Purpose – This paper seeks to review current approaches to learning from health and safety incidents in the workplace. The aim of the paper is to identify the diversity of approaches and analyse them in terms of learning aspects.Design/methodology/approach – A literature review was conducted searching for terms incident/accident/near misses/disaster/crisis modified with learning/training and safety. Shortlisted articles were analysed by questioning who is learning, what kind of learning process is undertaken, what type of knowledge is employed and the type of problem that these incidents addressed. Current approaches to learning from incidents were critically analysed and gaps identified.Findings – Very few papers addressed all the envisaged aspects when developing their learning from incidents approaches. We propose an approach to enhancing knowledge sharing and connectedness in distributed teams. Termed ‘Narrating Your Work’ (NYW), the approach involves members of distributed team using a microblogging tool to post regular updates about their current work, accomplishments, and issues. The NYW approach was evaluated within a geographically and temporally distributed team at Shell International for a period of one month, using a mixed-method research design. Methodology comprised of a quantitative survey, followed by semi-structured interviews and analysis of microblogging updates posted during the month in which the approach was being trialled. The evaluation results suggest that NYW was viewed as a valid and practical approach to enhancing knowledge sharing and connectedness. A range of barriers and enablers that could impact the future application and embedding of the approach are identified and recommendations for implementation are outlined.

Abstract This paper describes the development and evaluation of a toolkit to support organizations in improving their learning from incidents (LFI) activities. Grounded in adult learning theory, extensive literature review and empirical research within the energy sector, the LFI Toolkit has five key components: a Process Model; a Framework; a Questionnaire; a set of Guidelines and a series of Engagement Exercises. The LFI Toolkit fosters participatory learning enabling broader employee engagement, sensemaking and contextualization. The Toolkit was developed and evaluated through participatory co-design methodology including two large energy companies. The data were drawn from four participatory co-design workshops and a stakeholder engagement review meeting including practitioners from a variety of organizations and roles – shop floor workers, frontline and middle managers, senior leaders, health and safety specialists and representatives of professional bodies. The findings provide insight into the clarity, usability and relevance of the Toolkit and the feasibility of its application across other companies.
