Objective The synaptosomal-associated protein of 25 kDa (SNAP-25) gene is a presynaptic plasma membrane protein and an integral component of the vesicle docking and fusion machinery mediating secretion of neurotransmitters. Previously, several studies reported association between SNAP-25 and attention deficit hyperactivity disorder (ADHD). We investigated whether these SNAP-25 polymorphisms (MnlI T/G and DdelI T/C) were also associated with ADHD in the Turkish population. Methods Our study comprised unrelated 139 subjects who met DSM-IV criteria for ADHD and 73 controls and all were of Turkish origin. Genetic analyses were performed and patients were evaluated with Wender-Utah Rating Scale and Adult ADD/ADHD DSM IV-Based Diagnostic Screening and Rating Scale. Results SNAP-25 DdelI polymorphism was not associated with ADHD but there was a statistically significant difference between ADHD patients and controls for SNAP-25 MnlI polymorphism. For SNAP-25 MnlI polymorphism patients with G/G genotype of the SNAP-25 gene MnlI polymorphism had higher Wender-Utah scores and higher scores in the 1st and 3rd parts of adult ADD/ADHD Scale. Conclusion We detected a significant association of the MnlI polymorphism in our ADHD sample which was similar to previous findings. Our study also revealed that SNAP-25 MnlI polymorphism was also associated with symptom severity of ADHD. This study is also, the first report on the association of SNAP-25 with ADHD in the Turkish population.

Venous thromboembolism (VTE) is serious medical condition, which might be caused by psychotropic medications. Previously, antipsychotic-induced VTE due to olanzapine, risperidone, clozapine, and amisulpiride was reported. In this report, we present 2 cases of paliperidone-induced VTE.

Objective: Nicotine addiction is the most prevalent addiction type all over the world and attention deficit hyperac-tivity disorder (ADHD) is an important factor associated with nicotine addiction. Primary aim of this study was to evaluate the effect of ADHD symptoms on smoking cessation. Methods: Smokers who had admitted to the smoking cessation center were evaluated with using Wender Utah and Fagerstorm Scales at the beginning of study and for the success of smoking cessation at the end of 6 months. Results: Of the 353 smokers enrolled to the study, 99 individuals had a score of 36 or higher on Wender Utah scale (ADHD symptoms group). Of these 99 patients with ADHD symptoms, 11 (11.1%) and of the 254 non-ADHD symptoms group, 68 (26.8%) had quitted smoking after a period of six months. Logistic regression analysis revealed that having ADHD symptoms predicted failure of smoking cessation (adjusted odds ratios 2.12, 95% confidence interval 1.02-4.40) after controlling for sociode-mographic and smoking-related variables. Conclusions: ADHD symptoms may be an important factor affecting smoking cessation. Examining these symptoms in problematic nicotine addicts might help treatment outcome.

Purpose:Previous studies showed high percentage of alcohol and tobacco use in university students. But ratio of problematic tobacco and alcohol users weren’t handled sufficiently in many studies. Clinical trials revealed that using valid and reliable screening tests were better for estimating the problematic alcohol and tobacco users. The aim of study is to investigate the prevalence of alcohol and tobacco use and determine the ratio of problem drinkers and smokers in university students. Materials and methods:The sample of the study consisted of 371 university students (181 boys and 190 girls) of Pamukkale University. CAGE (Cut down, Annoyed, Guilty, Eye opener) and Fagerstrom Tolerance Test for Nicotine Dependency was used to detect problematic alcohol and tobacco users. Results:132 (35.6%) students were using tobacco and 131 (35.3%) were using alcohol. 48 (36.4%) smokers had 5 or more points from Fagerstrom test, 34 (26.2%) alcohol users had score of 2 or more from CAGE questionnaire and 12 students had 5 or more points from Fagerstrom test and 2 or more points from CAGE. Conclusion: Despite all efforts on reducing alcohol and tobacco use, it was still an important issue in university students. 1/3 of smokers and 1/4 of alcohol users were in risk for problematic tobacco and alcohol use. Increasing the preventive and therapeutic actions for university students might be helpful for decreasing the development of dependency in this group. Pam Med J 2015;8(2):82-87

Kırkdört yıllık iyileşme dönemi sonrası tekrarlayan mani: Bir olgu sunumu

Ceyhan Balcı Şengül; Cem Şengül

It is widely believed that bipolar disorder is typically a progressive condition, marked by shortening of stable mood periods and of inter-episode cycles over time and increasing number of episodes. However, in many bipolar patients an erratic and unpredictable course of illness may occur. Experienced clinicians are familiar with some patients, whom have not suffered from a relapse of illness for years despite lack of prophylactic treatment. In this report we present a case of mania with 44 years of remission without prophylactic treatment and discuss the preventive therapy in bipolar disorder. It is widely believed that bipolar disorder is typically a progressive condition, marked by shortening of stable mood periods and of inter-episode cycles over time and increasing number of episodes.
