Olfactory and Gustatory Functions after Anterior Palatoplasty in Patients with Primary Snoring

Objective In this study, we investigated the chemical senses (namely, olfaction), the 4 basic tastes, and umami taste in patients who underwent anterior palatoplasty (AP) for primary snoring. Study Design Prospective clinical study. Setting Multicenter, tertiary care hospitals. Subjects and Methods The age range of the 28 participants was 22 to 47 years, and all had been diagnosed with primary snoring by polysomnography. Orthonasal and retronasal olfactory testing was performed using the respective “Sniffin’ Sticks.” Tests for the 4 basic tastes (sweet, sour, salty, and bitter) and umami were performed before and 6 months after AP. Results The changes between the preoperative and postoperative results for the 4 basic tastes, which were applied to the anterior tongue region, were not significant. Retronasal olfactory function improved significantly 6 months after AP (P = .007). After AP, umami scores for the soft palate but not those of the anterior and posterior tongue decreased significantly (P = .001). Conclusion Patients scheduled for AP should be properly informed about potential postoperative taste and flavor changes. They should also be warned of a possible loss of appetite due to a postoperative decrease in the umami taste sensation of the soft palate.

The Role of Lifestyle Modifications in the Management of Migraine Associated Vertigo

Aim: To determine potential factors that may trigger vertigo attacks and evaluate the efficacy of lifestyle changes. Material and Method: We ana lyzed medical records of 23 patients with migraine associated vertigo retrospectively. In the management of vertigo, lifestyle modifications, including limitation of the possible triggering factors such as diet, sleep problems, stress, physical activity, and other factors associated with migraine were determined for therapy. Result: Response to therapy was defined as greater than 50 % reduction in symptom recurrence and severity rate. Overall, 69,6 % of patients experienced resolution or dramatic reduction of frequency and severity in symptoms of their vertigo attacks. Discussion: With the high rate reduction of the vertigofrequency and severity, life-style modifications is ef fective therapy for the common problem of migraine associated vertigo.

Comparison of Mometazon Furoate to Cetirizin for Symptom Severity Control and QOL in Allergic Rhinitis

Objectives: Evaluate and compare nasal symptom scores and quality of life (QOL) findings in allergic rhinitis patients. Methods: A total of 426 adult allergic rhinitis patients’ medical records were evaluated retrospectively. Group A consisted of 188 patients and received mometasone furoate nasal spray (100 mcg daily/nostril). Group B consisted of 238 patients receiving cetirizine (10 mg oral/daily) for 30 days. Nasal symptoms (watery discharge from the nose, nasal congestion, sneezing and nasal pruritus) and QOL (Short Form-36) were recorded before and after treatment. Results: When compared before and after treatment, each group’s total nasal symptom scores reduced significantly (Group A, P = .001; Group B, P = .002), and QOL scores improved significantly (Group A, P = .001, Group B, P = .001). But when compared between groups, Group A results were better than Group B (total nasal symptom scores P = .001, QOL scores P = .003). Conclusions: Although intranasal mometasone furoate is a better choice than oral cetirizine, both of these medicines can be used to control allergic rhinitis symptoms and improve patient QOL.

A retrospective analysis of thyroidectomy cases without drain usage: is it really necessary to use drainage routinely in thyroidectomy? -

INTRODUCTION: Routine use of drains may be the source of infection and tissue damage such as recurrent laryngeal nerve damage. In this study, we aimed to investigate thyroidectomy cases without drain usage retrospectively. MATERIAL AND METHODS: Without use of drain thyroidectomy patients undergoing primary benign and malignant diseases are included in the study between November 2007 and January 2012 at Gumussuyu Military Hospital. Second time for any reason, or neck dissection in patients undergoing thyroidectomy surgery were excluded. RESULTS: Patients were mostly woman 32(61.5%), mean age was 34.6 ± 4.3 (23-72) years, discharged postoperatively day first (71%) and second day (39%) respectively, diagnoses were multinodular goiter (n=35), soliter nodul (n=17) and papillary thyroid cancer (n=1). Patients underwent lobectomy (n = 17), near-total thyroidectomy (n = 23) and total thyroidectomy (n = 12). Postoperative seroma were occurred in 2 patients and spontaneously regressed. Wound infection was occurred in a patient. Temporary hypoparathyroidism was occurred in 2 patients. Major complication (hematoma or laryngeal nerve injury) was not occurred. CONCLUSION: In conclusion, since there are low complication rates in these patients and it is well tolerated and shorted hospital stay in cases which well provided bleeding control; routine drain usage is not necessary.

Percutaneous ethanol injection into thyroid cysts

Thyroid nodule which is one of the most frequently encountered thyroid diseases is characterized by growth of abnormal tissues into the thyroid gland. Its incidence varies among populations, and geographic regions. Nowadays, thanks to more frequent use of imaging modalities, thyroid nodules have been detected in individuals with complaints, and many asymptomatic patients as well. The incidence of thyroid nodules in our country is estimated to be 33.3 percent. Thyroid nodules have been classified as solid, and cystic based on the anatomic structure of the lesion. While majority of the nodules demonstrates benign characteristics, malignant nodules have been also reported in various frequencies. Though malignancy is more frequently encountered in solid lesions, many studies have indicated that cystic lesions can be malignant as well, and clinical findings, and demographic data of the patients might fail to discriminate between benign, and malign lesions. Tiroid Nodüllerine Yaklaşım

Thyroid nodules have been identified incidentally in significant proportion of patients parallel to the increase in use of imaging techniques for the diagnosis of diseases. The main point in the evaluation of nodules is to make appropriate diagnosis to differentiate between benign and malignant lesions, and determine which nodule is applicable for the surgery. Management of the patients with thyroid nodules has been presented and discussed briefly in this review.
