Umbilical serum concentrations of chemokines (RANTES and MGSA/GRO‐α) in preterm and term neonates

Background: The objective of this study was to explore the relationship between labor (preterm and term) and umbilical blood serum regulated on activation, normal T cell expressed and secreted (RANTES) and melanoma growth stimulatory activity/growth‐related oncogene‐a (MSGA/GRO‐α) concentration, and to determine whether early sepsis and pneumonia are associated with changes in concentrations of the chemokines (RANTES and MSGA/GRO‐α) in umbilical blood serum.

Late-preterm cesarean delivery and chemokines concentration in the umbilical cord blood of neonates

Objective: The objective of the study was to investigate whether concentrations of chemokines in the umbilical cord blood of neonates are affected by delivery via cesarean section. Study design: Umbilical cord blood was obtained from 116 singleton late-preterm and full-term neonates without infections, born to healthy pregnant women. Concentrations of chemokines – MIP-1α (CCL3), MIP-1β 1 (CCL4), RANTES (CCL5), GRO-α (CXCL1) and ENA-78 (CXCL5) – were measured by ELISA. Logistic regression was used to investigate regression relationships between the occurrence of neonatal chemokines concentrations in umbilical cord blood and mode and time of delivery. Results: Concentrations of CXC chemokines in late-preterm neonates were the same as those in term neonates. RANTES concentrations in late-preterm cord blood were significantly lower than concentrations in term cord blood. Concentrations of the CC chemokine – RANTES (CCL5) – were noted to be lower in neonates born to cesarean section than in neonates born vaginally. Any anesthetic taken by the mothers during cesarean section did not affect CC chemokine production in the cord blood of full-term neonates. In a logistic regression model including gestational age as a variable, late-preterm delivery was associated with RANTES concentrations (OR = 3.8). After adjustment for variable mode of delivery in regression model, RANTES concentration (OR = 4.75). Conclusion: Both late-preterm and cesarean delivery are essential risk factors of low RANTES (CCL5) concentrations in the umbilical cord blood.

Clinical immunology Quantitative changes of NK cells in umbilical cord blood of neonate in relation to the mode of delivery

Introduction: Natural killer (NK) cells are an important element connecting innate and acquired resistance in humans. They can be detected in fetal liver since the 6 th week of pregnancy and in the sec ond half of pregnancy the number increases to values observed in adults. The main aim of the study was the assessment of the NK cells and defining a relationship between the mode of delivery and NK cells quantitative changes in umbilical cord blood. Material and methods: The study included 72 neonates. Taking into consideration the time and mode of delivery, the children were divided into following groups. Group I included 40 full term neonates: 17 neonates born vaginally and 23 by cesarean section. Group II included 32 near-term neonates: 6 neonates born vaginally, 26 by cesarean section. Assessment of NK cells was performed with a flow cytometry technique. Results: It was shown that the percentage NK cells’ of in all full term neonates was statistically high er than in near-term ones. A higher mean percentage of those cells was also shown in full term and nearterm neonates born by cesarean section. The mean number of NK cells lymphocytes in full term neonates born by elective cesarean section was statistically significantly lower than in neonates born by emer gency cesarean section. Conclusion: Cesarean section can be related to significant quantitative changes in NK cells in the umbilical cord blood. Near-term neonates, regardless of the mode of delivery, show lower values of NK cells in umbilical blood. Elective cesarean section carried out at term can be a cause of decrease of mean number of NK cells in neonate umbilical blood.

Ocena ilościowa limfocytów B i ich subpopulacji we krwi pępowinowej noworodków w zależności od sposobu ukończenia ciąży u ich matek

Streszczenie Cel pracy Ocena ilościowa oraz określenie związku miedzy sposobem rozwiązania ciązy a zmianami ilościowymi limfocytow CD19+ i ich subpopulacji CD5+, CD23+. Material i metoda Zbadano 72 noworodki zdrowe urodzone w Klinice Perinatologii i Ginekologii ŚAM w Zabrzu. Uwzgledniając czas trwania ciązy oraz sposob jej rozwiązania, podzielono dzieci na nastepujące grupy. I Grupa to 40 noworodkow urodzonych o czasie, w tym: 17 noworodkow urodzonych silami natury, 23 noworodki urodzone cieciem cesarskim, w tym: 15 ze wskazan elektywnych, 8 ze wskazan naglych. II Grupa to 32 noworodki urodzone przedwcześnie, w tym: 6 noworodkow urodzonych silami natury, 26 noworodkow urodzonych cieciem cesarskim, w tym: 18 ze wskazan elektywnych, 8 ze wskazan naglych. W badaniach zastosowano metode znakowania krwi pepowinowej z nastepczą lizą krwinek czerwonych. Wyniki U noworodkow urodzonych o czasie wykazano istotnie statystycznie nizszy średni odsetek limfocytow CD5+ w porownaniu z noworodkami urodzonymi przedwcześnie. Podobne roznice wykazano dla ogolu noworodkow urodzonych cieciem cesarskim oraz urodzonych elektywnym cieciem cesarskim. Średni odsetek limfocytow CD23+ byl istotnie statystycznie wyzszy u noworodkow urodzonych o czasie cieciem cesarskim ze wskazan naglych niz u noworodkow urodzonych przedwcześnie tym sposobem. Średnia liczba limfocytow CD23+ u noworodkow urodzonych o czasie cieciem cesarskim ze wskazan elektywnych byla istotnie statystycznie nizsza niz u oworodkow urodzonych o czasie cieciem cesarskim z naglych wskazan. Wnioski 1. Czas ukonczenia ciązy u matki drogą ciecia cesarskiego moze wiązac sie z wystepowaniem u jej noworodka istotnych zmian ilościowych limfocytow B CD5+, CD23+. 2. Ciecie cesarskie wykonane u matki z naglych wskazan moze sprzyjac zwiekszeniu średniej liczby limfocytow B CD23+ u noworodkow urodzonych o czasie.
