Vitamin D status in patients with rosacea

Abstract Background: Rosacea is a common chronic skin condition affecting the face. In recent years, significant evidence shows that vitamin D plays an important role in modulating the immune system. Vitamin D and its analogues via these mechanisms are playing an increasing role in the management of atopic dermatitis, psoriasis, vitiligo, acne and rosacea. Objectives: In our study, we aimed to investigate the relationship between serum vitamin D levels in patients with rosacea and analyze the association of vitamin D with clinical features. Methods: Forty-four rosacea patients and 32 healthy control subjects were included into the study. 25-hydroxyvitamin D (25(OH)D), calcium and intact parathyroid hormone were measured. Deficiency of vitamin D is defined as the level of 25(OH)D being less than 20 ng/ml. Results: Thirty-three female and 11 male patients were included in the study. The mean age of patients was 48.6 ± 11.5. The mean levels of vitamin D levels were found as 21.4 ± 9.9 and 17.1 ± 7.9 in patients and controls, respectively. The difference was statistically significant (p = 0.04). The prevalence of vitamin D deficiency in patients with rosacea was 38.6% and 28.1% in healthy controls (p = 0.34). Conclusions: To the best of our knowledge, our study is the first study for evaluating serum vitamin D levels of patients with rosacea in the literature. Patients with rosacea have relatively high serum vitamin D levels compared to control groups. The result of our study suggests that increased vitamin D levels may lead to the development of rosacea. To confirm status of vitamin D levels in patients with rosacea, larger epidemiological studies are needed.

Irritable bowel syndrome in patients with chronic pruritus of undetermined origin.

Necrotizing fasciitis in a patient with bullous pemphigoid treating with systemic steroid

Abstract Necrotizing fasciitis (NF) is a rarely seen, life-threatening soft tissue infection characterized by progressive necrosis of skin, subcutaneous tissues and fasciae. Herein, we present a case of NF in a patient with bullous pemphigoid. A 78-year-old female patient was admitted with the left leg pain, swelling, redness and blistering. Streptococcus pyogenes was isolated from the wound culture. In the skin biopsy and the left leg magnetic resonance imaging, signs of NF were observed. The diagnosis of NF was considered. Then, urgent surgical debridement was performed by the department of plastic surgery. While under treatment with intravenous antibiotics therapy, unfortunately the patient died of acute renal insufficiency.

A case of atypical scleromyxedema mimicking angioedema which responded well to acitretin treatment

A 37-year-old male patient was admitted our clinic with the complaints of edema and mild pruritus of the eyelids, ears, and the neck. On dermatological examination, there were edema and pale erythema on the eyelids and ears as well as coarsening of the facial features and lines grooving the forehead. His medical history revealed that the patients received systemic corticosteroids, antihistamines and epinephrine treatments for about two months with the diagnosis of urticaria and angioedema, however, he did not benefit from these treatments. A diagnosis of atypical scleromyxedema without monoclonal gammopathy was established according to the clinical, histopathological and laboratory findings. Acitretin treatment with a dose of 35 mg/day was started. Marked regression in the lesions was observed two months later. (Turkderm 2015; 49: 81-4)

Psoriasis-related conditions that involve the eye are mainly blepharitis, conjunctivitis and keratitis and these conditions are seen in about 10% of psoriasis patients. However optic neuritis and pathologies of visual fields are rarely seen. Our study aims to investigate the mean retinal nerve fibres layer (RNFL) thickness using OCT in patient psoriasis. Methods: A total of 40 patients with psoriasis and 35 controls were included in our study. The average retinal nerve fibre layer thickness was measured using the optic disc cube protocol (200x200) and the Cirrus SD-OCT (Carl Zeiss Meditec, Dublin, CA) device. The statistical analyses were performed using the SPSS v 19 software programme. Results: The mean psoriasis area and severity index (PASI) was 8.04 and the mean disease duration was 8.9 years. The average RNFL thickness was not statistically different between the patients and the controls, and there was no correlation between the mean RNFL thickness and the severity and duration of the disease. Conclusion: To our knowledge this study is the first assessment of RNFL thickness in patient with psoriasis. However, of greater scope are needed to investigate possible retinal changes in such patients.

Koebner Phenomenon as a Complication of Tattoo; a Case Diagnosed as Psoriasis

Koebner fenomeni, diğer adıyla isomorfik fenomen derinin travmatik yada nonspesifik olarak uyarılması sonucu dermatolojik hastalığın uyarılma bölgesine sınırlı şekilde ortaya çıkması şeklinde tanımlanabilir (1,2). Bu uyarılma fiziksel ve termal travmalar (cerrahi insizyon, friksiyon, böcek ısırıkları), dermatozlar (dermatitler, dermatolojik enfeksiyonlar), alerjik ve irritan reaksiyonlar (fotosensitivite, yama testi, tatuaj, çizme testi) ve tedaviler (radyoterapi, fototerapi) ile gelişebilir (1,3). Başta psoriasis olmak üzere liken planus, vitiligo gibi inflamatuvar hastalıklarda iyi tanımlanmıştır (1). Uyarılmadan sonra lezyon ortaya çıkışı 2 gün ile 2 yıl arasında değişebilir. Ortalama 2 hafta sonra gelişir (1). Nadiren yıllar sonra keloid yerinde bildirilmiş Koebner fenomeni olguları da literatürde yer almaktadır (4). Tatuaj çok uzun yıllardır çeşitli pigment maddeleri ile deriye resim yapma sanatı olarak kullanılmaktadır. ‘Tattoo’ Grönland Eskimoları tarafından kullanılan bir kelimedir ve buradan dünyaya yayılmıştır (5). İlk kez 1853 yılında Hutin tarafından tatuaj’a bağlı cilt komplikasyonları görülebileceği bildirilmiştir (6). Burada sunmak istediğimiz olgu yirmi yaşında erkek klinik muayenesinde diz ve dirsek bölgesinde eritemli skuamlı plaklar görüldü (Resim 1). Dermatolojik muayenesinde mum lekesi ve Autspitz fenomenleri pozitif olarak izlendi. Klinik olarak plak psoriasis tanısı konulan hastanın dermatolojik muayenesinde sağ ön kol fleksör yüz ile sol kol ekstansör yüzde tatuaj bulunuyordu. Tatuaj üzerinde, tatuaj sınırlarını geçmeyen ve üzerini tamamen kaplayan eritemli skuamlı plaklar dikkati çekti (Resim 2, 3). Klinik olarak mum lekesi ve Autspitz fenomenleri pozitifti. Tatuaj bir buçuk ay önce yaptırılmıştı ve lezyonlar tatuaj yapıldıktan 1 hafta sonra ortaya çıkmıştı. Koebner fenomeni 1877’de Henrich Koebner tarafından psoriasisli bir hastada tanımlanmıştır (7). Önceki çalışmalar psoriasis hastalarında %25 oranında Köbner fenomeninin görüldüğünü göstermiştir (7). Boyd Koebner fenomenini, Koebner’in gerçek izomorfik yanıtı, Koebner’in yalancı izomorfik yanıtı, zaman zaman travmatik lokalizasyonlu lezyonların ortaya çıkması ve zayıf ve sorgulanabilir travma ile ilişkili durum olarak dört gruba ayırmıştır (3). Familial hypercholesterolemia (FH) is an inherited genetic disorder leading to increased plasma LDL levels due to defects in LDL-cholesterol metabolism. Atherosclerosis occurs early due to the high serum lipid levels and inability to eliminate LDL-cholesterol from circulation. This results in coronary artery disease and atherosclerotic aorta at early age. Atheromatous alterations in aortic root lead to coronary osteal lesions and irregularities in the inner aortic wall. We present an FH case with extensive involvement of ascending aorta with coronary ostial lesion. Keywords: Familial hypercholesterolemia, left main coronary artery
