Primary cutaneous and skeletal muscle lymphomas are particularly rare phenomena in HIV sero-positive patients. The authors report a case of primary extranodal malignant diffuse large B cell lymphoma of the left gastrocnemius in a 21-year-old young adult who presented to the Infectious Diseases Institute, Kampala, Uganda having been on antiretroviral therapy for 2 years. The patient is still undergoing chemotherapy.

Abscess formation in the thyroid gland is particularly a rare phenomenon and presents a medical and surgical dilemma especially in HIV seropositive patients. The authors report a case of a paradoxical tuberculous abscess immune reconstitution inflammatory syndrome involving the thyroid gland in a 37-year-old woman who presented 12 weeks after initiation of antiretroviral therapy at the Infectious Diseases Institute, Kampala, Uganda. The patient is still undergoing treatment.

The effectiveness of combination antiretroviral therapy (cART) continues to improve as treatment choices expand with the development of new antiretroviral agents and regimens. However, the successful long-term treatment of HIV/AIDS is under threat from the emergence of drug-resistant strains to multiple agents and entire drug classes.

HIV-1 treatment-failure patients are increasingly being initiated on second-line antiretroviral therapy. The case we describe is of a treatment-failure patient who developed intractable chronic vulval ulceration presenting as immune reconstitution inflammatory syndrome (IRIS), following complete viral suppression with second-line highly active antiretroviral treatment (HAART). To the best of our knowledge, this is the first reported case of intractable vulval ulceration IRIS in an HIV-1 treatment-failure patient.
