Developing an architecture of a knowledge-based electronic patient record

Medicine as knowledge-intensive domain has been the subject of various approaches of computer-based knowledge management. Most of them concentrated on the design and implementation of expert systems for clinical decision support. Today, medical knowledge bases are implemented for various purposes, including encyclopedic sources of information for clinicians. We present a prototypical development of architecture for an electronic patient record which structurally depends on such an encyclopedic representation and is therefore knowledge-based. Using the KADS approach for knowledge engineering, three modeling steps and architectural parts could be identified, definition of basic concepts, the structural knowledge base model, and the interactive process of knowledge instantiation which constitutes clinical documentation. Furthermore, we present an analysis of possible benefits of a knowledge-based electronic patient record in health care as well as in adjacent fields.

Management of Knowledge-Intensive Healthcare Processes on the Example of General Medical Documentation

Healthcare Processes are characterized by knowledge-intensive tasks. In contrast to this, most of the efforts for business process management in healthcare do not refer to this quality, and software engineering in healthcare relies on an unspecific process-oriented approach. In this contribution, we present a method of capturing and analysis of a knowledge-intensive process, from which we derive requirements to a knowledge- as well as process-oriented information system for the example of general medical documentation. We present the resulting implementation of a knowledge-based electronic patient record and discuss the potentials and open issues for our proposal.

An IHE-conform telecooperation platform supporting the treatment of dementia patients

Abstract Ensuring medical support of patients of advanced age in rural areas is a major challenge. Moreover, the number of registered doctors—medical specialists in particular—will decrease in such areas over the next years. These unmet medical needs in combination with communication deficiencies among different types of health-care professionals pose threats to the quality of patient treatment. This work presents a novel solution combining telemedicine, telecooperation, and IHE profiles to tackle these challenges. We present a telecooperation platform that supports longitudinal electronic patient records and allows for intersectoral cooperation based on shared electronic medication charts and other documents. Furthermore, the conceived platform allows for an integration into the planned German telematics infrastructure.
