AN OPEN LABELLED CLINICAL STUDY TO EVALUATE THE OCCUPATIONAL STRESS OVER WRIST JOINT (MANIBANDH SANDI) AMONG INDIAN AUTO RICKSHAW DRIVERS

Auto rickshaw drivers are more prone to musculoskeletal injuries related to their occupation when a vehicle is in motion the body is subjected to different forces: accelerations and decelerations lateral swaying from side to side, and whole-body up and down vibrations. In Auto rickshaws driver’s wrist joint are under optimum usage as there will be frequent flexion and extension of wrist joint especially of left hand because he must change the gears frequently. Therefore, due to over use of joints there may be a stress which may lead to structural changes in those joints. By considering the above information current study was taken into consideration to evaluate the influence of occupation on the structural changes of both the wrist joints in auto rickshaw drivers. 70 samples of auto drivers were randomly taken in study that were working in the same posture for 8– 10 hours and having minimum of 15 years of work experience. Study has revealed that there is positive correlation between total period of work in years and duration of complaint also there is positive correlation between daily working hours and duration of complaint, Stress revealed that it effects various functional and structural changes in auto rickshaw drivers. The presence of pain and swelling helps to conclude the stress on soft tissue like muscle and tendon i.e. involvement of musculoskeletal system. They can affect nearly all tissues, including the nerves and tendon sheaths, and most frequently involve the arms, wrist and back in auto rickshaw drivers.

A Clinico-analytical Study on Seed of Wrightia antidysenterica Linn. as a Therapeutic Emetic Agent (Vamaka Yoga) in the Management of Psoriasis.

Objectives: Wrightia antidysenterica Linn. (WA) is male variety Kutaja stated to be potent therapeutic emetic agent in skin disorders. Expulsion of doshas through oral route is termed as Vamana Karma (VK) (therapeutic emesis). However, so far, its utility for Vamana is not explored in detail, therefore there is a need to revalidate the utility of WA for Vamana. Hence, the above study was conducted to ascertain the efficacy as a therapeutic emetic agent (vamaka yoga) in the management of psoriasis along with quality control and standardization of this herb. Materials and Methods: The drug was standardized as per analytical procedures in Pharmacopeias. Thirty patients of psoriasis fulfilling inclusion criteria were taken for the study and Vamana with WA was conducted. Criteria were prepared to assess the signs and Symptoms of psoriasis. VK was assessed using the classical Lakshanas (features) such as Anthiki shudhi (Ending symptoms of emesis), Vaigiki shudhi (features of vomiting bouts), Maniki shudhi (Quantitative and qualitative purification), complications. Result: VK with WA showed significant relief in parameters of psoriasis such as scaling, itching, candle grease sign (P < 0.001), and psoriasis area and severity index score (P = 0.001). In VK with WA, mean number of Vegas (vomiting bouts) was 6.91. 66% patients showing quantitative purification between 301 and 600 ml. 73.33% showed all Symptoms of purification. 73.33% patients showed Kaphanta vamana (Moderate expulsion of desire humor). In the level of biopurification, 66.66% patients showed moderated purification. No complication was noted with moderate drug palatability. Conclusion: Pharmacopeial analytical study showed its standardized values for testing the drug used for the study. It is proved as potent therapeutic emetic agent with no complication showed its clinical benefits over skin disorder like psoriasis.

Effect and Mechanism of Virechana Karma (Therapeutic Purgation) Over Fructose-Induced Metabolic Syndrome An Experimental Study

Background. Panchakarma (biopurification methods) is one of the modes of ayurveda to treat disorders of the body. Virechana karma (therapeutic purgation), one among the Panchakarma, is a purification process that is commonly used to treat metabolic disorders like obesity and diabetes mellitus. Hence this study was planned to provide evidence through animal experiments. Methods. Albino rats were subject to Virechana karma (therapeutic purgation) to evaluate the influence of therapy and its mechanism over fructose-induced metabolic syndrome. Results. Results show that Virechana is effective in the management of the metabolic syndrome with decrease in the fecal fat content, fasting blood glucose, serum triglyceride, and reduced fatty changes in liver, heart, and kidney in comparison with the positive control group. Conclusion. Experimental evaluation showed decrease in fatty acid in the storage like liver, kidney, heart, and muscle adipose tissue can indirectly increase the insulin sensitivity in insulin receptor present at skeletal muscles.

Role of Ayurveda in the conservative management of avascular necrosis of the femoral head: A case study

Avascular necrosis (AVN) of the femoral head is the most common type of necrosis affecting the bones. Management of AVN aims at the preservation of structure, function and relief of from pain. Many surgical procedures such as drilling and insertion of bone grafts, modified Whitman or Colonna reconstruction and insertion of prosthesis are carried out to remedy the condition but all these procedures are costly with the prognosis being poor. Signs and symptoms of Avascular necrosis are nearer to asthivāha srotoduşṃi vikāra (disorders of musculoskeletal origin) and can be considered with gambhīra avasthā (chronic stage). An effort has been made in the present study to evaluate the efficiency of Ayurvedic formulations in the conservative management of AVN of the femoral head. A case of AVN with bilateral femoral head was treated with rūkşaṃa (Drying therapy) followed by śodhana (bio purification) and bṃhmaṃa (rejuvenation). Patient was observed for complications during whole course of treatment, untoward complications were not seen. Patient was observed for symptomatic improvements based on assessment done by the questionnaire over graded signs and symptoms before and after treatment. The results were encouraging. The therapy provided marked relief from pain, tenderness, stiffness and improvement in the gait. Conservative management of AVN through Ayurvedic principles provides significant relief and improves quality of life.

OBESITY AND INSULIN RESISTANCE: AN OVERVIEW

Central obesity is a leading preventable cause of death worldwide, with increasing prevalence in adults and children and authorities view it as one of the most serious public health problems of the 21 century. It is defined as to the presence of excess fat in the abdominal area. Of late, it has been shown that it has positive correlation with insulin resistance and is a strong risk factor for cardiovascular diseases and a strong predictor of future diabetes mellitus. This realization has brought the concept of measuring waist circumference as an indicator of insulin resistance. It is estimated that 20 % – 25 % of south Asians have developed metabolic syndrome and many more may be prone to it. There is currently no treatment to manage the metabolic syndrome. Many studies are looking at treatment that might improve some of the symptoms of metabolic syndrome.

A CLINICAL STUDY TO ASSESS THE EFFICACY OF PARIS HEKA SVEDA IN THE MANAGEMENT OF KATIGRAHA

Parisheka is the assortment of Svedana Chikitsa in Panchakarma. In Parisheka Sveda, medicated liquid is to be poured over the desired part or whole body. Parisheka Svedana with Taila provides benefits of both Snehana and Svedana together. In Ayurvedic classics Katigraha is consider as Vata Vyadhi. Shoola and Graha in Katipradesha are the symptoms of Kati graha. For understanding in contemporary science, Katigraha can be seeing as low back pain.In the treatment of Vata Vyadhi, repeated use of Snehana and Svedana is advocated. Ksheerbala Taila contains Balamoola, Ksheer and Tila Taila, which are having Shool a Hara properties. Tila Taila said as best among Taila and also having the quality to pacify Vata. Hence Ksheerbala Taila Parisheka Sveda over Katipradesha (lumbo - sacral region), with control group of Tila Taila was taken to assess its efficacy in Katigrah a. Parisheka Svedana with Ksheerbala Taila and Tila Taila was given to 30 patients of Katigraha, (divided in to two groups) for 7 days. With help of using Student paired ‘t’ test, the findings of research work were statistically analysed. Results of this s tudy show that both the study group were effective in reliving the symptoms of Katigraha, while Ksheerbala Taila Parisheka Sveda was found statistically significant in comparison of Tila Taila.Parisheka Svedana with Taila gives the dual effect of Snehana a nd Svedana thus controlled the symptoms of Katigraha efficiently. This study shows that, Ksheerbala Taila providing far better result in comparison with plain Tila Taila. Hence it can be said that Ksheerbala Taila has its own effect also in relieving Katig raha which is one of the Vata Vyadhi.

MANA GEMENT OF DUSHTA PRATHISHYAYA: A SUCCESS STORY

Dushta prathishyaya or chronic sinusitis is very common complaint in clinical practice seen in urban places. This drags patient to doctor because of severe headache, nasal obstruction, heaviness, altered olfaction with lot of GIT disturbance and sleep disturbances. Nasal congestion, mucous production, al tered secretion and blockage are chief events in dushta prathishyaya. Elimination of mucous, reliving of obstruction, cleansing the channel is the line of treatment. In other words shodhana with tikshna, ushna, kaphahara dravyas having ugra gandha are to b e selected. In the following case study all above issues are considered and thus success was achieved.

CLINICAL APPLICATION OF SHADVIDHOPKRAMA: A CRITICAL REVIEW

The eternal science of life Ayurveda has indicated various types of Chikitsa for the management of diseases. Amongst the Chikitsa mentioned, Shadvido pakrama bears lot of significance. In this context the knowledge of upakrama becomes very essential. The Sadvidhaupakrama, the combination of six p rime upakramas of Ayurveda can be considered a part of Yuktivipashraya chikitsa . Sadvidha upakrama plans to act by balanci ng the proportion of the Panchamahabhutas in the body. In other words those substances or p rocess which increases the agni , vayu and aakasha mahabhuta in the body can be termed as langhana. Result of any Upakrama is Dosha Saamyata. It is attained through b alancing the Gunas, ultimately by achieving the Panchabhouthika stability. Whatever may be the tre atment modalities it will come under the peer view of Shadvidhopkrama. That is the reason these principles of shadvidhopkrama are implemented in the day to da y clinical practice.

ROLE OF PANCHAKARMA IN DUCHENNE MUSCULAR DYSTROPHY

Duchenne muscular dystrophy (DMD) is an X - linked recessive dis order that affects muscles and causes progressive weakness. Dystrophin, the product of the DMD gene, is one of several membrane proteins that form the dystrophin – glycoprotein complex, which helps to maintain the integrity of muscle cells; loss of these pro teins l eads to the wasting of muscle. In Ayurveda it has been classified under Adibala Parvritta Vyadhi. Here pathogenesis occurs due to the Bheejabagahaavyava Dusti which lead to Medomamsa dusti further vitiates the Vata . Panchakar ma shows great improveme nt among which Virechana and B asti explained in the principle of Vata chi kits a . This pioneer approaches gives patient quality of the life and longer survival upon muscular dystrophy.
