Experience versus complication rate in third molar surgery

ObjectivesThe records of 1087 patients who underwent surgical removal of third molar teeth were prospectively examined to analyse the possible relationship between postoperative complications and the surgeons experience parameter.Method and materialsSeven surgeons (three specialists in surgical dentistry [specialists SD] and four oral and maxillofacial Senior House Officers [OMFS residents]) carried out the surgical procedures. For each patient, several variables were recorded including age, gender, radiographic position of extracted teeth, treating surgeon, duration of surgery and postoperative complications.ResultsAnalysis of the data revealed some differences in the incidence of complications produced by the specialists SD and OMFS residents. The main statistically relevant differences were increase the incidences of trismus, nerve paraesthesia, alveolar osteitis and infection in the resident-treated group, while the specialist-treated group showed higher rates of post-operative bleeding.ConclusionThe higher rate of postoperative complications in the resident-treated group suggests that at least some of the complications might be related to surgical experience.Further work needs to compare specialists of training programmes with different years of experience, using large cross – sectional studies.

Florid cemento-osseous dysplasia in a young Indian woman. A case report

We present a rare case of florid cemento-osseous dysplasia (FCOD) occurring in a 27-year-old Indian woman. She presented with three lesions, two of them symmetrical, at the posterior region of the mandible. Another lesion appeared nine months later, symmetrical to the existing third lesion. This confirmed the diagnosis of florid cemento-osseous dysplasia as being symmetrical asymptomatic bone lesions occurring in the jaw. The diagnosis was made radiographically depending on a series of radiographs, as biopsy is contraindicated. No treatment was required as the lesions were asymptomatic, causing no problem to the patient. The patient continues to be reviewed annually. The case is rare in its unusual combination of the disease itself (FCOD), age (27 years) and race (Indian). Clinical and radiological features are presented.

Lower lip numbness is a symptom that may be due to entirely benign causes, or it may be the first sign of a more sinister problem. It has been reported as being the sole symptom of pathological lesions and metastatic tumours in the mandible.

Bruxism secondary to brain injury treated with botulinum toxin-A: a case report.

We report a successful treatment of bruxism in a patient with anoxic brain injury using botulinum toxin-A (BTX-A). On examination the mouth opening was 0 mm, no feeding was possible through the mouth. Botulinum toxin was injected into the masseter and temporalis; great improvement in trismus and bruxism was noted after 3 weeks. One further treatment improved the mouth opening on the following week and the patient was discharged from our care to be reviewed when required.
