Results of biplanar chevron osteotomy performed on patients with mild-to-moderate hallux valgus deformity with an increased distal metatarsal articular angle (DMAA) are shown. The study included clinical data of 32 patients (54 feet) who had completed a 2-year follow-up, and radiological data of these 32 and other 29 patients (50 feet) for a total of 61 patients (104 feet, 53 right and 51 left). There were 59 females and two males with ages varying from 11 to 66 years. According to the AOFAS Hallux Rating, the preoperative average score (50) improved to 90 (average score after the surgery). The hallux valgus angle was improved from an average of 25° to 14°, the first intermetatarsal angle from 12° to 8° and the DMAA from 15° to 5°. At the end of treatment, 94% of patients were classified as having grade 0 or I sesamoid lateral sub-luxation. Given improvement in angles and 90% of patients satisfied with an average AOFAS postoperative score of 90, the technique seems indicated for treatment of symptomatic hallux valgus deformity with increased DMAA.

The results achieved with this technique after a mean follow-up of 8 years (85% excellent and good results) allow neoligamentplasty to be considered as a viable alternative to the classic procedures in the treatment of subtle intercuneiform and tarsometatarsal joint lesions.

Ankle arthritis is becoming more and more common. The search for solutions that preserve joint function has led to a new generation of prosthesis with three components and more degrees of freedom. This paper presents the results achieved in ten patients treated with the HINTEGRA Prosthesis (Integra, New Deal), a joint action of the Foot and Ankle Teams of DOT of Unifesp - Escola Paulista de Medicina and the School of Medicine of USP - Universidade de Sao Paulo. The ten patients (six women and four men), aged between 29 and 66 years, underwent a surgical procedure according to Hintermans technique, from January to June 2005. They were evaluated at pre-arranged intervals, and the data were submitted to statistical analysis. The surgery led to a significant improvement in ankle mobility. Radiological studies showed no signs of loosening or failure in the prosthetic components in any of the patients studied. Although the rate of complications in our sample was high, it was equivalent to those of other authors, and directly represents the long, steep learning curve associate with this kind of procedure. At the end of four years, there was a significant reduction in the pain, and an improvement in functional pattern of the patients, with AOFAS and Hinterman scores indicating 20% excellent, 70% good, and 10% poor results. Despite the drawbacks, treatment of ankle arthritis through total arthroplasty with the HINTEGRA prosthesis proved to be capable of giving good results over an average observation period of four years.

Stimulated by the growing interest in the intrinsic factors at the genesis of the deformity of the hallux valgus, the authors studied the distal articular angle of the first metatarsal (DMAA), the proximal articular (PFPAA), and the distal articular of the proximal phalanx of the hallux (PFDAA). To do so, they evaluated the AP incidence of 70 grownup and normal Brazilian individual (140 feet) in orthostatic position with ages ranging from 18 to 55 years (29 in the average), 40 of which (57.1%) were female and 30 (42.9%) were male. The statistical analysis to wich the data were submitted pointed out as upper limts of normality, for the population studied, the following figures: 8 degrees for DMAA, 8 degrees for PFPAA and 10 degrees for PFDAA.

The ten patients (six women and four men, aged between 29 and 66 years), underwent a surgical procedure consisting of Hintermanns technique, between January and June 2005. They were evaluated at prearranged intervals, and the data were subjected to statistical analysis. The surgery led to a significant improvement in ankle mobility. Radiological evaluation showed no signs of loosening or failure in the prosthetic components in any of the patients studied. Although the complication rate in our sample was high, it was equivalent to the rates found by other authors, and directly represents the learning curve associate with this kind of procedure. Four years after the procedure, it was found that the patients pain levels had significantly decreased, and that their functional patterns had significantly improved, with AOFAS and Hintermann scores indicating results that were excellent for 20%, good for 70% and poor for 10%. Treatment of ankle arthritis by means of total arthroplasty using the HINTEGRA prosthesis was capable of providing good results over an average observation period of four years.
