Validity of staining and marginal ditching as criteria for diagnosis of secondary caries around occlusal amalgam restorations: an in vitro study

This study determined the validity of marginal ditching and staining as criteria for the diagnosis of secondary caries around amalgam restorations. One hundred and twenty-four Class I amalgam restorations on extracted human teeth were submitted to standardized clinical examinations. A calibrated examiner recorded the presence of ditching in the tooth/restoration interface, the occurrence of bluish-grey staining on the occlusal surface, and their relationship with the presence and severity of secondary carious lesions. Examinations were repeated after 4 days to calculate the intra-examiner reliability. Stereomicroscopic inspection of the serial tooth sections was used as the gold-standard. Six teeth were lost during the study. Of the 118 remaining teeth, 19 (16%) showed occlusal bluish-grey staining and 26 showed more than 0.20 mm wide marginal ditches. Of the latter, 14 presented narrow ditches (0.20-0.45 mm), while 12 presented wide ditches (0.45 mm). The evaluation criteria presented low sensitivity and positive predictive values. Positive and negative likelihood ratios showed that the criteria exhibited, indistinctly, the same odds to determine a positive or negative result for a diseased or healthy tooth. It was concluded that marginal ditching and staining were not valid criteria for the diagnosis of secondary caries around occlusal amalgam restorations.

Biopsychosocial profile of patients with anophthalmia in the south of Minas Gerais - Brazil

PURPOSE To evaluate the biopsychosocial profile of patients with anophthalmia, with emphasis on the psychological and functional impact of eye loss and the social reintegration of this population. METHODS Prospective analyses of 84 patients (50 males and 34 females), rehabilitated or in the rehabilitation process with ocular prostheses were interviewed by means of a questionnaire with dimensions involving the etiology of the ocular defect, degree of adaptation to the ocular prosthesis, and impact on professional, family and social activities. RESULT The right eye was affected in 45.2% of the patients, the left eye in 51.2%, and the rest of the patients had bilateral anophthalmia. Difficulty in adapting to monocular vision was reported by 47.6% of the patients. The main causes of anophthalmia in males were eye injuries due to accidents (54%), and, in females, acquired diseases (38.2%). For the total studied population, the eye loss occurred at a mean of 20.5 +/- 18.41 years, and the elapsed time until the first rehabilitation with ocular prosthesis was of 8.6 +/- 13.10 years. Most patients (66.1%) reported satisfaction and good adaptation to the prosthesis. Feelings of sadness, shame and shyness were frequently reported. CONCLUSION Anophtalmic patients often exhibit psychic and/or functional disorders which hinder their social, professional and family readaptation, and this is aggravated by both economic factors and lack of public services that provide rehabilitative treatment. Public information campaigns could also be useful to prevent causes that lead to ocular loss.

Caries Experience in a Sample of Adolescents and Young Adults With Cleft Lip and Palate in Brazil

Objective To compare the caries experience of adolescents and young adults with cleft lip and/or palate (CL/P) with a noncleft control group. Design Thirty CL/P subjects and 30 controls were clinically examined to obtain the decayed, missing and filled teeth (DMFT) and the decayed, missing and filled surfaces (DMFS) indices, gingival bleeding index, plaque index, and active caries lesions. Data concerning oral hygiene, access to fluoridated water, mothers education level, and family income were also collected. Setting Pro-Smile Center, a reference center for the treatment of facial deformities, Alfenas, Minas Gerais, Brazil. Subjects Subjects aged 12 to 21 years with CL/P and without associated syndromes were matched to noncleft controls by sex, age, living habits, and use of orthodontic devices. Null Hypothesis Formulated Prior to Data Collection Caries experience in CL/P adolescents and young adults is similar to that observed in noncleft controls. Statistical Analysis Data were analyzed using SPSS 17.0 software for Windows Data Editor. The CL/P and control groups were compared using the McNemar test, paired t test and Wilcoxon test. A significance level of 5% was adopted for all tests. Results There were no significant differences between the groups for oral hygiene and contact with fluoride. Significant differences were found in per capita income, presence of active caries, decayed surfaces, plaque index, and gingival bleeding. Conclusions The caries experience of CL/P subjects was higher than that of the noncleft individuals.

Review of follow-up of clinical success of implant-supported complete dental prostheses

OBJECTIVE: The aim of this study was to assess the clinical success rate of implant-supported complete dental prostheses, which are being used as a feasible alternative in planning of oral rehabilitation. Over the years, the two-stage surgical protocol has increasingly been replaced by immediate loading procedures, especially in the mandibular anterior region in cases of rehabilitation of completely edentulous patients. This procedure has become feasible due to the high success rate of the technique with very satisfactory results.METHODS: By means of a literature review focused on published articles with clinical follow-up between 8 and 15 years.RESULTS: Implants placed in the mandible had higher success rates than those inserted in the maxilla. Cases with longer follow-up periods, 15 years, showed higher success rates in comparison with follow-up periods of 8 and 10 years.CONCLUSION: Success rates of implant-supported complete dental prostheses are very high.

Isolamento do Campo Operatório: Panorama de Utilização em Consultórios e Clínicas Privadas de Belo Horizonte, MG, Brasil

Objetivo: verificar a utilizacao de isolamento em consultorios e clinicas privadas em Belo Horizonte, observando-se: tipo, procedimentos executados com isolamento e o motivo da escolha; alem de fatores como: tempo de formacao dos cirurgioes-dentistas (CDs), classe economica dos pacientes e presenca de pessoal auxiliar (ASB). Metodo: a amostra elegivel foi formada por CDs atuantes na regiao Centro-Sul da cidade. A amostra final de conveniencia foi de 115 CDs que responderam a um instrumento de coleta de dados com questoes descritivas e objetivas. Os dados foram analisados descritivamente e pelo teste Qui-quadrado (p≤0,05). Resultados: dos CDs que participaram da pesquisa, 55% eram do genero feminino; 63,5% tinham entre 23 e 35 anos; 39% tinham menos de cinco anos de atividade profissional e 59% eram especialistas. Eram da regiao Central 69% e Sul 31%; tem ASB 66% dos CDs. Os pacientes foram classificados em classe economica A (11%), B (30%), C (34%), D (16%) e E (9%). Relataram utilizar algum tipo de isolamento 82% dos CDs (36% relativo; 28% absoluto; e 36% ambos). O grau de aceitacao dos pacientes quanto aos metodos de controle de umidade foi considerado alto para 52% dos CDs. O principal motivo para escolha do isolamento absoluto foi o controle de infeccao (27%) e para o relativo foi a praticidade (24%). O isolamento absoluto e utilizado principalmente na endodontia (38%) e o relativo em restauracoes (33%). Genero (p=0,6483), tempo de atividade (p=0,2576), especialidade (p=0,0733) e presenca de ASB (p=0,4127), nao influenciaram significativamente na escolha do tipo de isolamento. Conclusao: a maioria dos CDs relatou utilizar algum tipo de isolamento do campo operatorio e aponta como principais vantagens a biosseguranca, a praticidade e melhor qualidade dos procedimentos executados.
