The Transesophageal Echocardiographic Diagnosis of Left Atrial Myxoma Simulating a Left Atrial Thrombus in the Setting of Mitral Stenosis

We report the case of a 56‐year‐old woman with a history of rheumatic heart disease. The clinical, electrocardiographic, and radiologic findings suggested mitral stenosis. Left atrial obstructive myxoma simulating a thrombus was found by transthoracic echocardiography (TTE). The diagnosis was established by use of transesophageal echocardiography (TEE), confirmed after surgery and by anatomical investigation. Cardiac myxoma associated with mitral stenosis may be difficult to diagnose accurately using TTE. The advantage of TEE in this case and in patients with mitral stenosis is emphasized. Localization of tissue transglutaminase and N (epsilon)-(gamma) -glutamyl lysine in duodenal cucosa during the development of mucosal atrophy in coeliac disease

Expression and transamidation activity of tissue transglutaminase (tTG) may be involved in the morphological modifications leading to the mucosal atrophy observed in coeliac disease (CD). We aimed to investigate the localization of tTG within the duodenal mucosa during the development of villous atrophy. The localization and level of expression of Nɛ-(γ-glutamyl) lysine isopeptides which could reflect the transamidation activity of tTG were also analyzed. tTG and Nɛ-(γ-glutamyl) lysine were localized using an immunohistochemical technique on duodenal biopsies obtained from 75 patients with CD and 51 subjects with normal mucosa (control group). The number of cases displaying tTG-expressing cells in the basement membrane and lamina propria was significantly higher in CD patients than in the control group. Moreover, the intensity of tTG staining in these areas was higher in CD. In contrast, the number of biopsies with tTG-expressing enterocytes was significantly lower in CD than in the control group. There was no difference in Nɛ-(γ-glutamyl) lysine between the two populations. Tissue transglutaminase was differently expressed in the various areas of the mucosa according to the stage of atrophy, whereas the localization and the intensity of the labelling of Nɛ-(γ-glutamyl) lysine isopeptides did not show any modification. The preferential localization in the basement membrane and lamina propria may reflect the involvement of tTG in the development of mucosal atrophy in CD.

Hémangiome surrénalien: à propos d’un cas

L’hemangiome est une tumeur vasculaire benigne composee de cellules angioblastiques. Sa localisation surrenalienne est tres rare, en effet seule une cinquantaine de cas ont ete rapportes dans la litterature. Les auteurs rapportent un cas d’hemangiome surrenalien gauche decouvert fortuitement chez une femme de 55 ans dans le cadre d’un bilan d’extension d’un cancer du col. Devant la non specificite de l’aspect en imagerie, la taille lesionnelle et le contexte clinique, la chirurgie s’est imposee et c’est l’examen anatomopathologique qui a permis de confirmer le diagnostic d’hemangiome surrenalien partiellement thrombose.

Étude du contenu en ADN nucléaire dans les nodules thyroïdiens: Intérêt diagnostique et pronostique

Les nodules thyroidiens sont frequents et posent parfois un probleme de diagnostic et/ou de pronostic. L’etude de la ploidie et l’analyse du cycle cellulaire dans ce type de lesions pourraient etre un moyen de distinction entre une tumeur benigne et une tumeur maligne. Nous avons etudie le contenu en ADN nucleaire de 69 nodules thyroidiens fixes au formol et enrobes en paraffine a l’aide d’un analyseur d’images SAMBA 200. Les lames ont ete prealablement traitees selon la methode de Feulgen. Les tumeurs benignes etaient diploides dans 75 % des cas (15/20) et les tumeurs malignes etaient aneuploides dans 57,2 % des cas (28/49). L’aneuploidie etait correlee avec l’extension extra-thyroidienne (p = 0,007) et la presence de metastases ganglionnaires bilaterales et/ou mediastinales (p = 0,02). La majorite des tumeurs benignes (19/20) avaient un indice de proliferation inferieur ou egale a 3 % alors que plus que 83 % des tumeurs malignes (41/49) avaient un indice de proliferation strictement superieur a 3 % (pThyroid nodules are frequent and sometimes they pose a diagnostic and prognostic problem. DNA ploidy study and cell cycle analysis could be of value in the distinction between benign tumors and malignant tumors. Formalin-fixed and paraffin-embedded tissues from 69 patients with different benign and neoplastic lesions were investigated. Nuclear DNA content in thyroid cells was measured after Feulgen staining using SAMBA 200 image analysis system. A diploid DNA stemline was revealed in 75% of histologically proven benign thyroid tumors (15/20) and aneuploidy was found in 57.2% of malignant tumors (28/49). There is a significant correlation between aneuploidy and extra-thyroid extension (p=0.007) and bilateral and/or mediastinal lymph node metastasis (p=0.02). In the majority of benign tumors (19/20), the proliferation index was lower than 3% (< or =3%) however, this index value was higher than 3% (>3%) in more than 83% of malignant tumors (41/49) (p<0.001). The S phase fraction analysis revealed that the threshold of 14% divide the near whole of benign and malignant tumors (p<0.001). Our findings show that in follicular lesions, proliferation index and S phase fraction study appears interesting and helpful in the distinction between benign and malignant tumors, and aneuploidy seems more interesting in prognosis evaluation of these tumors.

Adénocarcinome colique révélateur de tumeurs carcinoïdes multiples du grêle

Neuroendocrine tumors of the small intestine are often discovered by accident. This tumor can be multiple and can be associated with other tumors, particularly gastrointestinal malignancy. We present a rare case involving a 68-year-old man in whom we discovered multiple neuroendocrine tumors in the ileum, associated with colonic adenocarcinoma. Following this observation we wish to draw attention to the concept of multiple neuroendocrine tumors and the possibility of synchronous malignancy.RésuméLes tumeurs neuroendocrines (TNE) du grêle sont souvent de découverte fortuite. Elles peuvent être multiples comme elles peuvent s’associer à d’autres tumeurs synchrones, notamment gastro-intestinales. Nous rapportons un cas rare d’un homme, âgé de 68 ans, chez lequel on a découvert, lors de la résection chirurgicale d’un adénocarcinome colique, de multiples TNE du segment iléal. À travers cette observation et après une revue de la littérature, nous insistons sur le caractère multiple des TNE et sur la possibilité d’association à d’autres tumeurs malignes synchrones.

Carcinome papillaire du rein: étude rétrospective à propos de six cas

IntroductionPapillary carcinomas of the kidney are relatively rare tumors. The purpose of this study was to discuss clinicopathological and imaging features of six cases of papillary carcinoma.Material and methodsIn a retrospective study, we studied the clinicopathological and imaging features of six cases of papillary carcinoma retrieved in a 10-years period at the pathology department of F. Hached hospital (Sousse, Tunisia). These tumors were graded according to the new 2004 WHO histopathological classification of renal tumors, on which the prognosis depends.ResultsIn our study, there were five men and one woman. The median age was 68 years, the tumors were asymptomatic in three cases and revealed by lumbar pain in the other three cases. Treatment consisted of radical nephrectomy in five cases and tumorectomy in one case. The tumors measured 15 to 110 mm. The tumor was multifocal in one case. Histopathological examination found type 1 in 3 cases and type 2 in three cases. Metastases were observed in one case.ConclusionRenal papillary carcinoma represents a specific group characterized by their less aggressive behaviour.RésuméIntroductionLes carcinomes papillaires du rein (CPR) représentent une entité anatomoclinique particulière, bien individualisée dans la classification des cancers du rein. Le but de notre travail est de discuter, à partir de six observations, les principales caractéristiques anatomocliniques et thérapeutiques de cette tumeur.Matériel et méthodesIl s’agit d’une étude rétrospective portant sur six cas de CPR, colligés sur une période de dix ans (1997–2006) dans le laboratoire d’anatomie pathologique, CHU de Sousse, Tunisie. Nous avons procédé à une relecture des lames et à une classification des cas en deux types, selon la classification histologique de l’Organisation mondiale de la santé (OMS) 2004. Les aspects histologiques sont confrontés aux données cliniques, thérapeutiques et évolutives.RésultatL’âge moyen des patients (cinq hommes et une femme) était de 68 ans, les signes révélateurs étaient des lombalgies dans trois cas et fortuits dans trois cas. Une néphrectomie élargie était réalisée dans cinq cas et une tumorectomie dans un cas. La taille tumorale variait de 15 à 110 mm, et le caractère multifocal était retrouvédans un cas. La tumeur était de type 1 dans trois cas et de type 2 dans trois cas. L’évolution était favorable dans cinq cas et des métastases étaient retrouvées dans un cas.ConclusionLe CPR est une entité anatomoclinique particulière par son aspect morphologique et son caractère évolutif globalement favorable.
