Significant changes in blood pressure, glucose, and lipids with gastric bypass surgery

Abstract. The morbidly obese have a disproportionately greater risk of hypertension, diabetes, and coronary artery disease than their lean or less seriously obese counterparts. Roux-en-Y gastric bypass surgery has been found to be highly effective in inducing, and sustaining, weight loss in individuals with morbid obesity. The purpose of the present study was to examine the effects of weight loss with Roux-en-Y gastric bypass surgery (GBP) on blood pressure, fasting blood glucose, and the lipid/lipoprotein status of 61 morbidly obese women and 21 men. Anthropometric and blood pressure assessments and blood samples for glucose and lipid/lipoprotein analyses were obtained before surgery and at 6 to 12 months postoperatively. By this time, morbidly obese (MO) males and females had lost 33% and 30% of their initial body weight, respectively, along with significant reductions in fasting blood glucose (p < 0.01) and systemic blood pressure (p < 0.05). Weight loss with GBP was also associated with significant reductions in the apoprotein B-containing lipoproteins and the triglyceride and cholesterol composition of these particles. There was a trend (p < 0.10) toward increased serum levels of high density lipoprotein (HDL)-cholesterol following GBP, and significant (p < 0.05) improvement in HDL subfraction distribution and composition. These findings demonstrate the effectiveness of GBP in inducing metabolic changes in the MO population, which may reduce the risk of coronary artery disease, diabetes, and hypertension.

Taste Acuity of the Morbidly Obese before and after Gastric Bypass Surgery.

Obese individuals have an increased preference for high caloric foods, such as sweets and fats. However, following gastric bypass (GBP) surgery, morbidly obese patients tend to avoid these foods. We hypothesize that this aversion may occur, in part, from permutations in taste acuity. To test this hypothesis, taste detection and recognition thresholds for the four basic tastes (salt, sweet, sour, and bitter) were assessed using a modification of the Henkin forced choice three stimulus technique. Taste acuity measurements were obtained at baseline and at 30, 60, and 90 days post-operative for six morbidly obese GBP women and ten non-surgical, lean female controls. We found non-significant differences in taste detection and recognition thresholds between morbidly obese and lean control study subjects at baseline, and no significant correlation between taste acuity and body size. Furthermore, in our study population of lean and obese women, ages 26 to 52, there were no significant interrelationships between baseline taste thresholds and known effectors of taste acuity, i.e., zinc levels, glycemic status, liver and kidney function, or age. Following GBP surgery, a significant up-regulation in taste acuity for bitter and sour was observed along with a trend toward a reduction in salt and sweet detection and recognition thresholds. These findings would suggest the following: (1) taste acuity does not influence taste preferences of the obese individual who has not had bariatric surgery; (2) taste effectors such as zinc, when within the range of normal values, do not alter thresholds of the 4 basic tastes; and (3) weight loss following gastric bypass surgery is associated with an up-regulation in taste acuity in the morbidly obese. Studies are currently under investigation at our center to identify the specific etiology of taste acuity upregulation in the morbidly obese following GBP surgery.

Dietary Protein Intake and Bariatric Surgery Patients: A Review

Bariatric surgery, a highly successful treatment for obesity, requires adherence to special dietary recommendations to insure the achievement of weight loss goals and weight maintenance. Postoperative consumption of protein is linked to satiety induction, nutritional status, and weight loss. Hence, we conducted an extensive literature review to identify studies focused on the following: protein and nutritional status; recommendations for dietary protein intake; the effects of protein-rich diets; and associations between dietary protein intake and satiety, weight loss, and body composition. We found that there have been few studies on protein intake recommendations for bariatric patients. Dietary protein ingestion among this population tends to be inadequate, potentially leading to a loss of lean body mass, reduced metabolic rates, and physiological damage. Conversely, a protein-rich diet can lead to increased satiety, enhanced weight loss, and improved body composition. The quality and composition of protein sources are also very important, particularly with respect to the quantity of leucine, which helps to maintain muscle mass, and thus is particularly important for this patient group. Randomized studies among bariatric surgery patient populations are necessary to establish the exact quantity of protein that should be prescribed to maintain their nutritional status.

Ethnic differences in obesity and surgical weight loss between African-American and caucasian females

Background: In the general population, African-American females are more obese and resistant to weight loss than Caucasian women. In the present study, we examined the severity of obesity among morbidly obese African-American and Caucasian females, studied the effectiveness of Roux-en-Y gastric bypass (RYGBP), and sought to identify factors contributing to obesity and weight loss. Methods: The study population included 153 morbidly obese females randomly selected from our general bariatric patient population. Anthropometric measurements consisted of body weight, body mass index (BMI), excess weight, and waist, hip, thigh, and neck circumferences. Factors that may contribute to obesity included age, age of obesity onset, number of childbirths, calorie intake, diet composition, and degree of psychological distress. The effects of RYBGP were studied in weight-matched groups of African-American and Caucasian females (n=37 per group) at weight loss nadir, i.e. 12 to 18 months after surgery. Results: We found that morbid obesity is more severe among African-American than Caucasian females. The greater degree of obesity of African-American, as compared to Caucasian, females is not due to ethnic differences in calorie intake, diet composition, age or age of obesity onset, number of childbirths, and psychological distress. RYGBP is less effective in reducing body fat and, consequently, excess body weight of the African-American than the Caucasian females, suggesting possible ethnic differences in fat metabolism. Conclusion: African-American females with morbid obesity have greater adiposity than do Caucasian women and lose significantly less body fat after RYGBP.

Amelioration of Insulin Resistance in Diabetes With Dehydroepiandrosterone

In hyperandrogenic females, the ratio of dehydroepiandrosterone (DHEA) to testosterone may be an important determinant of insulin sensitivity. This study involved changes in insulin sensitivity and glucose metabolism with therapeutic manipulation of DHEA (S)/testosterone in a female patient with non-insulin-dependent diabetes and hyperandrogenism. Therapeutic intervention included 1-month treatment with 0.25 mg dexamethasone at bedtime and 1-month dexamethasone + DHEA. Insulin sensitivity and glucose tolerance were assessed before and after each treatment regimen by examining: 1) fasting and oral glucose tolerance test glucose and insulin levels, 2) hypoglycemic response to intravenous insulin, and 3) erythrocyte insulin receptor binding. With dexamethasone alone, DHEAS, testosterone, and their ratio were reduced with a concomitant increase (30%) in oral glucose tolerance test insulin levels and a decrease (33%) in erythrocyte insulin binding. With DHEA + dexamethasone, the ratio of DHEAS/testosterone increased 16-fold along with a marked improvement in insulin sensitivity, as determined by a more than 30% reduction in fasting and oral glucose tolerance test insulin levels, a threefold stimulation of the rate of glucose disappearance with intravenous insulin, and a 30% increase in insulin binding. DHEA improved insulin sensitivity and reduced fasting and oral glucose tolerance test glucose levels and ameliorated the diabetic state. The ratio of DHEAS/testosterone is an important regulator of insulin sensitivity and glucose tolerance and that DHEA therapy may be beneficial in the treatment of certain forms of insulin resistance.

Significant Changes in the Lipid-Lipoprotein Status of Premenopausal Morbidly Obese Females following Gastric Bypass Surgery

The morbidly obese premenopausal female may be more dyslipoproteinemic and at greater risk for developing coronary heart disease than her lean or less seriously obese counterparts. The purpose of the present study was to examine the effects of weight loss with Roux-en-Y gastric bypass surgery on the lipid-lipoprotein status of morbidly obese, premenopausal females. Anthropometrics and blood samples for lipid-lipoprotein analyses were obtained before surgery and 6 - 12 months post-operatively. Following surgery, patients lost 30% of their initial body weight, along with a 40% decline (p < 0.01) in total triglyceride and a 20% decrease (p < 0.01) in total cholesterol. Levels of cholesterol in the high density lipoprotein (HDL) fraction were unaffected by weight loss, but there was a significant (p < 0.05) increase in the proportion of HDL in its more buoyant and anti-atherogenic form, i.e. HDL-L. The apolipoprotein B-containing lipoproteins, very low density lipoprotein (VLDL), intermediate density lipoprotein (IDL), and low density lipoprotein (LDL), were reduced up to 70% following surgery. There were no significant changes in VLDL or IDL particle composition, i.e. cholesterol/triglyceride, cholesterol/protein, but there was a significant (p < 0.01) increase in the ratio of cholesterol/apolipoprotein B in LDL, suggesting a shift from the small, dense atherogenic LDL to a larger, less atherogenic particle. We conclude that weight loss following gastric bypass surgery markedly improves the lipid-lipoprotein status of morbidly obese premenopausal females and, thereby, significantly reduces the risk of coronary disease.

Evidence for a defect in insulin metabolism in hyperandrogenic women with polycystic ovarian syndrome

It has been well established that the hypertestosteronemia of patients with polycystic ovarian syndrome (PCO) is associated with hyperinsulinemia and insulin resistance. We have recently noted a disparity between serum levels of insulin and C-peptide in certain hypertestosteronemic women with PCO and hypothesized a possible association between testosterone and insulin metabolism. Therefore, we have studied insulin clearance (baseline steady-state ratios of C-peptide to insulin) in 15 obese PCO women, 12 weight-matched controls (OC), and nine lean controls (LC), and examined the interactions of testosterone and insulin metabolism by examining the correlations between testosterone and insulin clearance and by studying the direct in vitro actions of testosterone on T-lymphocyte insulin binding and degradation. We found that the C-peptide to insulin ratio at baseline and T-lymphocyte insulin degradation of the PCO group were twofold below the LC and OC values. Basal C-peptide to insulin ratios and insulin-degradative activities were significantly and negatively interrelated (r = .56, P < .01), and both of these parameters were highly correlated (P < .01) with basal testosterone levels (r = .49 for basal C-peptide to insulin and r = -.61 for insulin degradation). In experiments where testosterone was added to cell cultures, insulin degradation was impaired in a biphasic fashion. We conclude that (1) elevated testosterone levels may contribute to impairments in insulin metabolism, and (2) the hyperinsulinemia of hyperandrogenic women may occur in part from defects in insulin clearance and peripheral tissue insulin degradation.

Diet-induced thermogenesis and respiratory quotient after Roux-en-Y gastric bypass

BACKGROUND Roux-en-Y gastric bypass (RYGB) surgery is an effective tool for long-term weight loss. Mechanisms underlying the effectiveness of such surgery might result not only from the anatomic changes due to the procedure, but also from favorable changes in energy metabolism. Our objective was to evaluate the respiratory quotient (RQ), resting metabolic rate (RMR), and diet-induced thermogenesis (DIT) among clinically severe obese patients (control group) and patients who had undergone RYGB ≥ 1 year previously. The setting was Gastrocirurgia de Brasilia (Brasilia, Brazil). METHODS The present study was cross-sectional and involved 35 clinically severe obese patients (body mass index ≥ 40 kg/m(2) or body mass index ≥ 35 kg/m(2) with co-morbidities) as the control group and 34 RYGB patients who had undergone the procedure ≥ 12 months previously (RYGB group). The anthropometric data (height and weight) were determined for both groups, and the RMR and RQ were measured using indirect calorimetry after a 12-hour fast. Patients then received a standard meal, and DIT was determined. The RMR and DIT were also adjusted per kilogram of body weight (BW), i.e BW-adjusted RMR and BW-adjusted DI. RESULTS The BW-adjusted RMR and RQ did not differ between the 2 groups in the fasting period. However, the DIT of the RYGB group, whether absolute or BW-adjusted, was >200% that of the control group (P <.0001). The BW-adjusted DIT of the RYGB group correlated significantly with the percentage of excess weight loss (P = .0097). The postprandial RQ value among the RYGB group was also significantly (P <.0001) greater than that of the control group, suggesting an increased use of carbohydrates. CONCLUSION Postprandial changes in energy expenditure and fuel use might contribute, in part, to the effectiveness of weight loss as a result of the RYGB procedure.

Phytohemagglutini (PHA) activated human T-lymphocytes: Concomitant appearance of insulin binding, degradation and insulin-mediated activation of pyruvate dehydrogenase (PDH)

Binding and degradation of A14125I-Insulin as well as the effect of insulin on pyruvate dehydrogenase (PDH) activation were studied in non-stimulated and phytohemagglutinin (PHA)-stimulated thymic-derived lymphocytes (T-lymphocytes) of man under varying conditions of time, temperature, and cell concentration. The nonstimulated viable T-lymphocytes exhibited neither binding, degradation, nor PDH activation in response to insulin. With PHA stimulation, a time and temperature-dependent binding was noted in T-lymphocytes which paralleled the appearance of cell-associated insulin degrading activity. Concomitant with the emergence of insulin binding and degrading activities in these cells, PDH activation was observed which was responsive to as little as 5.0 microU/ml of insulin. We conclude that in PHA-activated T-lymphocytes of man the process of insulin binding and degradation is closely related to insulin sensitive activation of PDH. These activated cells may serve as a useful model in which to study insulin binding and processing, as well as effects of insulin on postreceptor events.
