MEDICAMENTOS POTENCIALMENTE PERIGOSOS: IDENTIFICAÇÃO DE RISCOS E BARREIRAS DE PREVENÇÃO DE ERROS EM TERAPIA INTENSIVA

Objective: to investigate the knowledge of nursing professionals and pharmacists in relation to the identification of potentially hazardous drugs, as well as the recognition of error prevention barriers in their use in hospital institutions. Method: cross-sectional survey performed in intensive care units of four hospitals. A questionnaire constructed and validated based on the information provided by the Institute for Safe Practices in Drug Use was used for data collection. Statistical Package for Social Sciences, version 22.0 and the Kruskal-Wallis test were used to analyze the data between the professional categories. The significance level was set at 0.05. Results: 126 professionals were included, among those eligible for participation. Of the 33 potentially hazardous drugs indicated in the instrument, none were identified as such by all respondents, although 17 were used by more than 95% of respondents. No statistically significant difference was observed in the responses of the different professional categories regarding the identification of these drugs. Regarding the measures of error prevention, nurses were the professional category that distinguished the existence of barriers at a higher rate. Conclusion: this study pointed to important gaps in the recognition of potentially hazardous drugs and incipient adoption of barriers to prevent incidents, characterizing situations of fragility in hospitals because it implies the initial rupture of barriers, especially when the health professionals are inserted in high-complexity services

Standard Operational Protocols in professional nursing practice: use, weaknesses and potentialities

OBJECTIVE to evaluate the use of Standard Operational Protocols (SOPs) in the professional practice of the nursing team based on the theoretical framework of Donabedian, as well as to identify the weaknesses and potentialities from its implementation. METHOD Evaluative research, with quantitative approach performed with nursing professionals working in the Health Units of a city of São Paulo, composed of two stages: document analysis and subsequent application of a questionnaire to nursing professionals. RESULTS A total of 247 nursing professionals participated and reported changes in the way the interventions were performed. The main weaknesses were the small number of professionals, inadequate physical structure and lack of materials. Among the potentialities were: the standardization of materials and concern of the manager and professional related to patient safety. CONCLUSION The reassessment of SOPs is necessary, as well as the adoption of a strategy of permanent education of professionals aiming at improving the quality of care provided.

Use of instruments to evaluate leadership in nursing and health services

OBJECTIVE To identify the available scientific evidence about the use of instruments for the evaluation of leadership in health and nursing services and verify the use of leadership styles/models/theories in the construction of these tools. METHOD Integrative literature review of indexed studies in the LILACS, PUBMED, CINAHL and EMBASE databases from 2006 to 2016. RESULTS Thirty-eight articles were analyzed, exhibiting 19 leadership evaluation tools; the most used were the Multifactor Leadership Questionnaire, the Global Transformational Leadership Scale, the Leadership Practices Inventory, the Servant Leadership Questionnaire, the Servant Leadership Survey and the Authentic Leadership Questionnaire. CONCLUSIONS The literature search allowed to identify the main theories/styles/models of contemporary leadership and analyze their use in the design of leadership evaluation tools, with the transformational, situational, servant and authentic leadership categories standing out as the most prominent. To a lesser extent, the quantum, charismatic and clinical leadership types were evidenced.

Translation to Brazilian Portuguese and cultural adaptation of a questionnaire addressing high-alert medications.

Objective To describe the translation into Portuguese and cultural adaptation of a Questionnaire addressing High-Alert Medications to the Brazilian context. Methods Methodological study comprising the translation from Chinese to Brazilian Portuguese, synthesis of translations, back translation, panel of experts, and pretest to obtain the final version of the questionnaire. Results <inor lexical variations were resolved after obtaining consensus between the translators. The panel of experts concluded that the Portuguese version presented semantic, idiomatic, cultural and conceptual equivalence, though 50% of the items required adjustment. Thirty nurses from a teaching hospital participated in the pretest and considered the items to be understandable. Conclusions Satisfactory semantic, idiomatic, cultural and conceptual equivalence was obtained between the versions. The Portuguese version was also considered to be relevant to the Brazilian culture and easily understood. Nevertheless, its psychometric properties need to be assessed before making it available.

The prescription of drugs is essential in the pharmacotherapeutic treatment bringing important information to the pharmacy and the user. Therefore reduce the occurrence of prescription errors is important for promoting the rational use of medicines. This study analyzed drug prescriptions dispensed in the Pharmacy School of UNIFAL-MG identifying the most frequent mistakes. We analyzed 323 prescriptions between 2006 and 2008. Using the law 5991/73, there was the presence of the following information: readability; professional identification, patient identification, identification of substances, dosage, dosage form, quantity, address and date of consultation; and interactions and pharmacological aspects. Analysis of requirements showed that 30.95% were illegible or difficult to read; 15.78% failure to identify the prescriber; 6.19% lack of assay 4.02% absence of the dosage form; 5.57% description of quantity; 10.52% dosage wrong / incomplete; 43.34% lack of data / address, and 3.10% drugs incompatible. This study demonstrates the poor quality of drug prescriptions, indicating a lack of preparation or even indifference toward the prescribers of drug users. It is necessary to sensitize health professionals to the importance of quality of prescribing to dispensing and administration of medicines and promoting rational use, providing the patient with greater efficacy and safety of its pharmacological treatment.
