Relationships of beta-blockers and anxiolytics intake and salivary secretion, masticatory performance and taste perception

OBJECTIVE Assess the influence of salivary flow on physiological parameters of the stomatognathic system in patients who take beta-blockers or anxiolytic medications. DESIGN Sixty patients were divided into three groups based on the following criteria: Group 1, control (n=20; no use of medication); Group 2, use of antihypertensive beta-blockers (n=20); and Group 3, use of benzodiazepine anxiolytics (n=20). Salivary flow was assessed by determining stimulated and non-stimulated flow/minute. The quantification of the sense of taste was determined on a visual analogue scale (VAS) using solutions of 0.9% NaCl (salty), 50% sucrose (sweet), 20% unsweetened coffee (bitter) and 4.2% vinegar (sour). The DMFT index (number of decayed/missing/filled teeth) was determined by a calibrated examination, following the criteria of the World Health Organization (WHO). Masticatory performance was assessed with an Optosil comminution test and Rosim-Ramler equation. RESULTS The results did not reveal a significant correlation between salivary flow and masticatory performance (p>0.05). We observed significant decreased non-stimulated salivary flow for Group 2 (p=0.05) when compared to controls. However, taste perception was not influenced by salivary secretion amongst groups. Furthermore, we observed a significant negative correlation between non-stimulated salivary flow and DMFT in Group 1 (p=0.02; r=-0.52). CONCLUSIONS Patients under beta-blockers therapy presented reduced non-stimulated salivary flow when compared to controls, without influencing the sense of taste or masticatory performance. The use of anxiolytics did not affect salivary flow and taste perception in the studied sample.

Prevalence of Nickel Allergy and Longitudinal Evaluation of Periodontal Abnormalities in Orthodontic Allergic Patients

OBJECTIVE To determine the prevalence of nickel allergy in a sample of orthodontic patients and longitudinally compare the clinical periodontal status of these individuals with that of a group of nonallergic patients. MATERIALS AND METHODS The initial sample consisted of 96 patients selected randomly from a databank of patients who sought orthodontic care at a teaching institution. Following the selection and beginning of treatment, periodontal status was assessed over a 12-month period (one evaluation every 3 months-T(1), T(2,) T(3,) T(4)) using the Loe index. The evaluations were performed blindly by a single, calibrated examiner and were followed by prophylaxis and orientation regarding oral hygiene. The prevalence of nickel allergy was determined by the patch test 9 months after the beginning of treatment and occurred in 16 individuals (17.2%). Two groups were then established: the allergic group (AG, n = 16) and the age-paired nonallergic control group (NAG, n = 16). Data were analyzed using the Mann-Whitney U-test and Friedmans analysis of variance (ANOVA) tests (P < or = .05). RESULTS Significant differences were present between groups at the T(3) and T(4) evaluations for the LOE index (P = .045 and .009), with allergic individuals showing higher mean values than nonallergic individuals (hyperplasia, change in color, and bleeding). No significant differences were found in the intragroup evaluations between the four evaluations (P > .05). CONCLUSIONS The results suggest a cumulative effect from nickel throughout orthodontic treatment associated with clinically significant periodontal abnormalities in allergic individuals over time.

Candida spp. biotypes in the oral cavity of school children from different socioeconomic categories in Piracicaba - SP, Brazil

Two hundred and thirty-nine (239) Brazilian children, distributed into five distinct socioeconomic categories (A to E) were studied. Saliva samples were analyzed as to flow rate, pH, buffer capacity and microbial parameters. The results revealed the presence of Candida spp. in 47.3% of the samples. The most commonly isolated species was C. albicans, in all socioeconomic categories, followed by C. tropicalis, C. krusei and C. parapsilosis. There was no statistical correlation between secretion rate, buffer capacity and Candida spp. CFU/ml. The prevalence of Candida spp. did not differ substantially among the groups; however the microorganisms were more detected in categories B and C. Among all species, C. albicans was the most prevalent. Only 5% of the sample presented more than one species--C. albicans associated with C. tropicalis, C. parapsilosis or C. krusei. It was possible to detect a significant correlation between caries indices and the socioeconomic categories. All categories presented increased caries indices; however the studied population was considered of low caries risk. There was no positive correlation between the presence of Candida and caries risk in the analyzed population.

Detection and Clonal Analysis of Anaerobic Bacteria Associated to Endodontic-Periodontal Lesions

BACKGROUND Microbial agents in root canal systems can induce periodontal inflammation. The aims of this study are to detect anaerobic microorganisms in endodontic-periodontal lesions, determine the genetic diversity among them, and assess the simultaneous colonization of the pulp and periodontal microenvironments by a single clone. METHODS Twenty-seven teeth of patients with endodontic-periodontal lesions were selected. Samples were spread on an agar-blood medium, the detection of each species was performed using a polymerase chain reaction, and the determination of the simultaneous presence of the same species in the microenvironments by one or more clones was determined using arbitrarily primed PCR. RESULTS Prevotella intermedia (Pi) was the most prevalent species of the colonies in periodontal pockets, whereas Porphyromonas gingivalis (Pg) and Pi were the more prevalent in root canals. Isolates of Pi and Pg were simultaneously identified in root canals and periodontal pockets. Eighteen percent of teeth exhibited the simultaneous colonization by Pg, Tannerella forsythia (previously T. forsythensis), and Porphyromonas endodontalis in the pulp and periodontal microenvironments. The presence of these species was noted even in niches from which no colonies were isolated. Seventeen different genotypes were found in periodontal and pulp sites, with the majority of sites colonized by one or two different genotypes. A high degree of genotype similarity was found for samples of Pg isolated from only one site as well as for those isolated from both microenvironments. CONCLUSION Different clones of Pi and Pg with a high intraspecific genotype similarity were found to colonize the same anatomic sites in endodontic-periodontal infections.

Analysis of parity between protein-based electrophoretic methods for the characterization of oral Candida species

Electrophoretic studies of multilocus-enzymes (MLEE) and whole-cell protein (SDS-PAGE) were carried out in order to evaluate the parity between different methods for the characterization of five Candida species commonly isolated from oral cavity of humans by numerical taxonomy methods. The obtained data revealed that sodium dodecyl sulfate polyacrylamide gel electrophoresis is more efficient in grouping strains in their respective species while MLEE has much limited resolution in organizing all strains in their respective species-specific clusters. MLEE technique must be regarded for surveys in which just one species of Candida is involved.

Presença de Streptococcus mutans e Streptococcus mutans associado a Streptococcus sobrinus em escolares de diferentes classes sócio-econômicas e sua relação com a atividade cariogênica dessas populações

The group mutans streptococci, more predominantly isolated of salivary samples, have been the S. mutans and S. mutans/S. sobrinus. The detection of these microorganisms in students of different socio-economic levels and their relationship with the cariogenic activity of these populations was studied, correlating them with the clinical parameters DMFT, DMFS, dmf and dmfs. Two hundred (200) students belonging to five socio-economic levels were distributed in sub-groups, according to the species of microorganisms identified in their salivaries samples. Of all children analysed, 103 (51.0%) presented only S. mutans in the saliva, 33 (17.0%) the association S. mutans/S. sobrinus, and 64 (32.0%), other group mutans species (individually or in association). The statistic evaluation of the available data related to the number of children with dental caries showed, at first, values of 79.0% (81) e 76.0% (25), respectively, to the sub-groups S. mutans and S. mutans/S. sobrinus, showing a high cariogenic index. For the individuals colonized by S. mutans/S. sobrinus and only S. mutans, means 9.40 and 6.70, were seen respectively, to the dmfs index, suggesting that the association S. mutans/S. sobrinus is potentially more cariogenic than the colonization only by S. mutans.

Differentiation and numerical analysis of oral yeasts based on SDS-Page profiles. Influence of the culture media on the whole-cell protein extracts

The application of gel electrophoresis and numerical analysis of yeast soluble proteins analysis to the investigation of 12 oral yeast strains belonging to five species is described. It involves one-dimensional electrophoresis of SDS-solubilized whole-cell proteins using different culture media for the cultivation of the cells, integration densitometries in the areas of the gels and percentages of the proteins extraction. These extracts were prepared from four isolates of Candida albicans, two of C. tropicalis, C. guilliermondii, C. parapsilosis and C. krusei. The extracts from whole-cells proteins using different culture media for the cultivation of the cells were fractionated by slab electrophoresis using a discontinuous buffer system. The corresponding patterns showed at least 36 polypeptides in the range of 14.4-200 kDa. Different isolates of each species were clearly different in each of the five species. The data obtained suggest that different nutritional compositions led to the expression of different proteins derived from alternatives metabolic pathways expressed by the electrophoretic profiles. The construction of a database of protein fingerprints and numerical analysis based on such data, may have some implications in the classification and identification of such species with epidemiological, ecological and taxonomic purposes. A well defined or synthetic culture media seems to be much properly.

Cellular, humoral, and histopathologic analysis in rats implanted with orthodontic nickel brackets.

OBJECTIVE To determine the immune response related to nickel and nickel-free orthodontic appliances using cellular (total and differential leukocytes) and humoral (IgA) quantification. MATERIALS AND METHODS Thirty-five Wister rats were randomly distributed into three groups (A, B, and C). In the A group, the animals were subdivided into three subgroups (A1: control, without brackets; A2: nickel-free brackets; and A3: nickel brackets) with seven animals in each group; the animals were sacrificed after 14 days. Groups B and C received only nickel brackets and were kept for 14 days. After this period, the implants were removed and the rats were sacrificed. In group B, the animals were sacrificed 2 days after the appliance removal (16 days in total); and in group C, the animals were sacrificed 7 days after the brackets removal (21 days in total). Total and differential leukocyte number, IgA quantification and histopathologic analysis were done. Histopathologic analysis of subcutaneous tissue related to the orthodontic brackets was done as well. RESULTS There were significant differences between the number of leukocytes for the nickel-implanted animals and the nickel-free and control groups after 14 days of implantation (P < .05). However, there was no significant difference among groups A1, A2, and A3 for the differential number of leukocytes and for the IgA quantification, except for the number of monocytes, which was three times higher in the nickel group (A3). The histopathologic findings did not show differences between groups. CONCLUSIONS Nickel did not promote significant adverse biological effects.

Influence of disinfectant solutions on test materials used for the determination of masticatory performance

Masticatory function can be evaluated objectively as the capacity of an individual to fragment solid food after a fixed number of chewing cycles, the so-called masticatory performance (MP). The objective of this study was to evaluate the reliability of four different test materials (Optosil, Optocal, Zetapuls, and Perfil) and five disinfection protocols by aspersion and immersion (no disinfection, 2% glutaraldehyde, 2% chlorhexidine, 5.25% sodium hypochlorite, and 70% alcohol) on the MP, determined at three moments (24 hours, 15 and 60 days) after storing the fragmented blocks. MP was evaluated by calculating X50 through the sieving technique and the Rosim-Ramler equation. The weight and microbiologic count (colony forming units, CFUs) of chewed blocks were measured to identify any variations that would make MP determination unfeasible. Differences in MP were observed among the materials (p < 0.01). Perfil presented the highest X50 value (worst MP determination), followed by Zetaplus (both p < 0.05), Optosil, and Optocal (both p > 0.05). The time and disinfection type had no influence on MP (p > 0.05). The number of CFUs differed between the nondisinfected group and all other disinfection groups at all time points (p < 0.01). No other significant difference in CFU count between disinfection groups was observed. In conclusion, disinfection did not alter the reliability of the test materials for the MP calculation for up to 60 days.

IN VITRO BACTERIAL PLAQUE SUPPRESSION AND RECOLONIZATION BY S. MUTANS AND S. SOBRINUS

The in vitro study of the interactions between S. mutans and S. sobrinus is important to determine the role of these microorganisms in the formation of biofilms on dental structures and their potential to induce carious lesions. The objective of this research was to study the suppression of bacterial plaque formation and its recolonization by rifampycin-resistant S.mutans and streptomycin-resistant S. sobrinus. To study the competitive relationship between these species, previously standardized strains were incubated in media containing different fermentable carbohydrates. At determined time intervals, samples were collected from mixed cultures of S. mutans and S. sobrinus, diluted and plated on BHI-agar containing rifampycin or streptomycin to determine the number of viable cells of each species by counting colony-forming units. In order to study the bacterial colonization process and in vitro recolonization of bacterial plaque, three experiments were performed: I - co-cultivation of S. mutans and S. sobrinus; II - inoculation of bacterial plaque pre formed by S. sobrinus with S. mutans; and III - bacterial plaque pre formed by S. mutans dispersed and plated on BHI- agar containing streptomycin or rifampicin to determine the number of viable cells for each species. The results indicated a predominance of S. mutans in relation to S. sobrinus, demonstrating the capacity of S. mutans to inhibit plaque formation by S. sobrinus and recolonize the surfaces.
