UNLABELLED Admission to the Intensive Care Units (ICU) is perceived by both patients and their family as a stressful experience. This situation can cause shock, skepticism and anxiety in the patients setting. OBJECTIVE To analyze and present the needs of the family members of the critical patient. METHODOLOGY A review was made of the literature in the following data bases: Pubmed, CINAHL, Cuiden and Cochrane Library. Furthermore, three journals specialized in Intensive Cares were reviewed. RESULTS After including the articles that met the screening criteria, 30 articles were finally selected. Of these, 11 were reviews of the literature, 7 qualitative studies and 12 more were quantitative investigations. Four groups of needs in the families were identified after the analysis of these articles, these being cognitive, emotional, social and practical. DISCUSSION One of the main needs of the family is to receive better information from the professionals followed by the need for closeness with the ill family member. This review proposes that, in order to cover these needs, the information should be protocolized and the visiting hours should be made flexible. Furthermore, incorporation of two familial evaluation tools, the genogram and ecomap, is proposed. CONCLUSIONS This work has revealed the importance of having the Intensive Care nurses identify the needs of the family during the admission of the patient in the ICU in order to be able to provide help, support and counseling care that relieves the suffering of the families.

The aim of the article is to show the psychosocial impact of breast cancer in the long-term survival and to promote the idea of implementing a follow-up plan for survivors. A narrative review of published articles on the experience of survival in breast cancer survivors was conducted. Data were organised according to the domains of measurement (physical, psychological and social) of the Quality of Life of Long-term Breast Cancer Survivors Scale (LTQOL-BC). Breast cancer survivors may feel emotionally affected by the physical sequels (mastectomy, lymphedema, early menopause, and infertility), psychological (fear of recurrence and emotional stress) and/or social (family relationships and employment changes). The data are the foundation of a proposal of specific follow-up care for breast cancer survivors.

La detección temprana del cáncer ası́ como los avances médicos y terapéuticos han tenido efectos beneficiosos en la supervivencia del cáncer. Se estima que hay unos 24,6 millones de supervivientes de cáncer en el mundo. Otros factores, como la edad en el diagnóstico, el tipo de cáncer, el estadio de la enfermedad en el diagnóstico y el acceso a los servicios de salud también han influido en el aumento de la supervivencia. Considerando el aumento de la supervivencia del cáncer, el proyecto EUROCARE (European Cancer Registry Study on Survival and Care of Cancer Patients) se estableció en 1989 para medir y explicar las diferencias en la supervivencia del cáncer en Europa. Desde entonces, se han publicado 4 informes sobre la supervivencia del paciente con cáncer en la población europea, desde 1978 hasta 2003. El último proyecto, el estudio EUROCARE-4, llevó a cabo un análisis global de la supervivencia por localizaciones, sexos y grupos de edad, y analizaron los datos de casi 3 millones de personas diagnosticadas de cáncer entre 1995–1999 y seguidos hasta diciembre de 2003, en 23 paı́ses europeos. Los datos de supervivencia global, tanto en hombres como en mujeres, indican un aumento de la supervivencia a los 5 años tras el diagnóstico para la mayorı́a de los tumores, y se espera que esta tendencia continúe en los próximos años. OBJECTIVE To examine the experiences and responses of nurses caring for patients with recurrence of cancer. MATERIAL AND METHODS A qualitative cross-sectional study based on grounded theory. Data was collected, based on semi-structured interviews carried out in four cancer units of two hospitals in Navarra, after obtaining ethical approval. The sample consisted of 14 nurses. RESULTS Providing care during cancer recurrence is described as a difficult situation, even more than in the first diagnosis of cancer. This is because nurses have to deal with the loss of patient hope in treatment and the fear of the possibility of the patients death. To deal with this reality, nurses try to do their best to meet the emotional needs of the patient, at the same time as managing barriers including lack of time, workload, and inadequate training in psycho-oncology. CONCLUSIONS The data show the difficulty nurses have in dealing with the emotional care of patients with recurrence. If the Health Service commitment is to offer holistic care to cancer patients and their families, there should be a corresponding commitment towards improving nursing training in psychosocial issues.

El principal objetivo de este ensayo es evidenciar la importancia de fomentar estudios con supervivientes a largo plazo de cancer de mama, al mismo tiempo que se introduce el concepto de supervivencia a largo plazo en el contexto de la investigacion enfermera espanola. El cuerpo de este articulo se organiza en tres apartados que examinan estudios sobre la calidad de vida en mujeres supervivientes al cancer de mama. Con esta revision se demuestra que la calidad de vida de estas mujeres difiere de la de las mujeres sanas. Por lo tanto, se concluye que es importante promover la investigacion en Espana en este ambito de la enfermeria oncologica con el fin de ofrecer una atencion de calidad a esta poblacion creciente.

El progresivo envejecimiento de la poblacion espanola conlleva un aumento de la atencion sanitaria, por los servicios de urgencias extrahospitalarias (SUE), a este grupo de edad. El objetivo de este trabajo es evidenciar la importancia de realizar una valoracion global del anciano en el contexto domiciliario y comunitario, que contemple tanto los aspectos fisicos como los psicosociales y ambientales de la persona en situacion de urgencia. Para ello, se desarrolla un caso clinico, que se apoya en la evidencia cientifica, para determinar la actuacion mas adecuada que mejore las intervenciones de los servicios de urgencias extrahospitalarias. El analisis del caso se estructura en tres apartados: la valoracion de enfermeria, el registro de los datos y la transmision de la informacion a otros equipos de salud. Como conclusion, se identifica la necesidad de valorar y registrar datos sobre el estado fisico y fisiologico del anciano al mismo tiempo que se recoge informacion sobre su contexto familiar, comunitario o ambiental, para proporcionar unos cuidados integrales y de calidad. Asimismo, se recomienda el desarrollo de herramientas de valoracion geriatrica especificas para utilizar en situaciones de urgencias extrahospitalarias.
