Human Infections Due to Streptococcus dysgalactiae Subspecies equisimilis

Human streptococci that belong to Streptococcus dysgalactiae subspecies equisimilis (SDSE) have long been known under the name of beta-hemolytic groups C and G streptococci. Extensive taxonomic studies during the past years have distinguished most of the veterinary pathogens belonging to Lancefield groups C and G from those of human origin. After being considered nonpathogenic for many years, SDSE is now recognized as an important bacterial pathogen. The clinical spectrum of diseases caused by this species closely resembles Streptococcus pyogenes infections, including the occurrence of poststreptococcal sequelae. In accordance with these observations, many of the virulence factors present in S. pyogenes can also be found in SDSE strains. High nucleotide-sequence identities in virulence genes and the association of these genes with mobile genetic elements support the hypothesis of extensive horizontal gene-transfer events among streptococcal species of the pyogenic group. Recent epidemiological studies have shown increasing numbers of invasive SDSE infections, often among immunocompromised patients, and suggest that this species will probably gain even more clinical importance in the near future. For a better understanding of the changing epidemiology and pathogenicity of SDSE, an increased awareness of these microorganisms as human pathogens and proper identification are mandatory.

Mehr als nur eine Begatellverletzung

Nach einer fraglichen Bagatellverletzung im Bereich der Groszehe bei der Gartenarbeit kommt es bei einem 58-jahrigen Patienten innerhalb von 24 Stunden zu einer ausgepragten Schmerzsymptomatik im linken Unterschenkel sowie plotzlichem Fieber mit Schuttelfrost. Bei Aufnahme in der chirurgischen Klinik zeigten sich eine etwa doppelt handtellergrose livide Hautverfarbung mit tastbarem Hautemphysem des Unterschenkels bis zum distalen Drittel des Oberschenkels sowie ein Kompartmentsyndrom des Unterschenkels (⊡ Abb. 27.1). Innerhalb von einer Stunde nach Aufnahme verstarkt sich die Symptomatik mit ausgepragter Blasenbildung am ventralen Unterschenkel (⊡ Abb. 27.2). Klinisch fallen eine Tachykardie und eine diskrete Schweisneigung auf. Anamnestisch sind bei dem Patienten auser einer Tachyarrhythmia absoluta und einem Insulin-pflichtigen Diabetes mellitus Typ 2 keine weiteren Vorerkrankungen bekannt.

Der vergessene „blutliebende“ Feind

Ein fast drei Monate alter weiblicher Saugling wird in stark reduziertem Allgemeinzustand mit septischem Krankheitsbild nach einem Krampfanfall von ca. 10 bis 15 Minuten Dauer vom Notarzt in eine Kinderklinik eingewiesen.

Und er hat’s doch im Blut

Bei einem 22-jahrigen homosexuellen Mann wird im Rahmen der Abklarung eines akuten fieberhaften Beschwerdebilds mit Bronchitis, eitriger Infektion des linken Auges, Haarleukoplakie und Mundsoor die Erstdiagnose einer bereits in das Stadium AIDS fortgeschrittenen HIV-Infektion gestellt (HIV-PCR 356.000 Genomaquivalente/ ml, 26 CD4-positive T-Lymphozyten/μl, CD4/CD8 0,05; ein funf Jahre zuvor durchgefuhrter HIV-Test war negativ gewesen). Es wird eine antiretrovirale Therapie und die prophylaktische Gabe von Cotrimoxazol zur Prophylaxe einer Pneumocystisjiroveci-Pneumonie begonnen. Etwa einen Monat spater stellt sich der Patient erneut wegen anhaltendem Fieber und Schmerzen in beiden Unterschenkeln bei seinem behandelnden Arzt vor. Die laborchemischen Untersuchungen ergeben auser einer zunehmenden Anamie (Hb-Abfall in diesem Zeitintervall von 11,0 g/dl auf 8,7 g/dl) keine weiteren Auffalligkeiten.

Weichteilemphysem ungewöhnlichen Ausmaßes

Streptococcus agalactiae, known as a pathogen that causes meningitis and septicemia in neonates, emerges as an invasive organism in nonpregnant adults. This case report describes the fulminant course of a necrotizing fasciitis (NF) with streptococcal toxic shock-like syndrome (STSS) in a 76-year-old diabetic patient caused by S. agalactiae, serotype V. Chronic diseases and immunodeficiency are considered to be risk factors for the acquisition of group B streptococcal disease. Since early surgical treatment in conjunction with antimicrobial and intensive care therapy is critical for the outcome of patients with NF and/or STSS, clinicians should be aware of invasive S. agalactiae infections in adults with subcutaneous emphysema.
