The knowledge of the craniotopography allows the delimitation of the cranial approaches. In this study the landmarks, defined in relation to the craniometric points and used in the different cranial approaches, were systematized. Twenty two landmarks are described: the first twelve are in relation to the skull base and the remainder are in relation to the skull vertex.

Anatomia microcirúrgica do nervo laríngeo recorrente: aplicações no acesso cirúrgico anterior à coluna cervical

OBJECTIVE To present an anatomical description of the recurrent laryngeal nerve (RLN) on both sides of the larynx as it relates to the possible lesion mechanisms in anterior cervical spine surgery. METHOD Twelve adult cadavers were examined from the microsurgical laboratory at the School of Medicine at UFMG, MG, Brazil. The data collected were analyzed in terms of frequency, average and standard deviation. RESULTS The left RLN had a total average length of 9.4 +/- 1.6 cm entering the larynx in 36.3% of the cases at the approximate height of C5, 18.2% at C4, 18.2% at C5-C6, 18.2% at C6 and 9.1% at C4-C5. Recurrence appeared in 45.4% of the cases at the approximate height of T3, 18.2% at T3-T4, 18.2% at T4 and 18.2% at T5. The right RLN had a total average length of 5 +/- 0.3 cm entering the larynx in 44.4% of the cases at the approximate height of C5, 44.4% at C6, and 11.1% at C3-C4. Recurrence appeared in 60% of the cases at the approximate height of T1, 30% at C7 and 10% at T2. CONCLUSION The right RLN was found to be more vulnerable to operational lesions due to two complimentary yet different reasons, trajectory and length. Owing to the fact that a more oblique and unprotected trajectory is not related directly with the tracheoesphageal groove, there is a strong possibility of direct traumas occurring as with the resulting compression from the employ of surgical retractors or an accidental cut, principally in surgeries involving lower vertebral levels. In the same way, the smallest RNL length favors the stretching of fibers during the per-operative traction.

Rizotomia trigeminal por radiofrequência para tratamento da neuralgia do trigêmeo: resultados e modificaçäo técnica

The purpose of this study was to evaluate the efficacy of radiofrequency trigeminal rhizotomy in treating 135 patients harboring trigeminal neuralgia, and to introduce a technical modification to guide the puncture of the foramen ovale. A hundred and one (74.8%) patients were treated with a single surgical procedure whereas the 34 (25.2%) remaining patients required two procedures. Follow-up ranges from 6 months to 15 years. Pain relief in the immediate postoperative was achieved in 131 (97.0%) patients. After the initial procedure, recurrence happened in 33 (24.5%) patients. The complications included decrease corneal reflex (4.4%), masseter paresis (2.2%), painful dysesthesia (1.5%) and anesthesia dolorosa (0.7%). The radiofrequency trigeminal rizhotomy is a low risk, highly effective and minimally invasive procedure. The use of the computerized tomography guided fluoroscopy turns foramen ovales puncture easier, fast and precise.

Surgical treatment of the occipital condyle fracture: case report

We present a case of fracture of the occipital condyle showing neck pain, lesion of IX, X and XII cranial nerves and pyramidal syndrome of the four members. A review of the literature about the surgical treatment of the occipital condyle fracture is done.

Lesão cerebral penetrante por grande fragmento de fibra de amianto tratada por craniectomia descompressiva: relato de caso

We report the case of a 22-year-old man victim of penetrating brain injury due to a 15 x 12 asbestos fragment and a successfully treatment via decompressive craniectomy. Unlike gunshot wounds to the head, penetrating brain injury from low energy objects are unusual. Most cases reported involve cranio-orbitary injuries as well as self inflicted lesions in mentally ill patients. The reported case is noteworthy due to the large dimensions of the foreign body, the treatment via decompressive craniectomy and the good patient functional outcome.

Occipital bi-transtentorial/falcine approach for falcotentorial meningioma: case report

Lesions located in the bilateral posterior incisural space are difficult to treat due to limited exposure. The classical approaches to this area are limited for lesions located bilaterally and especially when the lesion extends also below the tentorium as it may occur with meningiomas. Kawashima et al. reported, in anatomic studies, a new occipital transtentorial approach: the occipital bi-transtentorial/falcine approach, to treat such lesions. We present a patient with a large falcotentorial meningioma, located bilaterally in the posterior incisural space. The occipital bi-transtentorial/falcine approach allowed an excellent surgical exposure and complete tumor removal with an excellent patient outcome.

Laminoplastia expansiva: uma alternativa no tratamento da mielopatia espondilótica cervical

We describe the surgical technique of expansive cervical laminoplasty and analyse the results in 28 patients treated by this method for cervical spondylotic myelopathy with a minimum follow-up of six months. Twenty-four patients (86%) had clinical improvement according to the Nurick scale while three (10%) had no improvement and one patient died on the first days post-operatively. The good results achieved demonstrate that this technique is simple, effective and has few complications on the treatment of spondylotic myelopathy.

Coleção subdural na criança: fisiopatologia e tratamento

Nine children harboring subdural effusions were treated by subduro peritoneal shunt. These patients were followed-up by CT scans. The area of the subdural effusions was measured by quantitative morphology with a planimeter. With the surgical treatment, the subdural effusion disappeared completely or near completely in 8 patients. The patients functional state were excellent in 4, good in 3 and bad in 2 in the postoperative follow-up. We aldo reviewed the literature as far as the pathophysiology and the treatment of the subdural effusions are concerned.
