ABSTRACT Acinetobacter baumannii is most often responsible for hospital-acquired infections and is occasionally associated with community-acquired infections. We report two cases of A. baumannii endophthalmitis, one with endogenous endophthalmitis and the other with postkeratoplasty endophthalmitis. Although endophthalmitis is rare, ophthalmologists should be alert to the possibility of patients having endophthalmitis caused by A. baumannii.

PurposeTo report a case of a mixed open globe injury in which a retained large iron nail and perforating injury of the eye coexisted.MethodsCase report.ResultsA 26-year-old construction worker was injured by a nail while using a nail gun at work. The iron nail was inadvertently stabbed into the left eye from the upper eyelid and superonasal sclera to temporal sclera, with lens involvement. His visual acuity was counting fingers in the left eye. Primary suture, removal of a retained iron nail, pars plana vitrectomy and lensectomy, and laser photocoagulation were performed. Three months later, an intraocular lens was implanted in the ciliary sulcus. During postoperative 6-month follow-up, final best-corrected visual acuity was 20/20 without complication.ConclusionWe describe one patient with a mixed open globe injury of a retained nail and perforating injury of the eye resulting from a nail gun. As most eye injuries are preventable, proper training of nail-gun operators and use of safety equipment could reduce the incidence of ocular injuries. Expeditious removal of nails and repair of the lacerations can prevent the complications and achieve good final visual outcomes.

ABSTRACT Melioidosis is an infectious disease caused by Burkholderia pseudomallei, a gram-negative bacillus. We report a case of endogenous endophthalmitis caused by B. pseudomallei that was treated with systemic and intravitreal ceftazidime. Finally, the patient achieved a final visual acuity of 20/60 without complication.

Keratoconus is a challenging disease for early diagnosis and management. We review the literatures of clinical characteristics and contact lens fitting of keratoconus. The aim for contact lens fitting is to gain the best corrected vision and provide the maximal comfort for patients over a prolonged period of wearing.

Purpose: The aim of this study was to describe a modified technique which created a manmade capsular support for posterior chamber intraocular lens (PCIOL) implantation in an aphakic eye without posterior capsular. Methods: One 10-0 polypropylene suture was used to make two over-lapping but opposite faced equilateral triangles (which formed a 6-rayed star shaped scleral fixation net) as a man-made capsular support for PCIOL implantation on a twenty-year-old female patient who had surgical aphakia since one year of age. Results: The PCIOL remained well centered and neither lilting, nor complication was noted after the surgery. Conclusion: The surgical technique for double triangle (6 rayed star shaped) scleral fixation net is easier than the sutured PCIOL surgical technique. Double triangle fixation also provides a better support for PCIOL and might result in less tilting problems.
