Lymphangiography in the determination of the extent of metastatic carcinoma. The potential value of percutaneous lymph node biopsy

In determining the extent of disease in patients with carcinoma, lymphangiography when read as positive has an accuracy of 90 to 95%. When considered negative 15 to 20% prove to have metastatic disease. The recent use of percutaneous transperitoneal aspiration biopsy of previously opacified lymph nodes has enhanced the value of lymphangiography. Eighty percent of the aspiration biopsies yield sufficient tissue for cytologic diagnosis.

Comparison of ultrasound and computed tomography in the detection of pancreatic malignancy

A retrospective analysis was performed on 102 patients who were examined by both ultrasound (US) and computed tomography (CT) for known or possible carcinoma of the pancreas. In 38% of the patients, ultrasonography was unsatisfactory due to overlying interfering intestinal gas or ascites, whereas only 2% of CT studies were unsatisfactory due to technical considerations. In comparing the two modalities, CT was found to be more accurate (96% CT vs. 84% US), and this became more significant when nondiagnostic studies were considered in evaluating accuracy (95% CT vs. 54% US). Because of the findings in this analysis, CT is recommened as the initial diagnostic imaging modality for the evaluation of possible pancreatic neoplasm.

Percutaneous retroperitoneal lymph node biopsy in carcinoma of the cervix

Abstract A percutaneous abdominal biopsy of retroperitoneal and pelvic lymph nodes was accomplished in 17 patients with carcinoma of the cervix. The indications for the needle biopsy are: (a) to determine the extent of neoplastic disease and (b) to facilitate treatment planning. The technique is discussed in detail.

---Adenoid cystic carcinoma (cylindroma, pseudoadenomatous basal cell carcinoma) is a malignant tumor of the salivary glands. It constitutes approximately 8% of all salivary gland tumors and arises with a relatively greater frequency in minor salivary glands where the incidence is 66%’ This observation is quite in agreement with the figures a t M. Changes in the Lymphatic Dynamics After Retroperitoneal Lymph Node Dissection

Lymphatic dynamics in the retroperitoneal space are altered after retroperitoneal lymph node dissection. Obstruction with or without visible collaterals is seen frequently. Lymphaticovenous communications and lymphocysts are also common findings. The immediate sequelae are of minor importance, although the mass effect of lymphocysts may be serious. The collateral pathways and lymphaticovenous anastomoses may result in the appearance of metastases in unusual sites.

Lymphangiography in Patients with Malignancy in a Non-Descended Testicle

Lymphangiography was performed on 23 patients with malignancy in non-descended testicles, 14 of whom had seminomatous and 9 non-seminomatous tumors. Lymph node metastases were shown by lymphangiography in 8 patients: 3 in lumbar nodes, 1 in iliac nodes alone and 4 in lumbar and iliac lymph nodes. Microscopic metastases were shown in lumbar nodes at retroperitoneal lymph node dissection in 2 patients when the lymphangiograms were negative. Iliac lymph node metastases are rare in testicular tumors but may be seen in tumors of non-descended testicles, alone or in combination with lumbar metastatic disease. This information is extremely important for the radiologist as well as the clinician in the management of patients.

Endometrial carcinoma: Radiologic assistance in diagnosis, staging, and management

Abstract Hysterography, pelvic pneumography, arteriography, venography, and lymphangiography can be utilized to describe the primary carcinoma of the endometrium and its extension. Hysterography localizes the endometrial neoplasm and delineates the uterine cavity which is of assistance in treatment by radium capsules. Pelvic pneumography will demonstrate the size and contour of the corpus and may suggest extension through the uterine wall. Arteriography and venography more specifically determines mural and contiguous involvement by invasion of the branches of the uterine arteries and veins. Lymphangiography complemented by venography is essential in defining lymph node metastasis which must be established prior to instituting adequate therapy.

Computerized tomography in rhabdomyosarcoma of the head and neck

Rhabdomyosarcoma of the head and neck may be treated with surgery, radiotherapy and chemotherapy or a combination thereof. Precise delineation of the full extent of the tumor in the head and neck is essential. Routine radiographs with hypocycloidal tomography and CT are complementary in defining the full extent of the lesion. Tomography is superior to CT scanning in demonstrating fine bone detail. Demonstration of the soft tissue tumor and its extensions is better seen on CT. The ability to discriminate differences more accurately in densities allows computerized tomography to appreciate better the integrity of the bone margins such as the thin walls of the sinuses. A New Radiological Technique in Evaluation of Prosthetic Fitting

Xeroradiography is an excellent technique in evaluating the fit of extremity prostheses. Because of its inherent characteristics, xeroradiography can clearly demonstrate the stump-socket relationship. Consequently, the degree of contact achieved and attendant pressure problems, if any, can be precisely determined.
