Acceptance of an intended smoking ban in an alcohol dependence clinic

Alcohol treatment professionals are often reluctant to address tobacco dependence in their patients or to implement smoke-free policies in inpatient treatment programs, fearing, among others, non-adherence to alcohol treatment. The aim of the present study was to evaluate the acceptance of an intended smoking ban in a specialized hospital for alcohol withdrawal. Fifteen of 54 patients reported that they would not begin or quit alcohol treatment if smoking were banned in the clinic, but only five would not begin or quit if nicotine replacement were available. The present study indicates that a non-smoking policy would be feasible in a Swiss alcohol clinic, without jeopardizing alcohol treatment adherence.

Le trouble d'utilisation de substances dans le film Limitless

Le film Limitless est un thriller américain sorti en 2011 et réalisé par Neil Burger. Il s’agit de l’adaptation du roman «The Dark Fields» d’Alan Glynn (2001). Le film s’inscrit dans un contexte contemporain prenant place à New York et plus précisément dans l’arrondissement de Manhattan. Eddie, le personnage principal du film, souffre d’un trouble de l’usage d’une substance autre selon le DSM-5. Le NZT, substance fictive, est une molécule qui augmente considérablement les capacités cognitives, permettant à son usager d’avoir une capacité de réflexion et d’apprentissage hors norme. Eddie en devient vite dépendant. Loin de le mener à la marginalisation, le NZT le rend cependant extrêmement brillant et performant en société. Il est à ce point intéressant de noter que le tableau clinique présenté par Eddie va plutôt à l’encontre des critères du DSM-5 puisqu’au lieu de l’entraver dans ses activités, cette substance nootropique lui permet au contraire d’exceller dans celles-ci. La présence de plus de six critères diagnostiques, dont celui du sevrage, permet néanmoins de faire le diagnostic. Le critère de tolérance reste difficile à déterminer Correspondance: Dr med. Les troubles bipolaires dans le film Touched with fire

C’est l’histoire de Carla (Katie Holmes) et Marco (Luke Kirby) qui est présentée dans ce film. Ou plutôt les histoires, devrait-on dire... . L’histoire de leur relation, l’histoire de la relation à leur maladie psychique ainsi qu’aux personnes et aux institutions qui les soignent. Ils souffrent tous les deux d’un trouble bipolaire. Ils se rencontrent le jour de leur internement à l’hôpital psychiatrique. Deux éléments les rapprochent: un qu’ils n’ont pas choisi, le trouble bipolaire. Et un autre qui est une compétence qu’ils ont développé, la poésie. Leur relation évolue et ils tombent amoureux malgré la désapprobation de leurs proches. Face à cette désapprobation, les deux protagonistes prennent ensemble la décision d’arrêter tout traitement. Plus tard, suite à un accident de voiture, Carla se retrouve à l’hôpital et apprend qu’elle est enceinte. Elle accepte de reprendre son traitement stabilisateur d’humeur. Parallèlement, sa relation avec Marco se dégrade, car celui-ci fait une décompensation maniaque et refuse toute médication. Carla décide alors d’avorter. Marco n’accepte pas cette décision et ils finissent par se séparer. Le film Touched with fire a été réalisé en 2015 par Paul Dalio qui est lui-même atteint du trouble bipolaire. Son expérience de la maladie a influencé les deux personnages principaux. Le film a notamment été inspiré du roman Touched with fire de Kay Redfield Jamison, publié en 1993, elle aussi atteinte du trouble. The efficiency of a carbamazepine-mianserine combination scheme in opiate detoxification

A recent comparative randomized double-blind study has suggested the utility of a carbamazepine/mianserin combination as a treatment for opiate withdrawal. The aim of the present study was to explore the feasibility and efficiency of this combination under naturalistic conditions. Five hundred and fifty mostly polysubstance abusing patients treated with a standardized scheme combining carbamazepine and mianserin were assessed with regard to deviations to the protocol, used co-medications and retention in treatment. Three hundred and sixty three patients (66.0%) received the carbamazepine/mianserin combination as specified by the standardized protocol. In 350 patients (63.7%) the whole 10 days was completed. The most frequently used p.r.n. medications were for anxiety (47.5%) and insomnia (54.5%). The treatment of opiate withdrawal with a carbamazepine/mianserin combination scheme in an inpatient setting seems to be feasible and applicable with few adaptations to most patients, and may represent an interesting treatment option for multidrug users.
