Causas externas entre menores de 15 anos em cidade do Sul do Brasil: atendimentos em pronto-socorro, internações e óbitos

OBJETIVO: Identificar causas de atendimento hospitalar ou de morte e lesoes entre menores de 15 anos vitimas de causas externas (acidentes ou violencias), residentes em Londrina, Parana, em 2001. METODO: Estudo transversal, com coleta retrospectiva de dados. As informacoes sobre morbidade foram coletadas em prontuarios dos cinco hospitais gerais da cidade, e as de mortalidade no Nucleo de Informacao em Mortalidade do Municipio, com classificacao dessas causas e do trauma gerado de acordo com a Classificacao Internacional de Doencas. RESULTADOS: Foram estudados 8.854 menores de 15 anos, com 95,7% de atendimento em pronto-socorro e alta. Entre os atendidos em pronto-socorro, com alta, houve predominância de quedas (33,9%), seguidas de eventos de intencao indeterminada (31,8%) e de acidentes causados por forcas inanimadas (15,5%), principalmente por penetracao de corpo estranho em orificio natural (4,6%) ou atraves da pele (3,2%). Nesse nivel de atencao predominaram traumatismos superficiais (32,9%) e ferimentos (29,3%). Quedas tambem foram as principais causas entre os internados (32,4%), seguidas de acidentes de transporte (19,5%), acidentes causados por forcas inanimadas (15,7%) e envenenamentos (13,5%). Entre os internados, as principais lesoes foram traumatismos superficiais (22,0%) e fraturas (19,5%). Acidentes de transporte (44,4%) e afogamento (16,7%) foram as principais causas externas de obito e o traumatismo crânio-encefalico a principal lesao fatal (50,0%). CONCLUSOES: As causas e os tipos de lesao diferem segundo o nivel de atencao e indicam a necessidade de reestruturacao dos servicos de saude visando descentralizar o atendimento as lesoes de menor complexidade.

Queimaduras em crianças e adolescentes: análise da morbidade hospitalar e mortalidade

Objective: this research analyzed the hospital incidence and mortality due to burns in children aged less than 15 who live in Londrina, and were treated in emergency services, hospitalized or who died because of this in 2001. Methods: This is a descriptive study. Morbidity data were obtained from the five geral hospitals of Londrina, and mortality data came from the Municipal Mortality Information Nucleus of the city. The Software Epi Info, version 6.04d was used for data analysis. Results: 182 Children under age 15, victims of burns have been studied, being 82.4% wounded by hot materials or heat source, 14.3% by exposition to smoke/fire/flame and 3.3% by exposition to electric current. Male victims were predominant (56.6%), with the highest coefficient for the age of one year (6.1 per 1000 children). The hospitalization rate was 12.6%; one death occurred. 56.5% of the cases stayed in hospital for one to three days. Acidentes na infância e adolescência: uma revisão bibliográfica

This study aims at reviewing the literature on accidents among young under the age of 15. Throughout Brazil, lesions and poisonings are responsible for 5 to 6 percent of total internments by the National Health System (SUS), generating 8 percent of the total hospital internment expenses. Sweden and Italy are the countries that have the lowest mortality rates related to external causes among youngsters under the age of 15. The rates of Denmark, Germany and Japan are regarded as intermediate. France, Belgium, Austria and Canada are regarded as countries with high mortality rates related to external causes among youngsters under the age of 15. Brazil together with the United States, Portugal, Mexico and Korea are considered countries with very high mortality rates related to childhood accidents, needing preventive actions. Furthermore, studies reinforce the importance of recognizing the nature and reality of childhood accidents in order to form a diagnosis that helps elaborate and accomplish specific prevention policies.O estudo objetivou apresentar uma revisao bibliografica dos acidentes entre menores de 15 anos. Em todo o Brasil, as lesoes e envenenamentos sao responsaveis por 5 a 6% do total das internacoes conveniadas pelo Sistema Unico de Saude, gerando gastos hospitalares de 8% do total de gastos com as internacoes. Os paises com menores taxas de mortalidade por causas externas em menores de 15 anos sao a Suecia e Italia. A Dinamarca, a Alemanha e o Japao sao considerados paises com taxas intermediarias. A Franca, a Belgica, a Austria e o Canada sao considerados paises com taxas altas de mortalidade por causas externas em menores de 15 anos. O Brasil esta, junto com os Estados Unidos da America, Portugal, Mexico e Coreia, entre os paises com taxas muito altas de obitos por acidentes em menores de 15 anos. A literatura aponta para a magnitude dos acidentes infantis, necessitando-se de acoes preventivas. Os estudos enfatizam, ainda, a importância de se conhecer a natureza e a realidade acerca dos acidentes na infância a fim de se formar um diagnostico que auxilie na elaboracao e implantacao de estrategias especificas de prevencao.

Envenenamentos acidentais entre menores de 15 anos em município da Região Sul do Brasil

This study analyzes the characteristics of accidental poisoning among children and young adolescents (< 15 years) in Londrina, Paraná State, Brazil, in 2001. Data were obtained from patient records in general hospitals, the local Poison Control Center, and the municipal Mortality Information Center. There were 473 cases of poisoning, with an incidence rate of 339.8 per 100,000, 58.5% due to poisonous substances and 39% due to venomous animals and plants. Increased risk was observed in the 1-3-year age group, due to poisonous substances. Medications (47.5%), pesticides (14.1%), and cleaning products (11.3%) were the main substances involved (284 cases), and 17.2% of children were hospitalized. Among 189 cases of contact with venomous animals and plants, caterpillars (29.1%), bees (25.9%), and spiders (22.8%) were the main agents, with a 1.1% hospitalization rate. Incidence of poisoning is high, and there are important differences in the types of agents involved according to age group, which may help orient the prevention of such accidents.

Epidemiologia dos acidentes e violências entre menores de 15 anos em município da região sul do Brasil

This study aimed to analyze the epidemiological characteristics of accidents and violence against children under 15 years old who lived in Londrina, a city in the South of Brazil, in 2001. Morbidity data were collected from general hospitals records and mortality data were obtained from the Municipal Mortality Information Center. A total of 8,854 children were studied, which corresponded to an incidence rate of 74.8/1,000 children. Admission and fatality rates were 4.2% and 0.2%, respectively. The incidence rate was higher among two-year olds (109/1,000) and male victims prevailed (60.7%). Other causes of injuries were the main subtype of external cause (61.0%), followed by events of undetermined intention (30.6%) and by transport-related injuries (7.5%). The head was the most affected body part (34.9%) and superficial injuries were the most frequent kind of trauma (32.4%). The results contribute to the planning of injury control and prevention actions.Objetivo: analisar as caracteristicas epidemiologicas dos acidentes e violencias em menores de 15 anos, residentes em Londrina, Parana, em 2001. Os dados de morbidade foram obtidos em prontuarios dos hospitais gerais da cidade e, os de mortalidade, no Nucleo de Informacao em Mortalidade. Foram estudadas 8854 criancas, representando incidencia de 74,8/1000 criancas. As taxas de internacao e de letalidade foram, respectivamente, de 4,2 e de 0,2%. Descriptive study of falls among children aged under 15 in the municipality of Londrina, Paraná state

This study seeks to analyze from an epidemiological standpoint the occurrence of falls among children aged under 15 living in Londrina, Parana, who were treated in emergency departments, were interned or died due to such events in 2001. Data were obtained from general hospital registries and from the Mortality Information Nucleus. 2,991 child victims of falls were analyzed, representing an incidence rate of 25.3 per 1000 children. The non-fatal internment rate was 3.9%; one death occurred in an emergency department. The risk of suffering falls was higher among boys, except among children under one year of age. Most frequently injured by falls were the groups of one-year-olds (46 per 1000 children) and two-year-olds (38.4 per 1000 children). The most affected body region was head/neck (55.2%) and superficial trauma was the most frequent kind of injury (46.4%). Falls occurred mainly from bed/cradle, furniture/couch, tricycle/skates, staircase/steps, playground equipment, wall/gate/roof/balcony, hammock, tree and stroller. Findings show a high incidence of falls among children and adolescents and contribute to increasing epidemiological knowledge of such events, thereby assisting in the planning of preventive and control measures.
