a large gallstone through a biliary enteric fistula or by impaction within the intestinal tract. In this study, we present the case of an 81-year-old woman with a mechanical bowel obstruction by a gallstone that was treated by laparoscopy.

Introduction. Schwannomas are quiet rare in the retroperitoneal region. Here, we describe an incidentally detected retroperitoneal schwannoma in the abdominal computerized tomography (CT) of a patient with acute appendicitis. Case Presentation. A 38-year-old woman was admitted to the emergency service with the complaints of progressive abdominal pain and nausea for the last 24 hours. Abdominal examination was compatible with acute abdomen. Acute appendicitis was diagnosed by CT. During CT evaluation, a round shaped soft-tissue mass at the retroperitoneal area inferior to the right kidney was detected, The mass was resected and histology revealed schwannoma. Conclusion. Rare tumoral lesions with benign course such as schwannoma can be detected incidentally.

Sirolimus (SRL) not only displays prophylactic activity, it also reverses acute rejection (AR) in humans with ongoing refractory renal allograft rejection and chronic liver allograft rejection. The present case of a human liver allograft recipient documents the utility of SRL therapy for a patient experiencing ongoing resistant recurrent AR just after orthotopic liver transplantation (OLT) and liver dysfunction that had not abated after treatment with corticosteroids and antilymphocyte globulin. SRL was added to the mycophenolate mofetil/steroid regimen to treat the ongoing rejection. The patient was rescued with SRL, not experiencing AR again. To date the patient has exhibited no significant side effects after more than 6 months of SRL therapy and his clinical and general condition have been good; he is completely involved in daily life. SRL is a new and safe immunosuppressive agent for rescue in patients with early OLT and recurrent AR.

Abdominal wall hernias are a common problem in the general population. A Western estimate reveals that the lifetime risk of developing a hernia is about 2%.1–3 As a result, hernia repairs likely comprise the most frequent general surgery operations. More than 20 million hernias are estimated to be repaired every year around the world.4 Numerous repair techniques have been described to date however tension-free mesh repairs are widely used today because of their low hernia recurrence rates. Nevertheless, there are some ongoing debates regarding the ideal approach (open or laparoscopic),5,6 the ideal anesthesia (general, local, or regional),7,8 and the ideal mesh (standard polypropylene or newer meshes).9,10

Skin-sparing mastectomy with sentinel lymph node biopsy (SLNB) and synchronous breast reconstruction are widely used in breast cancer surgery nowadays. Difficulties in feeling confident in this technique and postoperative surgical complications are the major obstacles against the widespread usage of this technique. Compared with the other surgical techniques, the complications are hard to treat. Cutaneous necrosis because of methylene blue used for sentinel lymph node mapping in patients who underwent skin-sparing mastectomy and SLNB is already reported in the literature. We present here two cases with cutaneous necrosis because of isosulphane blue injection after skin-sparing mastectomy and SLNB as a rare complication of dye injection.

Subjective global assessment (SGA) of nutritional status is a widely used and validated method for identifying and classifying malnutrition. It has been thought that this semiquantitative feature restricts the reliability and precision of the SGA. Recently, in an effort to assess nutritional status, a modified quantitative SGA system has been devised in which scores are assigned for items or components of the SGA. This prospective study evaluated the correlation of the quantitative SGA with objective nutritional parameters and compared this method with conventional SGA in a group of patients on the renal transplant waiting list.

Although diverticular disease of the colon is common, the occurrence of rectal diverticula is extremely rare with only sporadic reports in the literature since 1911. Symptomatic rectal diverticula are seen even less frequently, and surgical intervention is needed for only complicated cases. Here we report the case of a 63-year-old woman presenting with rectal diverticulitis mimicking rectal carcinoma with intestinal obstruction.

Tail gut cysts are rare congenital lesions located in retrorectal area arising from the vestiges of the embryonic hindgut and often present in middle-aged women with perirectal symptoms. Although tail gut cysts are usually benign congenital lesions, malignant transformation has been rarely reported. In this case report we present a 48year-old woman who was admitted to hospital with an asymptomatic palpable perirectal mass for two months. The radiological appearance of tail gut cyst was three lobulated and it was successfully excised with combined perineal and abdominal approaches.
