Adenocarcinoma of the lacrimal gland: a case report

BackgroundPrimary ductal adenocarcinomas of the lacrimal gland are very rare. This neoplasm shares some histological and immunohistochemical similarities with salivary duct carcinoma.Case presentationHere, we present a case of a 55-year-old Moroccan man with lacrimal gland adenocarcinoma. He underwent orbital exenteration with lymph nodes dissection and ipsilateral parotidectomy. After surgery, he was lost to follow-up and died 13 months later.ConclusionsLacrimal gland tumors are rare but highly aggressive salivary gland tumors. Complete excision with adjuvant radiotherapy is recommended.

Squamous Cell Carcinoma in Bladder Extrophy : A Case Report andLiterature Review

Background: Bladder extrophy is a rare congenital anomaly, it is now well reconized that an exstrophy of the bladder is associated with an increased risk of bladder cancer. Case presentation: We report the case of 34-year-old male with squamous cell carcinoma of an unreconstructed exstrophic bladder. Because of the locally advanced disease, he received neoadjuvant chemotherapy with favorable response. Then, he underwent a total cystectomy with uretero-intestinal anastamosis. Results: The patient progressed three months after the surgery. Chemotherapy was established and he received 2 cycles, however, he died 2 months later. Conclusion: Squamous cell carcinoma associated with bladder extrophy remains relatively high. These tumors tend to be aggressive, follow-up is mandatory in these cases.

Adjuvant Chemoradiation for Localized Gastric Adenocarcinoma: AnInstitutional Experience

cancer with locoregional relapse in 40% to 60%. The frequency of relapses makes regional radiotherapy an attractive possibility for adjuvant therapy. The survival benefit of adjuvant chemoradiation over surgery alone was first established by the US Intergroup 0116 study. Patients and methods: Between January 2010 and December 2014, 48 patients with localized gastric adenocarcinoma, managed at radiotherapy department of the national institut of oncology, they underwent adjuvant chemoradiation according the classical MacDonald regimen. Results: This series consisted of non-metastatic patients, 68.8% (33) males with a mean age 51 years. 20.8% (10) of patients had relapses (all distant relapse). 22 (45.8%) patients died, only 8(16.6%) patients were lost to follow up. Almost all patients complete full course chemoradiation. The median follow-up duration was 25.4 (15.6- 48) months. The 5 years OS and DFS was 40.8% and 49% respectively. Disease stage, operative procedure on primary tumor was an independent prognostic for both 5-year OS and DFS. The pN, surgical margin had a significant impact on OS. Conclusion: Adjuvant chemo-radiation may be an effective and safe regimen for patients who have undergone gastrectomy with curative intent in locally advanced stomach cancer and did not receive preoperative chemotherapy.

Sleep Disorders and Sleep Quality in Moroccan Adult Patients with Cancerduring Treatment

Background: Sleep disruption is a common problem for patients with cancer. Several recent studies have reported an incidence of 30% to 50% in this group, compared to 15% in the general population, but it has received little attention from the oncology community compared with other symptoms accompanying cancer. Patients and methods: It is a cross-sectional study where we included a total of 284 patients with cancer during treatment. All patients were offered brief sleep questionnaires, judgment criteria were based on the Pittsburgh Sleep Quality Index (PSQI), the Insomnia Severity Index (ISI), and the Epworth Sleepiness Scale (ESS) to assess respectively sleep quality, insomnia, and sleepiness. We analyzed data by calculation of Cronbach’s alpha coefficient for the reliability of measurements, and by simple and multiple logistic regressions. Results: Internal consistency measurement of the ISI, ESC, PSQI subscales questionnaires found a Cronbach’s alpha coefficient of 0.895, 0.618 and 0.669, respectively. 52% of patients have no clinically significant insomnia, 12.5% have Subthreshold insomnia, 25% have clinical insomnia (moderate severity) and 10.5% have severe clinical insomnia. Patients with urologic, head and neck, and gastro-intestinal cancer had higher scores than patients with breast and gynecologic cancer. As for patients receiving surgery and chemotherapy, the ISI average score was respectively 14.3 ± 4.4 and 13.8 ± 4.9. it was statistically different (p 8.8, p=0.001), followed by urologic cancer (OR>4, p=0.001), head and neck cancer (OR=1.979, p=0.006), and lowest level of SES (OR=4.119, p=0.001), regarding type of treatment there is no significant difference between different treatment. 74% of patients with sleep disturbances report that their sleep disorders decrease quality of life, 41% report fatigue, 53% experienced mental capacity degradation and 39% of patients report that affect Interpersonal relationship, and their humor in 42%. Conclusion: Patients with head and neck cancer and urological cancer, patients receiving chemotherapy and surgery should be investigated especially among younger ones, in order to detect those at risk of sleep disruption and offer them appropriate support.

Undifferentiated Lymphoepithelial Carcinoma of Cervix: A Rare Case Report

Lymphoepithelial Carcinoma (LEC) is a rare malignancy. It has been diagnosed in head and neck region, but it was extremely rare in the cervix. There are only few case reports describing the cytology and the management of these tumors. In this report, we describe a very rare case of undifferentiated lymphoepithelial carcinoma of cervix in 50-year-old woman.

Localized Kaposi Sarcoma

Kaposi sarcoma (KS) is a lymphoangio-proliferative disease often caused by KS associated herpes virus, also known as human herpes virus 8. There are 4 known variants of KS: Epidemic or AIDS-associated KS, Endemic or African KS, iatrogenic post transplantation KS and Mediterranean or classic KS (CKS). We report a case of CKS treated successfully by radiation therapy.

The Impact of Radiotherapy in the Treatment of Recurrent Desmoid Tumor of Neck: A Case Report with Literature Review

Background: Extra-abdominal desmoid tumor of the head and neck in particular commonly infiltrate vital neurovascular structures. we report a case of a large and recurrent extra-abdominal desmoid tumor causing significant morbidity and who was treated successfully by radiotherapy. Case presentation: A 34-year-old male patient presented with a history of right neck mass for 15 months, which was gradually increasing in size. Cervical CT scan revealed a large mass of the posterior soft tissues of the neck extended to the soft tissues of the posterior thoracic wall. The CT density of the mass was that of fat tissue. It was measuring 12.4 × 85.9 mm and enhanced after injection of the contrast product. A biopsy was taken. The histopathological findings were suggestive of desmoid tumor. Discussion: This case was discussed in multidisciplinary oncological team and radiotherapy was planned. The surgery was proposed but not performed due to the recurring character of the tumor and the risk of a high postoperative morbidity. After 4 weeks of treatment, the clinical evaluation showed a good response. Conclusion: Alternative treatment modalities such as primary radiotherapy may be preferable to mutilating surgery.

An Atypical Squamous Cell Carcinoma of the Cervix

A 74-year-old female presented in our department with 4 months history of post-menopausal bleeding and pelvic pain, which was increasing in intensity over 2 months. However, she denied any weight loss, fever or altered urinary or bowel habits. Her previous medical history revealed hypertension since 10 years for that she was taking antihypertensive drugs. On physical assessment she was not pale, with good performance status. Per vaginal examination showed an ulcer and budding tumor destroying the cervix and the the upper vagina.

Lympho-Epithelial Carcinoma of the Orbit

In our hospital, a one patient had 75 years old male and from Morocco was referred for us for realized the exophthalmia, conjunctival hyperhemia and visual disturbance. The biopsy was revealed a lympho-epithelial carcinoma; the patient has received a chemoradiation therapy. the clinical response was complete.
