Consultation-liaison psychiatric service delivery: results from a European study

The reported findings of the European Consultation-Liaison Workgroup (ECLW) Collaborative Study describe consultation-liaison service delivery by 56 services from 11 European countries aggregated on a C-L service level. During the period of 1 year (1991), the participants applied a standardized, reliability tested method of patient data collection, and data were collected describing pertinent characteristics of the hospital, the C-L service, and the participating consultants. The consultation rate of 1% (median; 1.4% mean) underscores the discrepancy between epidemiology and the services delivered. The core function of C-L services in general hospitals is a quick, comprehensive emergency psychiatric function. Reasons to see patients were the following. deliberate self-harm (17%), substance abuse (7.2%), current psychiatric symptoms (38.6%), and unexplained physical complaints (18.6%) (all means). A significant number of patients are old and seriously ill. Mood disorders and organic mental disorders are most predominant (17.7%). Somatoform and dissociative disorders together constitute 7.5%. C-L services in European countries are mainly emergency psychiatric services and perform an important bridge function between primary, general health, and mental health care.

Interactive patterns of social support and individual coping strategies in melanoma patients and their correlations with adjustment to illness.

Combined patterns of social support and coping style and correlations with adjustment to cancer were investigated in early-stage melanoma patients. The authors studied 358 consecutive patients attending regular follow-up who answered standardized instruments that assess social support, coping behavior, and tumor-related distress. Regression analyses identified high active and low depressive coping behavior as stronger predictors for perceived support than sociodemographic and clinical variables. Cluster analyses yielded four coping-support patterns. High social support, combined either with active coping or with stoicism, was associated with good adjustment, whereas low perceived support in the subjects living alone or in the patients exhibiting depressive coping behavior was associated with poor adjustment.

BACKGROUND In the frame of the European study on quality assurance in consultation liaison psychiatry and psychosomatics (supported by the BIOMED 1 program), the clinical <<process>> of consultation-liaison psychiatry units pertaining to six Spanish general hospitals is analyzed. PATIENTS AND METHOD A sample of 3. 608 consecutive patients referred to the consultation-liaison psychiatry units of five public general hospitals (Clínico of Zaragoza, Clínico of Barcelona, General of Alicante, Ramón y Cajal of Madrid, Princesa of Madrid) and one private gynecological hospital (Dexeus of Barcelona) was studied. The data were recorded with a standardized instrument (CL-BDoK-P), validated in a previous study. RESULTS Consult request took place 10.6 days (on average) after the patients admission (<<lagtime 1>>), half the requests were urgent, and psychiatric consultants examined the patients 1.9 days (on average) after the request (<<lagtime 2>>). The most frequent reasons for referral were current psychiatric symptoms (50.3%), unexplained physical symptoms (15.2%), substance abuse (9.2%), psychiatric history (8.5%), suicide risk (6%) and coping with illness (5.8%). The main referral services were internal medicine (17.5%), traumatology (7.5%) and general surgery (7.3%). An important clinical activity is documented in patients frequently considered to be <<complex>>, with broad spectrum diagnostic and interventions processes and both in-hospital and out-patient follow-up. However, some problems were also detected in the clinical <<process>>. CONCLUSIONS The results outline the clinical importance of Spanish consultation-liaison psychiatry in the general hospital, but the possibility of improving its efficiency through the implementation of integrative models, organizational changes and modern models of <<quality assurance>> is also emphasized.

Repräsentationen früher Bindungsbeziehungen und Emotionsregulation bei Patienten mit Burnout-Syndrom

OBJECTIVE Burnout describes a syndrome of exhaustion resulting from insufficient coping with work-related distress. We investigated if patients that are being clinically treated for burnout show insecure and unresolved attachment representation more often compared with healthy controls. METHODS 50 out of 60 consecutive burnout patients participated in the study. Mental representation of attachment was measured by using the Adult Attachment Interview. Additionally, we administered the Self Report Questionnaire to Assess Emotional Experience and Emotion Regulation and several burnout specific questionnaires. A population sample was used as control group. RESULTS Burnout patients were classified as insecurely attached significantly more often than controls. Unresolved attachment status concerning loss or trauma was found significantly more often within the burnout sample. Patients with insecure attachment representation reported a lower subjective significance of work. Patients with avoidant insecure attachment showed more depersonalisation. Patients with unresolved loss/trauma reported less social support. They showed more passive-negative emotion experience and emotion regulation characterized by externalization. CONCLUSION The results of the study suggest that an insecure or unresolved attachment representation might constitute an intrapersonal risk factor for the development of burnout syndrome.

The relationship between attachment orientations and the course of depression in coronary artery disease patients: A secondary analysis of the SPIRR-CAD trial

OBJECTIVE The relationship between attachment orientations and the recovery from depressive symptoms in patients diagnosed with coronary artery disease (CAD) with and without a psychotherapeutic intervention was examined in this study. METHODS In a multicenter trial of 570 depressed CAD patients (SPIRR-CAD), assigned to usual care plus either a stepwise psychotherapy intervention or one information session, 522 patients provided attachment data at baseline. Attachment was measured with the Relationship Scales Questionnaire (RSQ), yielding four attachment orientations. The primary outcome was change in Hospital Anxiety and Depression Scale depression (HADS-D) scores from baseline to follow-up at 18 months. Secondary outcomes were HADS-D scores at 1, 6, 12, and 24 months. RESULTS Independent of treatment assignment, attachment was related to change in depression at 18 months (p < 0.01) with secure attachment resulting in a significant reduction (-2.72, SE = 0.27) in depression compared to dismissive-avoidant (-1.51, SE = 0.35, p = 0.040) and fearful-avoidant (-0.65, SE = 0.61, p = 0.012) attachment. Patients with anxious-preoccupied attachment showed changes similar to secure attachment (-2.01, SE = 0.47). An explorative subgroup analysis across all assessment time points revealed patients with a dismissive-avoidant attachment benefitted from psychotherapy (average mean difference = 0.93, SE = 0.47, p = 0.048). CONCLUSION Attachment played an important role for improvement in depressive symptoms. Only dismissive-avoidant patients seemed to benefit from the intervention. ZusammenfassungCirca ein Viertel der über 65-Jährigen leidet unter psychischen Erkrankungen. Bei gleichzeitigem Vorliegen somatischer Erkrankungen steigt deren Prävalenz. Erkrankungen, Verluste wichtiger Bezugspersonen und Lebenskrisen im Alter reaktivieren oft traumatische Erfahrungen aus früheren Lebensphasen. Differenzialdiagnostisch kann die Abgrenzung psychischer Störungen von den psychischen Begleitsymptomen somatischer Erkrankungen schwierig sein. Die biografische Anamnese und die Einschätzung der kognitiven Fähigkeiten mithilfe eines geriatrischen Basisassessments gehören deshalb zum Standard der geriatrischen Diagnostik. Psychische Störungen alter Menschen werden häufig übersehen oder inadäquat behandelt. Bei der Therapie mit psychotropen Substanzen müssen Auswirkungen auf somatische Erkrankungen und Arzneimittelinteraktionen bedacht werden. In der Psychotherapie sind ressourcenorientierte und die soziale Unterstützung fördernde gruppentherapeutische Verfahren besonders hilfreich.AbstractApproximately 25% of people over 65 years old suffer from psychiatric disorders but in cases of simultaneously occurring somatic diseases the prevalence is increased. The biographic medical history and estimation of cognitive skills within a geriatric basis assessment should be standard in geriatric diagnostics. Psychological disorders are often overlooked or inadequately treated in older people. In the case of psychopharmacological treatment, effects on somatic sickness as well as drug-drug interactions have to be kept in mind. Hans-Roemer-Preis 2012: Preisträgerin Mechthild Hartmann (Heidelberg) Das Deutsche Kollegium für Psychosomatische Medizin (DKPM) prämiert mit dem Hans-Roemer-Preis seit 1976 herausragende Forschungsarbeiten zur Integration psychosomatischer Aspekte in der klinischen Medizin. Der Hans-Roemer-Preis gilt im deutschsprachigen Raum als wichtigster Forschungspreis auf dem Gebiet der Psychosomatischen Medizin und Psychotherapie. Der Preis basiert auf einer Stiftung durch Dr. med. Elisabeth Roemer in Gedenken an ihren 1973 verstorbenen Ehemann Prof. Dr. med. h. c. Hans Roemer, Ordinarius für Gynäkologie und Geburtshilfe in Gießen und Tübingen. Mit der Stiftung und der jährlichen Preisvergabewird der Wunsch Hans Roemers auf Förderung der klinischen Psychosomatik erfüllt. Prämiert werden in erster Linie Arbeiten aus dem deutschsprachigen Raum. Bevorzugt werden Arbeiten, in denen die Bedeutung psychosozialer Faktoren bei körperlichen Krankheiten wissenschaftlich untersucht oder innovative Modelle psychosomatischer Versorgung in Klinik Praxis und Ausbildung erprobt wird. Die diesjährige Roemer-Preisträgerin ist Dipl.-Psych. Mechthild Hartmann (Heidelberg). [German College of Psychosomatic Medicine (DKPM) rejects take-over plans by the German Association for Psychiatry (DGPPN)].

Das Fachgebiet „Psychosomatische Medizin und Psychotherapie“ wurde 1970 als eigenständiges Fach auf Wunsch der deutschen Ärzteschaft in die Approbationsordnung für Ärzte aufgenommen. Seither hat sich das Fachgebiet „Psychosomatische Medizin und Psychotherapie“ rasant entwickelt und wissenschaftlich etabliert. Die hohe Nachfrage nach psychosomatisch-psychotherapeutischer Versorgung spiegelt sich u.a. darin wider, dass sich die Zahl der stationären psychosomatischen Behandlungen in den letzten 10 Jahren auf über 50000 Fälle pro Jahr verdoppelt hat und eine abgestufte ambulante Versorgung (Psychosomatische Grundversorgung, Zusatzbezeichnung Psychotherapie, FA für Psychosomatische Medizin und Psychotherapie) mit entsprechenden genehmigungspflichtigen Qualifikationen entstanden ist. In der Wissenschaft hat die Psychosomatische Medizin und Psychotherapie substanziell zum besseren Verständnis der Wechselwirkungen zwischen Soma und Psyche und zur Entwicklung von empirisch validierten neuen Therapieverfahren beigetragen. Diese Erkenntnisse sind in die aktuellen Behandlungsleitlinien eingegangen und kommen direkt den betroffenen Patienten zugute. Hochspezialisierte Psychotherapieverfahren sind entstanden. Der Beitrag des Faches Psychosomatische Medizin und Psychotherapie zu Patientenversorgung und Forschung ist international anerkannt und wird als modellhaft für die differenzierte Versorgung von Betroffenen mit psychischen und psychosomatischen Störungen angesehen. Die Psychosomatische Krankheitslehre und Psychotherapeutische Methodik ist in die Lehre curriculär integriert. Am 20. November 2012 hat die DGPPN beschlossen, „Psychosomatik“ in den Namen ihrer Fachgesellschaft zu integrieren. Das „Deutsche Kollegium für Psychosomatische Medizin (DKPM)“ lehnt die Vereinnahmung ihres Fachgebiets durch die DGPPN ab: 1. Die von der DGPPN gewünschte Integration der Psychosomatischen Medizin in das Psychiatrische Fachgebiet wäre ein klarer Rückschritt in der Versorgung und Behandlung der Betroffenen mit psychosomatischen Erkrankungen – ein solcher Schritt ginge somit primär zulasten der Patienten. Das Wissen in der Medizin steigt jedes Jahr sprunghaft an – zudem ist in den letzten Jahren ein steiler Anstieg psychischer und psychosomatischer Erkrankungen festzustellen. Kranke Menschen werden dort gut behandelt, wo professionelles Wissen und Erfahrung mit ihren Krankheiten vorliegen und Betroffene mit ähnlichen Beschwerden behandelt werden. Ohne Qualitätsverlust ist es nicht möglich, in die 5 Jahre dauernde Weiterbildung zum Facharzt für Psychiatrie mit Zusatzbezeichnung Psychotherapie die ebenfalls 5 Jahre dauernde Weiterbildung zum Facharzt für Psychosomatischen Medizin und Psychotherapie zu integrieren. Auch umgekehrt wäre die Integration wenig sinnvoll. Das DKPM lehnt daher den formulierten Anspruch der DGPPN, alle Fachgebiete im Bereich psychischer Störungen vertreten zu wollen, ab.
